Indications and usage     
Esomeprazole is a type of medication from the group of the proton-pump inhibitor (PPI). It reduces the production of stomach acid.

This medicine is indicated for:

- Treatment of gastroesophageal reflux disease (GERD): acid from the stomach amounting into the esophagus producing a burning sensation in the esophagus, inflammation, and pain.
- Eradication of Helicobacter pylori, duodenal ulcer healing, and prevention of relapse of peptic ulcers associated with Helicobacter pylori, in combination with an appropriate antibacterial therapeutic regimen.
- Healing of gastric ulcers associated with NSAIDs treatment (drugs that reduce inflammation, pain, and fever, used primarily in the treatment of disorders of the muscles and joints) in patients requiring continued treatment with these drugs.
- Prevention of gastric and duodenal ulcers associated with treatment with NSAIDs in patients at risk who need continued treatment with these drugs.
- Zollinger-Ellison syndrome: a disease characterized by a large increase in gastric acid secretion.

Tablets

Besides the active ingredient esomeprazole, excipients are Glycerol monostearate, hyprolose, hypromellose, magnesium stearate, methacrylic acid copolymer and ethyl acrylate, microcrystalline cellulose, synthetic paraffin, macrogol, polysorbate 80, crospovidone, sodium stearyl fumarate, sugar spheres (sucrose and corn starch without gluten), talc, triethyl citrate. Dyes: iron oxide (red-brown, yellow) (E172) and titanium dioxide (E 171).

Granules

Each sachet contains 10 mg esomeprazole. Remaining components are:

Esomeprazole granules: Glyceryl monostearate 40-55, hydroxypropyl cellulose, Hypromellose, magnesium stearate, methacrylic acid copolymer and ethyl acrylate (1: 1) 30% dispersion, Polysorbate 80, Sugar spheres (sucrose and corn starch), talc, triethyl citrate.Carrier granules: anhydrous citric (for pH adjustment) Acid, Crospovidone, glucose, hydroxypropyl cellulose, iron oxide yellow (E172) and xanthan gum

Injectable solution

Each vial contains 40 mg of esomeprazole (as sodium salt). The other ingredients (excipients) are disodium edetate and sodium hydroxide.

Important information     

Tablets: This medicine contains sucrose. If your doctor has told you that you have an intolerance to some sugars, contact him before taking this medicine.

Granules: This dosage form contains sucrose and glucose, which are two types of sugars. Therefore, it is important to maintain a careful oral hygiene and a careful brushing regularly. If your doctor has told you that you have an intolerance to some sugars, contact him before taking it.

Before taking the drug     

Do not use esomeprazole

If you are allergic (hypersensitive) to the active substance esomeprazole, other medicines in the same group (proton-pump inhibitor, PPI) or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor take this medicine if:

If you are being treated with atazanavir (a medicine for the treatment of patients infected with HIV-1 virus).

Take special care with esomeprazole

- If you have severe liver problems. In this case, you should consult your doctor, as he may reduce your dose.

Taking other medicines

- Itraconazole, ketoconazole or voriconazole (for fungal infections),
- Diazepam (for anxiety and/or insomnia),
- Phenytoin (for epilepsy),
- Citalopram, imipramine, clomipramine (for depression),
- Warfarin or other coumarin derivatives (for blood clotting),
- Cisapride (to accelerate gastric emptying) or
- Atazanavir (for treatment of patients infected with HIV-1 virus)

Use with food and beverages

If you have difficulty swallowing, the tablets may be dispersed in half a glass of still water. Do not use other liquids.

Granulated gastro-resistant can be taken with or without food.

Pregnancy

Before taking this medication, tell your doctor if you are pregnant or trying to become pregnant. If you are pregnant, take only esomeprazole if your doctor prescribes it.

Lactation

Children

Do not give this medicine to children younger than 1 year or weighing less than 10 kg (<10 kg).

Driving and using machines

There is no evidence of effects on the ability to drive or operate machinery.

How to use esomeprazole     

Follow carefully the instructions of use of esomeprazole provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Method of using and route of administration

Your doctor will indicate you how much medicine to take and when to take it. You can take the tablets or granules with food or on an empty stomach.

The tablets should not be chewed or crushed. Should be swallowed whole with half a glass of liquid.

For patients with difficulty swallowing, tablets can also be dispersed in half a glass of still water. Do not use other liquids. Stir until the tablets disintegrate (observe that is not formed a clear solution ). You can drink it immediately or within 30 minutes. Always stir the liquid dispersion just before drinking.

To ensure that you have taken all the medication, rinse the glass filling it halfway with water and drink it again. The solid particles contain the medication. The granules should not be chewed or crushed.

For patients who can not swallow, the tablets can be dispersed in non-carbonated water and administered through a gastric tube. It is important to carefully check the suitability of the syringe and the selected probe.

Dosage, frequency of administration and duration of treatment:

Adults and adolescents from 12 years of age

Treatment of gastroesophageal reflux disease (GERD)

The usual dose is one or two tablets of 20 mg or 40 mg esomeprazole once daily or its equivalent in granules. The duration of treatment is generally up to 4 weeks.

Your doctor may choose that you keep taking the tablets, depending on how you respond to treatment. Once completed, your doctor may tell you to continue taking a maintenance dose of esomeprazole once daily or only when needed.

Adults

Treatment for duodenal ulcer (elimination of Helicobacter pylori) and preventing relapses of ulcers (associated with Helicobacter pylori) in combination with an appropriate antibacterial therapeutic regimen:

The usual dose is 20 mg esomeprazole twice daily. Your doctor will also tell you to take the following antibiotics: amoxicillin and clarithromycin. The usual duration of treatment is one week.

Follow carefully the instructions for taking the medication and ask your doctor if you are unsure.

Therapy for the healing of gastric ulcers and prevention of gastric and duodenal ulcers associated with NSAIDs treatment:

The usual dose is 20 mg esomeprazole once daily. The duration of treatment for healing of gastric ulcers is 4 to 8 weeks.

Treatment of Zollinger-Ellison syndrome:

The usual starting dose for adults is 40 mg esomeprazole twice daily. Your doctor will adjust the dose according to your needs and also will decide how long to take this medicine.

Children

Esomeprazole should not be administered to children younger than 1 year or weighing less than 10 kg (<10 kg).

Children between 1 and 11 years old can take this medicine. The usual dose is one sachet (10 mg) or two sachets (20 mg) once daily. The dose for children is based on body weight and the doctor will decide the correct dose.

Administration of solution for injection or infusion:

This drug is administered intravenously by a health professional.

Patients who can not take oral medication may be treated intravenously with 20-40 mg esomeprazole once daily.

Patients with reflux oesophagitis should be treated with 40 mg of esomeprazole (1 vial), once a day.

Patients with reflux disease treated symptomatically should be treated with 20 mg of esomeprazole (half a vial), once a day.

For healing of gastric ulcers associated with NSAIDs treatment, the usual dose is 20 mg of esomeprazole (half a vial), once a day

For the prevention of gastric and duodenal ulcers associated with treatment with NSAIDs, risk patients should be treated with 20 mg of esomeprazole (half a vial), once a day.

Usually, the duration of intravenous therapy is short and change to oral treatment should be performed as soon as possible.

This drug can be administered by injection or intravenous infusion after reconstitution of the solution from the powder contained in the vial.

Injection

40 mg dose: The reconstituted solution should be administered as an intravenous injection for a period of at least 3 minutes.

20 mg dose: Administer half of the reconstituted solution as an intravenous injection over a period of about 3 minutes. Dispose of all unused solution.

Perfusion

40 mg dose: The reconstituted solution should be administered as an intravenous infusion for a period of 10 to 30 minutes.

20 mg dose: Administer half of the reconstituted solution as an infusion intravenously over a period of 10-30 minutes. Dispose of all unused solution.

If you think that the effect of the medicine is too strong or too weak, consult your doctor.

While you take esomeprazole     

If you become pregnant, contact your doctor immediately.

If a doctor prescribes another medicine , tell you are taking esomeprazole.

Check with your doctor regularly developments of disorder that motivates the use of esomeprazole. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with esomeprazole at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with esomeprazole, immediately consult your doctor.

If you use more medicine than you should

Consult your doctor or pharmacist immediately. In the case of an overdose or accidental ingestion, consult the Information Poison Center in your country, indicating the product and the amount ingested.

If you forget a dose:

Take the missed dose as soon as you remember, and then continue the usual dosing schedule. However,If is near the time for next dose, skip the missed dose and wait until be the time to taking it.

Do not take a double dose to make up for missed doses.

Side Effects of esomeprazole

- Immune system disorders: Hypersensitivity reactions (allergic reactions), eg, fever, swelling of face, lips, tongue and/or throat(angioedema) and anaphylactic reaction/shock,
- Metabolism and nutrition disorders: Low sodium levels
- Psychiatric disorders: Agitation, confusion, depression,

- Disorders of the blood and lymphatic system: Agranulocytosis (decrease of granulocytes, leukocytes with a defensive function), pancytopenia (global decline in the number of cells in blood)
- Psychiatric disorders: aggressiveness, hallucinations,
- Hepatobiliary disorders: Liver failure, disorientation, confusion, decreased consciousness in patients with pre-existing liver disease,
- Disorders of the skin and subcutaneous tissue: Severe skin reactions (erythema multiforme, toxic epidermal necrolysis),
- Musculoskeletal, connective tissue, and bone disorders: muscle weakness,
- Renal and urinary disorders: Interstitial nephritis (kidney disorder)
- Reproductive system and breast disorders: Enlarged breasts (gynecomastia).
