Indications and usage     

Gemfibrozil belongs to a class of lipid-lowering drugs used to lower cholesterol and other blood fats.

This medication is used along with diet and other non-pharmacological treatments (exercise, weight loss) in:

-Treatment of hypercholesterolemia (abnormally high levels of cholesterol in the blood) and/or hypertriglyceridemia (abnormally high levels of triglycerides in blood) in adults when other medicines to reduce cholesterol (statins) are inappropriate or not tolerated.
- Prevention of cardiovascular problems in male patients with high cholesterol (non-HDL cholesterol) and high risk for a first cardiovascular event, especially when other treatments (statins) are inappropriate or not tolerated.

In addition to the active ingredient gemfibrozil, other ingredients (excipients) are:

Core excipients: gluten-free corn starch, anhydrous colloidal silica (E551), polysorbate 80 (E-433), magnesium stearate (E-572), sodium carboxymethyl starch and silicon dioxide precipitate.

Coating excipients: Hypromellose (E464), titanium dioxide (E171) dioxide, macrogol 6000, simethicone (E-900) and talc (E-553 (b)).

Before taking the drug     

Do not use gemfibrozil

If you are allergic (hypersensitive) to the active substance gemfibrozil or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor take this medicine if:

- You have liver failure,
- You have severe kidney disease,
- You have a history or have had a disease of the gallbladder or bile ducts, including gallstones
- You are taking repaglinide (a medicine used to treat type 2 diabetes),
- You have a history of photosensitivity or phototoxicity reactions (skin changes when the sun is taken) during treatment with fibrates (other cholesterol medicines in the same family of gemfibrozil).

Take special care with gemfibrozil

- If you experience pain, tenderness or weakness of muscles. inform your doctor immediately. This risk is greater in patients taking gemfibrozil with other medicines that increase blood levels of gemfibrozil and therefore increases the risk of muscle disorders.
- Tell your doctor if you have kidney failure, hypothyroidism, if you are older than 70 years, if you have a family history or have previously suffered from muscular disorders or if you usually consume alcohol, as these factors can increase the risk of muscle disorders.
- If you are at risk of stone formation in the gallbladder.
- If you take hypoglycemic medications (to treat diabetes).
- If you take anticoagulant drugs (to prevent blood clots).

Consult your doctor if you are included in any of the above cases.

Your doctor may want to do blood tests or liver function tests to check your liver is working properly, before and during treatment with gemfibrozil.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

Do not take gemfibrozil while taking these medicines:

- Repaglinide

Certain medicines can interact with gemfibrozil; in these cases, it may be necessary to change the dose or stop treatment with any of them.

It is important to tell your doctor if you are taking or have recently taken any of the following medications:

- HMG-CoA reductase inhibitors (statins, drugs to treat elevated cholesterol levels in blood),
- Hypoglycemic drugs
- Rosiglitazone (medicine to treat type 2 diabetes)
- Anticoagulant drugs,
- Bexarotene (anticancer drug);
- Resins (medications to treat high cholesterol levels in the blood).

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Use with food and beverages

Is recommended to take the medicine half an hour before breakfast and dinner (if administered twice a day) or half an hour before dinner if administered in a single dose.

Special physiological situations

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy and lactation

It is not known if is safe using gemfibrozil in pregnant women, therefore this drug should be used during pregnancy only when, in the opinion of your doctor, the expected therapeutic benefits justify its use.

It is not known whether gemfibrozil is excreted in human milk, therefore it should not be used during lactation.

Children

It is not recommended for use in children.

Driving and using machines

There is no evidence that this drug impairs the ability to drive or operate machinery.

In isolated can occur cases of dizziness and visual disturbances that can negatively affect driving, so do not drive until you know how you tolerate the treatment.

How to use gemfibrozil     

Follow carefully the instructions of use of gemfibrozil provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Usually, the dose of gemfibrozil is 900 to 1200 mg per day.

The dosage regimen of 1200 mg is administered in two doses of 600 mg twice daily, half an hour before breakfast and dinner. The dose of 900 mg taken as single dose half an hour before dinner.

Remember to take your medicine. Your doctor will indicate the duration of treatment with gemfibrozil. Do not stop treatment early although you have started to improve.

Elderly patients (over 65 years): No dose adjustment is required in patients over 65.

If you think that the effect of the medicine is too strong or too weak, consult your doctor.

While you take gemfibrozil     

If you become pregnant, contact your doctor immediately.

If a doctor prescribes another medicine , tell you are taking gemfibrozil.

Check with your doctor regularly developments of disorder that motivates the use of gemfibrozil. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with gemfibrozil at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with gemfibrozil, immediately consult your doctor.

If you use more medicine than you should

If you have taken more gemfibrozil than should, consult your doctor or pharmacist immediately. In the case of overdose or accidental ingestion, contact the Poison Control Center in your country.

If you forget a dose:

Do not take a double dose to make up for missed doses.

Side Effects of gemfibrozil     

Like all medicines, gemfibrozil can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Adverse effects observed are:

Very common side effects (approximately 7% of patients): Indigestion (dyspepsia).

Common side effects (affect 1 in 10-100 patients): Abdominal pain, diarrhea, flatulence, nausea, vomiting, constipation, vertigo, headache, eczema, rash, and fatigue.

Uncommon side effects (affect 1 in 100-1000 patients): abnormal heart rhythm (atrial fibrillation).

Rare side effects (affect 1 in 1.000-10.000 patients): blood disorders, dizziness, drowsiness, tingling (paraesthesia), inflammation of nerves (peripheral neuritis), depression, decreased libido, blurred vision, inflammation of pancreas (pancreatitis), appendicitis, gallbladder obstruction (cholestatic jaundice), abnormal liver function, gallstones (cholelithiasis), inflammation of gallbladder (cholecystitis), rash (dermatitis, urticaria), itching (pruritus), hair loss (alopecia), joint pain (arthralgia), inflammation of joints (synovitis), muscle pain (myalgia, myopathy, myasthenia, myositis), pain in limbs, erectile dysfunction, allergic skin reaction to light (photosensitivity), allergic inflammation of the eyes and lips, which can also affect the hands, feet and throat (angioedema) and inflammation of the larynx (laryngeal edema).
