Cefaclor belongs to oral cephalosporins group, with a preferential action on gram-positive bacteria.

It is indicated for the treatment of the following infections caused by susceptible bacteria:

Ear infections (otitis media), nose (acute sinusitis), throat(pharyngoamygdalitis) and lung (pneumonia). Urinary tract infections (cystitis, pyelonephritis, and gonococcal urethritis). Skin and subcutaneous tissue infections.

Powder for suspension:

In addition to the active ingredient, the other ingredients are sucrose, xanthan gum, sodium lauryl sulfate, strawberry essence, erythrosine and colloidal silica.

Capsules:

Capsules include pregelatinized corn starch, anhydrous colloidal silica, croscarmellose sodium, magnesium stearate, gelatin, tartrazine, erythrosine, bright blue, titanium dioxide, lacquer, water, ammonium hydroxide, black iron oxide, and potassium hydroxide. The printing ink contains shellac, soya lecithin, black iron oxide, and simethicone emulsion.

Some brands of cefaclor contain sucrose, which should be considered in patients with hereditary fructose intolerance, poor absorption of glucose/galactose, sucrose-isomaltase deficiency, and diabetic patients.

Do not use cefaclor

If you are allergic (hypersensitive) to cefaclor, to any antibiotic in the cephalosporins and penicillins class or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Take special care with cefaclor

If you are allergic to penicillin, since you may also be allergic to other beta-lactam antibiotics such as cefaclor. If you have an allergic reaction to cefaclor, discontinue treatment and tell your doctor.

- If you have or have had some form of allergy, this drug should be administered with caution.
- There have been reports of pseudomembranous colitis with most broad-spectrum antibiotics, including cefaclor.

If any of these circumstances is applicable to you, tell your doctor before taking it.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any medications, even those without a prescription or herbal remedies.

The following drugs can modify activity of cefaclor:

- Probenecid inhibits renal excretion of cefaclor, delaying disposal of this in the body.
- Rarely, when administering simultaneously oral anticoagulants (acenocoumarol, warfarin) and cefaclor may increase the anticoagulant effect.
- Antacids, such as almagate and cimetidine, reduce the effect of cefaclor.

Cefaclor may interfere with certain laboratory tests.

Remember your doctor has prescribed this medicine only for you. Never give it to someone else.

Use with food and beverages

Oral absorption of cefaclor is the same when given with or without food.

Special physiological situations

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy and lactation

Have not been established the safety of this medicine during pregnancy. Cefaclor should be given only during pregnancy if clearly necessary.

Cefaclor is excreted in human milk, reason by which should not be administered to lactating women.

Children:

Have not been established safety and efficacy of cefaclor in children younger than one-month-old.

Driving and using machines

It is unlikely that cefaclor affects ability to drive and use machines.

Follow carefully the instructions of use of cefaclor provided by your doctor. Consult your doctor or pharmacist when in doubt. Cefaclor is administered orally. The form in powder for the extemporaneous solution should be invested gently tapping the canister until the powder is not adherent to walls. Add a little water and stir a few moments, completing the suspension with water up to the height of the arrow on the label and stir again until obtaining a homogeneous suspension.

Adults: Usual dose is 250 mg every 8 hours. For infections more severe or caused by less susceptible organisms, doses may be doubled (500 mg) every 8 hours. Total daily dose should not exceed 4 g.

For treatment of acute gonococcal urethritis in men and women, it is recommended a single dose of 3 g of cefaclor with 1 g probenecid.

Children:

The usual recommended dose is 20 mg/kg/day, divided into 3 doses a day.

In more severe infections, otitis media, sinusitis and infections caused by less susceptible organisms, 40 mg/kg/day is recommended, divided into 3 doses a day. Total daily dose should not exceed 1 gram.

Optional treatment twice daily:

For the treatment of otitis media and pharyngitis, the total daily dose may be divided and administered every 12 hours.

Impaired renal function:

An adjustment of the dose of cefaclor in patients with moderate or severe impairment of renal function is not required.

The usual duration of treatment is 7 to 14 days.

In the treatment of infections by beta-hemolytic streptococci, sinusitis and infections by S. pyogenes, therapeutical doses must be administered, at least, during 10 days.

If you feel unwell during treatment with cefaclor, immediately consult your doctor.

If you take more than you should:

If you take more cefaclor than you should, it can occur nausea, vomiting, epigastric discomfort, diarrhea, and seizures. You may need medical attention.

If you forget to take cefaclor

Take your dose as soon as possible but do not take a double dose to make up for missed doses.

Like all medicines, cefaclor can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

5-50% of patients experiences some sort of adverse effect, but only 2-10% of patients has effects of a certain intensity.

Commonest side effects include allergic disorders (rashes, itching, fever, eosinophilia), gastrointestinal (diarrhea, nausea, vomiting) and genitourinary (vaginitis, vaginal candidiasis).

Rarely Have been reported other side effects such as allergic reactions or other serious hypersensitivity reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis, and anaphylaxis). As with other penicillins and cephalosporins, rarely have been reported transient hepatitis and yellowing of the skin (jaundice) by bile flow problems (cholestatic jaundice). Rarely has appeared reversible kidney inflammation (reversible interstitial nephritis).

During the treatment with cefaclor have been reported slight increases in liver enzymes (AST, ALT, and alkaline phosphatase) and disorders of blood cells (eosinophilia and, rarely, thrombocytopenia).
