Indications and usage     

Dexamethasone is an active ingredient belonging to the group of glucocorticoid drugs, such as prednisone, deflazacort and other, sharing its pharmacological effects, mainly anti-inflammatory, antiallergic and immunosuppressive effects (suppressor of immune reactions in the body). Because its high power, dexamethasone is typically used for short periods of time but in high doses.

Among the wide variety of diseases in which are used dexamethasone are included:

Dexamethasone systemically (oral or parenteral):

- Suprarenal insufficiency, thyroiditis.
- Arthritis from various types (rheumatoid arthritis, gout, traumatic arthritis, osteoarthritis, synovitis, bursitis, ankylosing spondylitis, collagenosis such as lupus erythematosus, dermatomyositis, and scleroderma).
- Dermatoses such as pemphigus vulgaris, erythema multiform, exfoliative dermatitis, psoriasis, contact dermatitis, urticaria, eczema, rosacea.

Tablets:

Besides the active principle, tablets may contain lactose monohydrate, propylene glycol, corn starch or potato starch, microcrystalline cellulose, talc, povidone, and magnesium stearate.

Injectable solution:

Besides the active principle, may include disodium edetate, sodium citrate, sodium hydroxide, sodium, creatinine, chloride and water for injections.

Elixir:

Besides the active ingredient, it may contain sucrose, propylene glycol, benzoic acid, alcohol, anhydrous citric acid, red dye No. 40, sodium citrate, citric acid monohydrate and water.

Topical foam:

Besides the active principle (dexamethasone 0.1%), excipients include disodium EDTA, potassium citrate, Hostaphat KL 340 N, Genapol C-090, cetyl alcohol, Hostacerin CS200, propylene glycol, alcohol, sodium bisulfite, Germall plus liquid, DSBC, citric acid and purified water.

Ophthalmic Solution:

Besides the active principle (dexamethasone 0.1%), the excipients may include methylcellulose, glycerol, and benzalkonium chloride.

Nasal ointment:

Besides the active principle (dexamethasone 0.1%), the excipients may include benzyl alcohol, liquid paraffin, purified water, sodium hydroxide, peppermint oil, white petrolatum and lanolin.

Topical cream:

Besides the active principle, the cream may contain cetomacrogol 1000, methylparaben, propylparaben, dimethicone, dequalinium chloride, paraffin palmitate, petrolatum, cetostearyl alcohol, glycerol, isopropyl myristate, propylene glycol, disodium edetate, sorbic acid, lanolin, and purified water.

Additional warning     

Dexamethasone use in sport:

This product contains a component that may produce a positive outcome in doping controls.

Lactose:

Specialties for oral administration may contain lactose. If your doctor has told you that you have an intolerance to certain sugars, contact him before taking this medicine.

Sucrose:

This medicine as syrup or elixir can contain sucrose. If your doctor has told you that you have an intolerance to certain sugars, contact him before taking it.

Ethyl alcohol:

Elixir may contain ethanol (ethyl alcohol 5.1%), representing a dose of 50 mg per ml of alcohol in this solution.

Cetostearyl and cetyl alcohol

Some forms of topical foam or topical cream contain cetostearyl alcohol or cetyl alcohol, that can lead to reactions like contact dermatitis.

Methyl and propyl paraben:

Some specialties for topical use can cause skin reactions like contact dermatitis due to containing lanolin (wool fat).

Before taking the drug     

Do not use dexamethasone

If you are allergic (hypersensitive) to the active substance dexamethasone, to other corticosteroids o or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Also, you should not take this medicine:

- If you have a general infection, either a systemic mycosis (fungal infection), a bacterial acute infection or a virus infection (such as chickenpox or shingles).
- If you will receive a live virus vaccine, attenuated or not. As with administration of other corticosteroids, it should avoid the use of dexamethasone the eight weeks previous and the two following a vaccination with live virus.
- For topical specialties (nasal, ocular, skin), should be avoided its use in case of skin fungal infections (mycoses), cutaneous tuberculosis, viral or bacterial infections, ulcers, acne, rosacea, skin changes induced by recent vaccinations or dermatitis. - If you have severe hypertension,
- If you have glaucoma.

Take special care with dexamethasone

Before taking dexamethasone, tell your doctor if:

- You have a cancer of the blood, as you may be at risk of a very rare, potentially fatal disease caused by the sudden rupture of tumor cells.
- You suffer kidney or liver problems,
- You have high blood pressure or a heart disease,
- You have diabetes or a family history of diabetes,
- You have osteoporosis, especially if you are a woman and have reached menopause,
- You have previously suffered muscle weakness by taking corticosteroids,
- You have glaucoma (ocular hypertension) or a family history of glaucoma,
- You have or have had stomach or duodenal ulcer,
- You suffer from epilepsy,
- You suffer from a migraine
- You have a parasitic infection,
- You have tuberculosis.

If any of these situations is applicable to you or you are not sure, tell your doctor before taking this medicine.

General precautions with corticosteroids:

Risk of adrenal insufficiency:

Patients undergoing prolonged treatment with high doses of corticosteroids suffer a decreased function of the adrenal glands, which can cause a poor response of the organism to situations of physical or psychological stress (trauma, burns, surgery, infections, etc.).

In these situations, the needs of natural corticosteroids are increased, as happens in the case of exacerbation of the disease that motivates the administration of dexamethasone, so it may be necessary to give additional doses of corticosteroids.

The risk of poor response of the adrenal gland persists for months after stopping treatment with dexamethasone, forcing to monitor closely to patients in situations of physical stress, which may require replacement or supplemental doses of corticosteroid.

When the doctor deems necessary to stop the drug, he will proceed to its gradual reduction, in order to avoid a functional failure of the adrenal glands.

Susceptibility to infection and decreased defensive reaction:

Treatment with dexamethasone (or another corticosteroid) may increase the risk of infection, especially concurrent infections and mask their symptoms, making it harder to detect. Also, certain infections that remained latent, as tuberculosis or hepatitis B, can become active .

Corticosteroids also decrease the body's ability to maintain isolated a localized infection, allowing the spread of this disorder.

Patients with acute and chronic bacterial infections should receive appropriate antibiotic treatment for their process.

As steroids suppress the defensive response of the body, vaccination with the live virus should be avoided in patients treated with dexamethasone or other corticosteroids. Corticosteroids may increase the incidence of eye infections by fungi and viruses. In the case of patients with ocular herpes simplex,can occur corneal perforation when corticosteroids are used.

Cardiovascular and metabolic risk

- Acute and chronic bacterial infections, such as acute tonsillitis
- Fungal infections such as candidiasis,
- Parasitic infections, such as intestinal worms

Withdrawal syndrome:

The suspension of prolonged treatment with corticosteroids can lead to a syndrome characterized by fever, muscle aches (myalgia) and pain on joints (arthralgia), with malaise, even if are not evident signs of adrenal insufficiency .

Laboratory tests / allergy tests

Dexamethasone treatment can alter the results of allergy skin tests and determination of thyroid hormone in the blood. They can also be altered bacterial infections tests based in nitroblue tetrazolium.

Mental disorders

Check with your doctor if:

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription. It is important, especially:

- Medicines to treat heart disease (eg digitalis) and blood, as oral anticoagulants (acenocoumarol, warfarin), medicines for high blood pressure, including diuretics,
- Antibiotics such as rifampicin and rifabutin. Fluoroquinolones may increase the risk of tendon rupture.
- Epilepsy drugs such as phenytoin, carbamazepine, phenobarbital, primidone,

- Medicines used to treat pain or inflammation, such as aspirin, ibuprofen or other nonsteroidal anti-inflammatory drugs (NSAIDs),
- Medicines to treat diabetes,
- Medicines to treat fungal infections (itraconazole, ketoconazole, amphotericin B),
- Medications to lower potassium levels,
- Drugs used to treat myasthenia gravis,
- Antiretroviral drugs (to treat AIDS) such as ritonavir, indinavir or saquinavir,
- Oral contraceptives containing estrogen,
- Growth hormone,
- Certain laxatives,
- Antispasmodic drugs, such as rocuronium, vecuronium, atropine and other drugs called anticholinergics,
- Medicines to treat cancer, such as aminoglutethimide,
- Methotrexate, a drug used to treat certain inflammatory processes and malignant lesions,
- Immunosuppressants used to prevent transplant rejection, such as cyclosporine.
- Ephedrine still used to relieve nasal congestion,
- Acetazolamide used for the treatment of glaucoma.

Use with food and beverages

This drug, taken orally, should preferably be taken after meals.

Special physiological situations

Pregnancy and lactation:

How to use dexamethasone

Dexamethasone doses used are usually highly variable from patient to patient, depending on the nature and severity of the disease. The doctor will prescribe a dosage for each patient individually.

Oral administration:

In general, the dosage for adult patients is between 0.5 mg and 10 mg daily, but may be higher in more severe cases.

As improvement occurs, your doctor may recommend a lower dose or even gradually replace this drug by other less potent corticosteroid. The most common maintenance dose ranges between 0.5 and 2 mg per day in the adult.

The usual dose for children varies between 0.01 and 0.1 milligrams per kilogram of body weight per day.

If you forget to take the dose of dexamethasone:

Do not take a double dose to make up for a forgotten dose.

If you stop taking dexamethasone

Do not stop by own initiative dexamethasone treatment, because it can be dangerous.

Side Effects of dexamethasone     

Like all medicines, dexamethasone can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients

Mental disorders:

Corticosteroids, including dexamethasone, can cause serious mental health problems, affecting 6% of the exposed population, both children and adults.

These includes:

- Feeling depressed, to have suicidal thoughts or suicidal behavior,
- Fluctuations in mood, ranging from a state of exaggerated exaltation, with increased patient activity (mania) to states with low or very low mood (depression), similar to bipolar disorder,
- Anxiety, sleep disturbances, impaired thinking, memory loss or confusion state,
- Emotional instability, irritability, euphoria,

Allergic reactions

If you suffer an allergic reaction to dexamethasone, consult your doctor immediately.

Allergic reactions may arise as shortness of breath or wheeze (bronchospasm), angioedema, various rashes, usually accompanied by itching or be more intense and lead to anaphylactic shock (fainting, loss of consciousness, pallor and down sudden blood pressure.
