Before taking the drug

Do not use dexamethasone

If you are allergic (hypersensitive) to the active substance dexamethasone, to other corticosteroids o or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Also, you should not take this medicine:

- If you have a general infection, either a systemic mycosis (fungal infection), a bacterial acute infection or a virus infection (such as chickenpox or shingles).
- If you will receive a live virus vaccine, attenuated or not. As with administration of other corticosteroids, it should avoid the use of dexamethasone the eight weeks previous and the two following a vaccination with live virus.
- For topical specialties (nasal, ocular, skin), should be avoided its use in case of skin fungal infections (mycoses), cutaneous tuberculosis, viral or bacterial infections, ulcers, acne, rosacea, skin changes induced by recent vaccinations or dermatitis. The Ocular application should be avoided in the case of glaucoma.

- If you have glaucoma.

If any of these situations is applicable to you or you are not sure, tell your doctor before taking this medicine.

Take special care with dexamethasone

Before taking dexamethasone, tell your doctor if:

- You have a cancer of the blood, as you may be at risk of a very rare, potentially fatal disease caused by the sudden rupture of tumor cells.
- You suffer kidney or liver problems,
- You have high blood pressure or a heart disease,
- You have diabetes or a family history of diabetes,
- You have osteoporosis, especially if you are a woman and have reached menopause,

- You have glaucoma (ocular hypertension) or a family history of glaucoma,
- You have or have had stomach or duodenal ulcer,
- You suffer from epilepsy,
- You suffer from a migraine
- You have a parasitic infection,
- You have tuberculosis.

If any of these situations is applicable to you or you are not sure, tell your doctor before taking this medicine.

General precautions with corticosteroids:

Risk of adrenal insufficiency:

Patients undergoing prolonged treatment with high doses of corticosteroids suffer a decreased function of the adrenal glands, which can cause a poor response of the organism to situations of physical or psychological stress (trauma, burns, surgery, infections, etc.).

In these situations, the needs of natural corticosteroids are increased, as happens in the case of exacerbation of the disease that motivates the administration of dexamethasone, so it may be necessary to give additional doses of corticosteroids.

The risk of poor response of the adrenal gland persists for months after stopping treatment with dexamethasone, forcing to monitor closely to patients in situations of physical stress, which may require replacement or supplemental doses of corticosteroid.

When the doctor deems necessary to stop the drug, he will proceed to its gradual reduction, in order to avoid a functional failure of the adrenal glands.

Susceptibility to infection and decreased defensive reaction:

Treatment with dexamethasone (or another corticosteroid) may increase the risk of infection, especially concurrent infections and mask their symptoms, making it harder to detect. Also, certain infections that remained latent, as tuberculosis or hepatitis B, can become active .

Corticosteroids also decrease the body's ability to maintain isolated a localized infection, allowing the spread of this disorder.

Patients with acute and chronic bacterial infections should receive appropriate antibiotic treatment for their process.

As steroids suppress the defensive response of the body, vaccination with the live virus should be avoided in patients treated with dexamethasone or other corticosteroids. Is the case of smallpox and other vaccine-preventable diseases, since a generalized infection by the virus contained in the vaccine may occur. Exceptions to this rule are the cases of patients in which corticosteroid is administered as replacement therapy if exists a natural hormone deficiency (Addison's disease).

Vaccination with inactivated vaccines may be less effective that can be expected if patients should continue treatment with dexamethasone or other corticosteroids.

For similar reasons, patients undergoing treatment with corticosteroids should avoid exposure to natural viral infections such as chickenpox, measles or shingles.

If corticosteroids are administered to patients with latent tuberculosis or positive tuberculin reaction (Mantoux test) the disease can active again; so, in these cases, close medical supervision is needed.

Risk of eye complications

Prolonged use of dexamethasone (or other corticosteroids) may cause or aggravate a glaucoma, cataracts, and other diseases or pre-existing lesions, so is necessary to do at regular intervals an eye close monitoring, such as every 3 to 4 months.

Corticosteroids may increase the incidence of eye infections by fungi and viruses. In the case of patients with ocular herpes simplex, can occur corneal perforation when corticosteroids are used.

Cardiovascular and metabolic risk

Corticosteroids can raise blood pressure, cause salt and water retention and increased excretion of calcium and potassium, so your doctor may prescribe potassium supplements and recommend reducing consumption of sodium (salt).

In the case of a severe heart failure, a careful medical supervision is necessary .

Children and adolescents

Dexamethasone may affect growth in children, so should be controlled the development of pediatric patients on prolonged treatment.

Prolonged treatment in children can cause damage to the pancreas.

Other precautions

As far as possible, the doctor will use the lowest dose that is effective, for the shortest time possible.

The effect of corticosteroids increases in patients with hypothyroidism or cirrhosis; such patients may require lower doses of dexamethasone.

Dexamethasone and other corticosteroids can lead to psychiatric disorders or aggravate existing situations of emotional instability or psychosis. can appear sleep disorders requiring a change in treatment.

Corticosteroids may alter motility and number of spermatozoa in some patients.

If you have diabetes, your doctor may increase your diabetes medicine and more closely monitor your treatment.

If you have risk factors for osteoporosis (hereditary, elderly, post-menopause, heavy smoking and lack of physical activity) treatment with dexamethasone can cause bone mass loss, so your doctor can prescribe a treatment to prevent osteoporosis.

Corticosteroids should be used with caution in:

- Gastroduodenal ulcers,
- Severe ulcerative colitis at high risk of perforation, abscess or other pyogenic infection
- Diverticulitis,
- Recent intestinal anastomosis (surgery for joining two sections of intestine),
- Kidney failure,
- Arterial hypertension. Hypertensive patients should be monitored more frequently.
- Myasthenia gravis,
- Hepatitis B,
- Tuberculosis,
- Poliomyelitis, a viral infection that causes the so-called infantile paralysis,
- Acute viral infections, such as chickenpox, herpes simplex, herpes zoster or measles,
- Acute and chronic bacterial infections, such as acute tonsillitis
- Fungal infections such as candidiasis,
- Parasitic infections, such as intestinal worms

Withdrawal syndrome:

The suspension of prolonged treatment with corticosteroids can lead to a syndrome characterized by fever, muscle aches (myalgia) and pain on joints (arthralgia), with malaise, even if are not evident signs of adrenal insufficiency .

Laboratory tests / allergy tests

Dexamethasone treatment can alter the results of allergy skin tests and determination of thyroid hormone in the blood. They can also be altered bacterial infections tests based in nitroblue tetrazolium.

In either case, tell your doctor if you are being treated with dexamethasone.

Mental disorders

Check with your doctor if:

- You have had severe depression or bipolar disorder. This includes having had previously depression while taking steroids such as dexamethasone,
- If a family member has had any of these diseases,
- If you have had any of these processes unrelated to steroids, consult a doctor before taking this medicine
. - If you have had another kind of mental disorder while taking corticosteroids,

Mental illnesses caused by corticosteroids may be severe and usually start a few days or weeks of starting treatment. The risk increases with high doses.

Most of these disorders are reduced or disappear upon dose reduction or discontinuation of therapy.

If you experience depression or other mental symptoms, consult your doctor because you may need treatment. This is especially important if you are depressed, or may be thinking about suicide. In some cases, these mental problems have occurred during the dose reduction or even when you have stopped treatment with dexamethasone.

Other important precautions

- If you develop an infection while taking this drug, you should inform your doctor.
- Tell any doctor or dentist that can assist you that you are taking or have taken corticoids in the past.
- Virus infections: While taking this type of medication, you should not come into contact with people who are suffering from a viral infection (chicken pox, measles, herpes zoster, etc.) if you are not sure you have sufficient defenses against them. If you think you may have been exposed to any of these conditions, you should inform your doctor immediately.
- If you are going to be vaccinated, need assistance by have suffered an accident, including burns, or you are having surgery or feel sick, tell the doctor who treats you that you are taking corticosteroids.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

If any of these situations is applicable to you or you are not sure, tell your doctor before taking this medicine.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription. It is important, especially:

- Medicines to treat heart disease (eg digitalis) and blood, as oral anticoagulants (acenocoumarol, warfarin), medicines for high blood pressure, including diuretics,
- Antibiotics such as rifampicin and rifabutin. Fluoroquinolones may increase the risk of tendon rupture.
- Epilepsy drugs such as phenytoin, carbamazepine, phenobarbital, primidone,
- Medicines to treat stomach disorders, such as antacids or carbenoxolone. An interval of two hours or more should be respected between dexamethasone and these drugs.
- Medicines used to treat pain or inflammation, such as aspirin, ibuprofen or other nonsteroidal anti-inflammatory drugs (NSAIDs),
- Medicines to treat diabetes,
- Medicines to treat fungal infections (itraconazole, ketoconazole, amphotericin B),

Dexamethasone passes to the baby through breast milk, so it is not recommended breastfeed your child while on treatment with this medicine. Your doctor will tell you whether you should stop treatment with corticosteroids or breastfeeding.

Children and adolescents:

Besides following general precautions in the use of dexamethasone, development of these patients should be closely monitored, since corticosteroids may alter the growth in long-term treatment.

Elderly:

In elderly patients, the physician should evaluate the benefits and risks of treatment, taking the highest cautions indicated above, since the risks inherent in the use of corticosteroids are more frequent and with greater severity at these ages and appear at lower doses than in younger adults.

Driving and using machines

It is unlikely that the administration of dexamethasone affects your ability to drive or operate machinery, but have not been conducted studies on this point. However, if as result of treatment, you experience eye pain, blurred vision or loss of visual acuity, avoid carrying out these activities and consult your doctor.

Consult your doctor or pharmacist before taking any medicine.

How to use dexamethasone

Follow carefully the instructions of use of dexamethasone provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given on this website, that may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Dexamethasone doses used are usually highly variable from patient to patient, depending on the nature and severity of the disease. The doctor will prescribe a dosage for each patient individually.

Oral administration:

In general, the dosage for adult patients is between 0.5 mg and 10 mg daily, but may be higher in more severe cases.

As improvement occurs, your doctor may recommend a lower dose or even gradually replace this drug by other less potent corticosteroid. The most common maintenance dose ranges between 0.5 and 2 mg per day in the adult.

The usual dose for children varies between 0.01 and 0.1 milligrams per kilogram of body weight per day.

Parenteral administration:

Presentations of injectable solution can be administered intravenously, intramuscular, intraarticular, intralesional or intradermal.

Duration of treatment will be established by the physician. In the treatment of natural hormone replacement, treatment is indefinite.

Ocular or nasal administration:

Solutions and suspensions for ocular use are applied at regular intervals, every 4-8 hours, as medically indicated, placing one drop in each eye. Similarly, nasal applicators are administered at regular intervals, as recommended by the doctor for each case.

Other topical applications;

Creams and foams for external use are applied once or twice a day.

The golden rule of dexamethasone for all applications is to use the minimum amount effective during the shortest time possible.

If you think that the effect of the medicine is too strong or too weak, consult your doctor.

If you have any further questions on the use of this product, ask your doctor or pharmacist.

While you take dexamethasone

If you become pregnant, contact your doctor immediately.

If a doctor prescribes another medicine , tell you are taking dexamethasone.

Check with your doctor regularly development of disorder that motivates the use of dexamethasone. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.
