Indications and usage     

Itraconazole belongs to a group of medicines called antifungal imidazole.

This drug is indicated for the treatment of fungal infections of the vagina, skin, mouth, eyes, nails or internal organs.

Capsules

Each capsule contains as active ingredient itraconazole, 100 mg. The other ingredients are sugar spheres (sucrose, corn starch, starch syrup), hypromellose and macrogol. The capsule itself is made of titanium dioxide, sodium indigotin disulfonate, gelatin and sodium erythrosine.

Oral solution

Each milliliter of solution contains as active principle 10 milligrams of itraconazole. The other ingredients are: hydroxypropyl beta-cyclodextrin, sorbitol, propylene glycol, cherry flavor 1 (containing 1,2-propylene glycol and acetic acid), cherry flavor 2 (containing 1,2-propylene glycol and lactic acid), caramel, sodium saccharin, hydrochloric acid, sodium hydroxide (for pH adjustment) and distilled water.

Solution for infusion

Each milliliter of solution for infusion contains as active principle 10 milligrams of itraconazole. The other ingredients are: Propylene glycol, hydroxypropyl-beta-cyclodextrin, concentrated hydrochloric acid, sodium hydroxide and water for injection. Each bag contains 9 milligrams of sodium chloride per milliliter of water for injection.

Additional warning     

This medicine in capsules contains sucrose, which should be taken in mind in diabetic patients. If your doctor has told you that you have an intolerance to some sugars, contact him before taking it.

Before taking the drug     

Do not use itraconazole

If you are allergic (hypersensitive) to the active substance itraconazole or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Also, you should not take this medicine:

Take special care with itraconazole

Taking other medicines

- Certain allergy medications, mainly terfenadine, astemizole, and mizolastine.

- Certain drugs used to treat irregular rhythm of the heartbeat, mainly quinidine and dofetilide.

Certain drugs can reduce in a very extensive way the action of itraconazole. This is particularly applicable to some active ingredients used for the treatment of epilepsy (for example, carbamazepine, phenytoin, and phenobarbital) and tuberculosis (eg, rifampin, rifabutin, and isoniazid). Therefore you should always tell your doctor if you are taking any of these products so that he can take appropriate actions.

The combination with other drugs may require adjustment of the dose of itraconazole or the other drug. Examples are:

- Certain antibiotics called clarithromycin and erythromycin,
- Certain drugs that act on the heart and blood vessels (digoxin, disopyramide, some calcium channel blockers, cilostazol);
- drugs that slow the blood flow;
- methylprednisolone, budesonide, and dexamethasone, drugs administered orally, by injection or inhalation for the treatment of disorders such as inflammations, asthma, and other allergy disorders;
- Cyclosporine A, tacrolimus, and rapamycin (also known as sirolimus), which are usually used after a transplant of organs;
- Certain HIV protease inhibitors;
- Certain drugs used in cancer treatment;
- certain drugs used to treat anxiety or sleeping (tranquilizers), mainly buspirone, alprazolam, and brotizolam;
- ebastine, a medicine for allergy symptoms.

There must be sufficient acidity in the stomach to ensure the absorption of itraconazole from the digestive tract. Therefore, medicines that neutralize stomach acidity should not be taken until after two hours of taking this medicine. For the same reason, if you are taking medicines that stop the production of acid in the stomach, take itraconazole with a cola drink.

Use with food and beverages

Do not take itraconazole with meals but immediately after completing one of the main meals.

How to use itraconazole

The following are the most common doses:

Vaginal infections:2 capsules (200 mg) twice a day one day or 2 capsules once a day for three days.

Skin infections: 2 capsules once daily for 7 days or 1 capsule once a day for two weeks. if the infection affects the palms or soles of the feet, you may require 2 capsules twice daily for 7 days or 1 capsule once a day for 1 month.

Mouth infections: 1 capsule once a day two weeks

Eye infections: 2 capsules once a day three weeks.

Internal infections: 1-4 capsules daily over more long periods

Nail infections: depending on your personal needs your doctor will opt for a continuous or pulse therapy.

- Continuous treatment for nails: 2 capsules once a day three months

-Pulse therapy for nails:

It begins with two capsules twice a day for 1 week; then stop taking for 3 weeks. Then repeat the cycle, once for fingernails and twice for toenail (with or without infection in the fingernails).

This is characteristic of stains caused by fungi: the drug removes the fungus itself but the injury does not go away until growing a new skin.

Nail lesions disappear 6-9 months after completion of treatment because the drug only eliminates the fungus itself. The nail has to grow and this takes several months. So do not worry if you do not notice an improvement during treatment: the medicine remains in the nails for several months and fulfills its mission. Therefore, you should discontinue treatment at the time as directed by your doctor even if you do not see any improvement.
