Indications and usage     

Lisinopril belongs to the group of drugs known as inhibitors of angiotensin converting enzyme (ACE). In addition to blocking the conversion of angiotensin I into angiotensin II, this medicine dilates blood vessels, lowers blood pressure and favors the heart function for carrying the blood to all the body, reducing its overload.

Lisinopril is indicated for the treatment of:

- Arterial hypertension in patients six or more years (high blood pressure),
- Heart failure, as adjunctive therapy
- During the recovery phase of acute myocardial infarction,
- Diabetic or hypertensive nephropathy (kidney damage due to diabetes and/or hypertension).

The active ingredient Lisinopril is indicated for children 6 years old or more for the treatment of hypertension alone. It is not recommended if the child has a significant alteration of renal function.

Other ingredients of available commercial presentations:

Before taking the drug     

Do not use lisinopril

If you are allergic (hypersensitive) to the active substance lisinopril, to other ACE inhibitors or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Also, you should not take this medicine:

- If you suffer hereditary angioedema,
- Any member of your family have had an allergic reaction to ACE inhibitors,
- You have ever had an allergic reaction for no apparent reason,
- You are breastfeeding your child,
- You are pregnant for more than 3 months. Even if you are in the first three months of pregnancy, it is preferable not to take this medicine.

Take special care with lisinopril

- If you think you are pregnant or you are planning a pregnancy. The use of lisinopril is not recommended during the first trimester of pregnancy and is contraindicated during the remaining two-quarters. Like other ACE inhibitors, lisinopril may cause severe damage to the fetus.
- If you suffer from dehydration caused by diuretics, vomiting, or diarrhea, you are on dialysis or you follow a diet with reduced salt content,
- If you have recently had a myocardial infarction,
- If you have a narrowing of the aortic or mitral valve or suffer hypertrophic cardiomyopathy. These disorders obstruct normal blood flow through the heart,
- If you have any kidney disease, including narrowing of blood vessels in this organ (renal artery stenosis) or have recently undergone a kidney transplant,
- If you have a disease that affects the brain's blood vessels,
- If you have rheumatoid arthritis or another joint disease,
- If you are being treated with a machine for removing cholesterol (cholesterol LDL apheresis)
- If you had previously angioneurotic edema (angioedema),
- If are under desensitizing treatment to hymenoptera venom (bee, wasp),
- If you have a liver disease,
- If you will have to undergo surgery, inform the doctor that you are taking lisinopril,
- If you have a collagen disease (group of diseases that affect the blood vessels along with the connective tissue, such as lupus erythematosus and other),

If you think you are (or previously could be included) in any of the above circumstances or have concerns, see your doctor before taking lisinopril, as you may be more sensitive to the effects of this medicine.

Black patients or of afro-caribbean origin may have more often allergic adverse reactions to lisinopril. The medicine may not be as effective as in the rest of population.

If you have any infection with fever, impaired general condition, or symptoms of a throat infection or urinary disorders, consult your doctor.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription. It is important, especially:

- Potassium-sparing diuretics, as spironolactone triamterene or amiloride, since potassium level in blood can be raised too.
- Thiazide diuretics (chlorothiazide, hydrochlorothiazide, indapamide, xipamide and other), furosemide or other medicines to lower blood pressure, medicines to treat angina. Concurrent use of lisinopril can cause a greater than expected reduction in blood pressure.
- Lithium (for some types of mental illness such as bipolar disorder,
- Medicines for depression, as amitriptyline, medications for severe mental illness as chlorpromazine, morphine (for severe pain) or anesthetics. Taking these medicines with lisinopril may increase the reduction of blood pressure.
Nonsteroidal anti-inflammatory drugs (NSAIDs), as indomethacin, diclofenac or high doses of acetylsalicylic acid (for pain or inflammation). These drugs can reduce the effectiveness of lisinopril.
- Sympathomimetic as ephedrine and pseudoephedrine (used in airway colds), noradrenaline or adrenaline. The simultaneous taking of lisinopril may decrease the effectiveness of this.
- Medicines for diabetes, as insulin. The active ingredient lisinopril may increase the fall in blood sugar levels when taken with insulin, especially during the first weeks of treatment and in patients with previous kidney damage. In these circumstances, should closely monitor levels of blood sugar.
- Allopurinol (a medicine to treat gout and kidney stones),
- Procainamide (to treat heart rhythm disorders),
- Immunosuppressive (medicines used to block the immune system) as cyclosporine.
- Gold salts, as sodium aurothiomalate (used in rheumatoid arthritis), because it can cause flushing, dizziness, nausea, feeling unwell and may decrease blood pressure too.
- Potassium supplements, either in the form of foods high in this substance or salt substitutes containing potassium,
- Salbutamol, a drug to treat asthma.

Use with food and beverages

Lisinopril can be taken with or without food.

Consult your doctor or pharmacist before taking any medicine.

Special physiological situations

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy and lactation:

Tell your doctor if you think you are (or might be) pregnant or are planning to have a baby. Your doctor will advise you to stop taking lisinopril before you become pregnant or as soon as you know you are pregnant and will advise you to take another medicine instead. Lisinopril is not recommended during the first trimester of pregnancy and is contraindicated since that time, as it may cause serious harm to your baby if used after the third month of pregnancy.

If you have taken lisinopril inadvertently during pregnancy, it is advisable to undergo an ultrasound scan to check the status of the kidneys and the skull of the fetus.

It is highly recommended to continue an effective contraceptive treatment while taking lisinopril.

Tell your doctor if you are breastfeeding or about to start breastfeeding. The use of lisinopril for mothers who are breast-feeding, especially in the case of premature babies and newborns is not recommended. Your doctor may substitute this drug by another compatible with breastfeeding.

Children and adolescents:

Lisinopril may be used in children over six years for treatment of arterial hypertension alone.

Driving and using machines

Lisinopril may make you feel tired and dizzy. If this happens, do not drive or operate any tools or machines.

How to use lisinopril     

Follow carefully the instructions of use of lisinopril provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given on this website, that may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Swallow the tablets whole without chewing or crushing, accompanied with water. Do it every day at about the same time, which also helps not to forget to take the medicine.

The first few days might notice that feel dizzy or lightheaded, by a more pronounced reduction in blood pressure. You can lie down until you feel better. If you are concerned, talk to your doctor.

The doctor can check your response to taking lisinopril, controlling your blood pressure and doing some blood tests.

Adults with hypertension:

Recommended starting dose is 10 mg a day, which your doctor may gradually rise to reach a dose that normalizes blood pressure, up to a maximum dose of 80 mg.

If you are taking high doses of diuretics, your doctor may advise you to stop this drug for a few days before starting treatment with lisinopril.

Adults with heart failure:

The recommended usual starting dose is 2.5 mg a day, which your doctor may gradually increase to a maximum of 35 mg per day, depending on your response to treatment.

Myocardial infarction:

Treatment can begin within the first 24 hours of the attack.

The usual starting dose is 5 mg daily for 2 days, which can be increased to 10 mg the third. Treatment may continue for about 6 weeks after the heart attack.

Kidney disorders:

The doctor will adapt the dose of lisinopril to the degree of kidney function you have. In case you be on dialysis, your doctor may change each day the dose of lisinopril.

Elderly:

Your doctor will recommend a dose according to functional capacity of your kidneys.

Children under 6 years:

The use of lisinopril is not recommended in this age group.

Children and adolescents aged 6 to 16 years:

The usual initial dose is between 2.5 mg and 5 mg once a day, which can be increased up to 20 to 40 mg once daily. Patients with kidney problems should take a lower dose. The doctor will decide the most appropriate dose for the patient.

Kidney transplant patients:

This medication should not be used in patients who have recently had a kidney transplant.

While you take lisinopril     

If you become pregnant, contact your doctor immediately.

If you forget to take the dose of lisinopril:

Wait for the next dose to resume treatment.

Do not take a double dose to make up for a forgotten dose.

If you stop taking lisinopril

Do not stop treatment without the knowledge and approval of your doctor, as this is likely this could reduce the effect of medicine.

Side Effects of lisinopril     

Like all medicines, lisinopril can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Stop taking this medicine and call your doctor immediately or go to the nearest hospital if:

- You have an allergic reaction. Signs and symptoms of allergic reactions are indicated above    ▲
- You experience a widespread reaction with blistering and/or peeling of the skin or mucosa in any part of the body, including the mouth, nose, eyes and genitals.

As noted earlier, allergic reactions to lisinopril and other ACE inhibitors are more common in black patients or afro-caribbean origin patients.

Tell your doctor immediately if you notice any of the following side effects. These are rare side effects (affect less than 1 in 1,000 patients).

- Severe stomach pain or back pain. Could be a pancreatitis,
- Blood diseases, including disorders of bone marrow and anemia. Symptoms include having an easy bruising, bleed longer after trauma, easily bleeding gums or other places, More ease to get infections.
- High fever, tiredness, loss of appetite, stomach ache, nausea, yellowing of the skin or eyes (jaundice) and liver failure. These are symptoms of hepatitis (liver inflammation).
- A reaction that may include some or all of the following: High fever, inflammation of blood vessels, inflammation, and muscle or joint pain, disorders of blood detected in a blood test, rash, hypersensitivity to light, and other manifestations on the skin.

- Dizziness,
- Headache,
- Cough,
- Fainting or dizziness when standing up quickly (orthostatic hypotension)
- Vomiting diarrhea
- Kidney disorders, including renal failure.

Uncommon side effects< (affect 1-10 in 1.000 patients):

- Mood swings,
- Unusual sensations on the skin, such as numbness, tingle, punctures, burning, or sensations as objects or insects crawling on the skin (paresthesia),
- Impaired balance and dizziness,
- Altered taste,
- Sleep disorders,
- Difficulty breathing,
- Chest pain (angina), heart attack and stroke in high-risk patients,
- Fast heartbeat (tachycardia), palpitations,
- Raynaud's phenomenon, consisting in transitory artery spasms of the fingers or toes, which change of color when exposed to cold or pressure, accompanied by pain when are cold and tingling or pain when heated,
- A runny nose, nasal congestion, and nasal itching, sneezing (rhinitis),
- Erectile dysfunction, difficulty ejaculating,
- Tiredness, lack or loss of strength (weakness)
- Feeling sick (nausea),
- Abdominal pain,
- Indigestion,
- Rash and/or pruritus (itching),
- Allergic reactions, especially angioedema.
- Increase in blood levels of urea, creatinine and liver enzyme values (transaminases),
- Increased blood potassium (hyperkalemia), that may manifest with muscle cramps, diarrhea, nausea, dizziness, and headache.

Rare side effects (affect 1-10 in 10.000 patients):

- Feeling confused,
- Dry mouth,
- Hair loss (alopecia), baldness,
- Psoriasis, a skin disorder,
- Increased bilirubin in the blood, a substance produced in the liver from hemoglobin,
- Decreased level of sodium in the blood, which can cause fatigability, tiredness, confusion and muscle spasms,
- Other allergic reactions,
- Breast enlargement in men (gynecomastia),
- Impaired renal function, which leads to an increase of urea and creatinine in the blood,
- Urticaria, with itching and hives

Very rare side effects (affect less than 1 in 10.000 patients):

- Low levels of blood sugar (hypoglycemia). The patient may feel a sense of nervousness, tremor or profuse sweating.
- Intestinal angioedema, similar to skin angioedema, which is manifested by stomach pain with or without nausea and vomiting,
- Reduced volume of urine passed during the day
- Severe respiratory distress, wheezing, chest tightness,
- Pain and tenderness in the face (area of paranasal sinuses), obstruction and/or a runny nose (sinusitis). The patient may have other sinusitis symptoms such as fever, fatigue, headache, cough, bad breath, a pressure in the ears, loss of taste and smell and malaise.

- Swelling and rash (skin pseudolymphoma),
- Feeling sick, confusion, weakness, nausea, loss of appetite e irritability. These symptoms may correspond to a syndrome of inappropriate secretion of ADH (antidiuretic hormone),
- profuse sweating.

Side effects of unknown frequency:

- Fainting,
- Depression.

Side effects on the results of blood tests:

- Decreased hemoglobin and red blood cells (anemia),
- Decreased level of sodium in the blood, which can cause fatigability, tiredness, confusion and muscle spasms,
- Elevated levels of potassium, creatinine, urea, liver enzymes or bilirubin.

Skin reactions may be accompanied by fever, inflammation of blood vessels (vasculitis), muscle pain (myalgia), joint pain (arthralgia) and swollen joints (arthritis), increased antinuclear antibodies (ANA), increased erythrocyte sedimentation rate, rash and increased number of white blood cells (eosinophilia and leukocytosis), itching, sensitivity to light and other skin reactions.

Safety of use in children:

Available data on the safety of lisinopril in children suggest that this drug is well tolerated and comparable to the adult population.
