Indications and usage     

Ofloxacin is an antibiotic belonging to the group of quinolones, which act by killing bacteria that cause infection.

This substance is indicated for the treatment of the following bacterial infections when applied orally or parenterally:

- Infections of the lower respiratory tract, including pneumonia
- Chronic or recurrent throat infections, nose or ears (Not for acute tonsillitis)
- Infections of the abdomen and biliary tract
- intestinal infections caused by bacteria
- Urinary tract infections (kidney, prostate or bladder)
- Infections of sexual organs, including gonorrhea, a sexually transmitted infection that can affect both sexes
- Infection of the skin and soft tissues, including muscles
- Bone and joint infections
- Blood infections (sepsis, septicemia).

Ofloxacin is not the drug of first choice for pneumonia caused by pneumococcus or mycoplasma, or for tonsillitis (tonsillitis) caused by beta-hemolytic streptococci.

Ofloxacin in solution for ophthalmic use is indicated for treating infections of the conjunctiva and cornea and corneal ulcers caused by bacteria sensitive to the action of this antibiotic in adults and children over one-year-old.

Ofloxacin in solution for otic use is indicated for the treatment of the following ear infections produced by microorganisms susceptible to the effect of ofloxacin:

- External ear infections in adults and children six or more months produced by Escherichia coli, Pseudomonas aeruginosa and Staphylococcus aureus
- Chronic suppurative otitis media in patients 12 years or older with perforated eardrum caused by Proteus mirabilis, Pseudomonas aeruginosa, and Staphylococcus aureus
- Acute otitis media caused by Haemophilus influenzae, Moraxella catarrhalis, Pseudomonas aeruginosa, Staphylococcus aureus and Streptococcus pneumoniae in children one year or older with tympanostomy tubes.

Medizzine recommends consulting your doctor if you are unsure (or you do not know) why you have been given ofloxacin.

Tablets:

Besides the active ingredient, tablets may contain lactose monohydrate, corn starch, hypromellose, polysorbate 80, sodium starch glycolate, magnesium stearate, hypromellose, titanium dioxide, polyethylene glycol (macrogol), talcum, colloidal silicon dioxide, pregelatinized starch and yellow iron oxide.

Solution for otic/ophthalmic use:

In addition to the active ingredient, the other ingredients are benzalkonium chloride, sodium chloride, purified water, sodium hydroxide and hydrochloric acid. These last two are for adjusting the pH's solution.

IV solution:

The non-active ingredients include sodium chloride, hydrochloric acid and water for injection.

Warning:

Important information     

The use of the ophthalmic solution while wearing contact lenses has not been studied properly. Therefore, ofloxacin use is not recommended if you wear these lenses.

Ofloxacin eye drops may cause eye irritation due to the benzalkonium chloride content. Avoid contact of ofloxacin with soft contact lenses. Remove these before application and wait at least 15 minutes before you put them back.

The eye drops can discolor soft contact lenses.

This drug for the oral administration contains lactose. If your doctor has told you that you have an intolerance to some sugars, contact him before taking it.

Before taking the drug     

Do not use ofloxacin

If you are allergic (hypersensitive) to ofloxacin or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you suffer a rare enzyme disorder called glucose-6-phosphate dehydrogenase deficiency

- If you are under 18 years old or you are still growing.

Take special care with ofloxacin

This medicine should be used with caution in patients with allergy to other antibacterial drugs belonging to the group of quinolones. If a skin rash or any other sign of possible allergic reaction occurs during treatment, stop treatment and consult your doctor.

In ocular application

Ofloxacin solution for ocular application should be used with caution in patients with an abnormality on the surface of the eye, including corneal ulcers. If this is the case, Consult your doctor.

Prolonged use of ofloxacin eye can lead to a new bacterial eye infection that could be not susceptible to treatment with ofloxacin.

In ear application:

Prolonged use of ofloxacin may lead to a growth of bacteria resistant to this active ingredient, including fungi. If the infection does not improve within a week, tell your doctorIf necessary, he will perform tests for sensitivity to other antibiotics.

If otorrhea persists (output secretions by the ear canal) after completing the treatment or if they occur two or more episodes of otorrhea during six months, consult your doctor. Hypersensitivity reactions (allergy)

In rare cases, patients receiving quinolones orally or intravenously have suffered severe or very severe hypersensitivity reactions, even after taking it for the first time, although only a few patients had a history of drug allergy. Some of these reactions are accompanied by cardiovascular collapse, loss of consciousness, tingling, angioedema with possible swelling of the larynx, pharynx, or face, airway obstruction, respiratory distress, urticaria and generalized itching.

Superinfection by Clostridium difficile:

Peripheral neuritis:

Glucose-6-phosphate dehydrogenase deficiency:

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

- Antacids containing magnesium or aluminum
- Glibenclamide (glyburide), an oral medication for the treatment of adult-onset diabetes
- Probenecid, a drug for the treatment of gout
- Cimetidine, an anti-ulcer drug
- Sucralfate, a drug used for antiulcer treatment

The drugs listed below can increase the risk of side effects:

- Antibiotics macrolide (erythromycin, clarithromycin) o azalide (azithromycin)
- Tricyclic antidepressants (imipramine, clomipramine, nortriptyline or amitriptyline)
- Theophylline, an anti-asthma drug used for bronchial diseases
- Antiarrhythmic, as amiodarone, quinidine, procainamide or disopyramide
- anti-inflammatory (NSAIDs), used for relief of pain and inflammation (ibuprofen, diclofenac, etc.)
- Corticosteroids
- Antipsychotic drugs, such as drugs used to treat schizophrenia or bipolar disorder.

There have not been conducted specific studies on interactions of ofloxacin for otic or ophthalmic use

Use with food and beverages

Do not drink alcohol while on oral or intravenous treatment with ofloxacin, or you may feel dizzy or drowsy.

Special physiological situations

Pregnancy and lactation:

Always consult your doctor or pharmacist before taking any medicine during pregnancy or breastfeeding.

You should not take this medicine if you are pregnant, think you might be, you can get pregnant or planning to become pregnant (Category C risk). This recommendation extends to all forms of presentation of the drug (oral, IV, for application to the eye or ear).

There are not available specific studies conducted on the use of topical presentations during pregnancy. It is justified only the ofloxacin use if, at the discretion of the physician, the potential benefit to the mother is greater than any risks to the fetus.

Children:

Have been established the efficacy and safety of ofloxacin in the pediatric population in the indications and ages listed below:

- Children six months or more for external otitis with eardrum intact
- Children of a year or more for acute otitis media with tympanostomy tubes
- Children aged twelve or more years with chronic suppurative otitis media with perforation of the eardrum.

For children below the age indicated it has not been established safety and efficacy of the drug in the indications mentioned.

Not established the efficacy and safety of the drug in solution for ocular use in children under one year of age.

The ofloxacin orally or intravenously should not be used in children and adolescents.

Elderly:

The elderly are more likely to suffer side effects from the use of this medicine and may require a closer clinical monitoring. No need to adjust the dose of ofloxacin depending on age.

Driving and using machines

You may feel drowsy, dizzy or have vision problems while taking this medication. If this happens, do not drive or operate any tools or machines.

How to use ofloxacin     

Follow carefully instructions of use of ofloxacin provided by your doctor. Consult your doctor or pharmacist when in doubt. Warning for local applications (ocular, otic):

Avoid contamination of the applicator or eyedropper material with discharge from the eye, ears, hands or other sources to maintain sterility of the otic or ophthalmic solution.

External Otitis:

Before administration of the drug, hot with a hand the solution for one or two minutes to prevent the instillation of cold liquid that could produce dizziness. The pinna should be facing upward and then the drops placed carefully in the duct, holding the position for five minutes to facilitate penetration of the drug. If necessary, repeat the operation on the opposite side.

The usual dose for children under 13 is 5 drops once a day in each affected ear for a week.

The usual dose for children 13 or more years is 10 drops once a day in each affected ear for a week.

Your doctor may change the dosage according to the circumstances of each case.

Acute Otitis media in children with tympanostomy tubes:

The usual dosage in children aged 1 to 12 years is 5 drops in each affected ear twice a day for 10 days. Remember heating the solution for one or two minutes with the hand before applying to prevent the child can get dizzy. Place the child lying on the side so that the ear canal is up. Once the dose is placed in the ear canal, you must press on the tragus (the prominence which faces the pinna) up to four times for the liquid to penetrate to the middle ear. Keep your child in that position for at least five minutes. then repeat on the opposite side if necessary.

Your doctor may change the dosage according to the circumstances of each case.

Chronic suppurative otitis media with ruptured eardrum:

The usual dose is 10 drops in the affected ear twice a day for two weeks. Remember heating the solution for one or two minutes with the hand before applying to prevent the child can get dizzy. Place the child lying on the side so that the ear canal is up. Once the dose is placed in the ear canal, you must press on the tragus (the prominence which faces the pinna) up to four times for the liquid to penetrate to the middle ear. Keep your child in that position for at least five minutes. then repeat on the opposite side if necessary.

Conjunctivitis:

For children over one year and adults, the usual dose is one or two drops in the diseased eye at intervals of two to four hours at the start of treatment to continue after the third or fourth day with an application every six hours. Do not put more drops of the indicated because the eye's ability is limited.

Bacterial Corneal Ulcers:

Apply one or two drops every half hour while awake. When going to bed, apply the same dose at 4 and 6 hours after lying down. From the third day of treatment, the interval between doses will be 1 hour during the day. From the week of treatment, you can apply one or two drops four or five times a day.

The doctor may modify these treatment guidelines taking into account your personal circumstances.

At 4 weeks of treatment, the container should be discarded.

If after a week you have not improved, discontinue treatment and consult your doctor again.

Tablets

Swallow the tablet whole with a glass of water. While in oral or IV treatment with ofloxacin avoid excessive sun exposure and using UV lamps.

Medicines containing antacids, sucralfate or iron should be avoided for two hours before taking ofloxacin until four hours later.

The drug dosage will be established by your doctor.

Depending on the severity of the infection, in most cases should be applied the following average dosages, including elderly:

- kidney or bladder infections: 200-800 mg/day
- Lung infections: 400-800 mg/day
- Infections of sexual organs (both sexes) : 400 mg. Gonorrhea is treated with a single dose of 400 mg ofloxacin.
- Infections of skin and soft tissue infections: 400-800 mg a day

The doses up to 400 mg can be taken in a single administration. In all cases, swallow the tablets whole with a glass of water.

The duration of treatment in routine cases is between 5 and 10 days, but can be longer; in no case more than two months.

Patients with renal or hepatic impairment should receive a proportional dose to the degree of kidney or liver disease.

Taking ofloxacin can influence the results of some tests, such as cultures of the tuberculosis bacillus, which can be falsely negative by the effect of the antibiotic. Taking this antibiotic may give false positive for opiates or porphyrins in urine results, so it should be checked by other means if necessary.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take ofloxacin     

If a doctor prescribes another medicine, tell you are taking ofloxacin.

If you feel unwell during treatment with ofloxacin, immediately consult your doctor.

If you take more than you should:

If you have taken too high a dose of ofloxacin, tell your doctor immediately or go to the emergency room of the nearest hospital. Take the pack and the rest of unconsumed medicine. The following side effects may occur: Feeling of confusion or dizziness, unconsciousness, convulsions or blood in the stool

Ofloxacin for eye application: If you use more medicine than you should, rinse the eyes with water and then continue your topical treatment

The accidental ingestion of ofloxacin in ophthalmic or otic solution provides only (in the worst cases) 30 mg ofloxacin. Although this amount is not relevant in terms of overdose, it can increase the risk of adverse reactions, so you should consult your doctor immediately.

If you forget to take ofloxacin

If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose.

Ofloxacin for local application (ocular, otic): In patients with eye or ear infection, if you miss a dose, apply it as soon as you remember and then continue with the plan. However, if it is almost time for the next dose, skip the missed dose and continue as usual.

Do not take a double dose to make up for forgotten individual doses.

If you stop taking ofloxacin

Side Effects of ofloxacin     

Like all medicines, ofloxacin can cause side effects although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Stop taking ofloxacin and contact your doctor immediately if you suffer an allergic reaction. This can be manifested by a rash (spots or hives on the skin), difficulty swallowing or breathing, swelling of lips, tongue or throat.

Stop the treatment and consult your doctor if you experience any of the following side effects as you may need urgent medical attention:

Very rare side effects:

- Irregular or rapid heartbeat, often accompanied by weakness
- Watery diarrhea, accompanied by abdominal cramps, fever, and sometimes Blood in the stool
- Seizures
- Hearing problems, including hearing loss
- Liver disorders, including yellowing of the skin or eye conjunctiva (jaundice)
- Severe skin rash with peeling or blisters around the lips, eyes, mouth, nose or genitals (Stevens-Johnson syndrome)
- Skin rash caused by exposure to sun or UV rays lamps
- Hypotension, which can cause weakness or fainting, dizziness, and drowsiness
- Muscle weakness or muscle or joint pain
- Hypoglycemia, manifested by weakness, irritability, sweating and trembling
- Hyperglycemia, manifested by excessive thirst, and water intake beyond usual
- Swelling or discomfort on a tendon, particularly the Achilles' tendon

Rare side effects

Uncommon side effects:

- Feeling sick, with diarrhea or stomach ache
- Headache, sleep problems, dizziness or restlessness
- Rash or itching

Rare side effects:

- Loss of appetite
- Confusion, anxiety, nightmares, see or hear things that are not there (hallucinations), depression or mental illness, drowsiness, tremor, problems walking due to lack of muscle control
- Vision changes
- Changes or loss of sense of taste or smell
- Altered levels of liver enzymes
- General ill feeling

Very rare side effects

- anemia (fewer red blood cells than normal), manifested by pallor, weakness, fatigue or dizziness
- Thrombocytopenia (fewer platelets than normal), which can manifests by bruises or bleeding more easily than usual
- Lung inflammation, which may be manifested by a cough or shortness of breath.

Porphyria:

This medicine may trigger an attack of porphyria, a rare metabolic disease. Tell your doctor if any of the side effects becomes severe or lasts more than a few days or has any side effects not listed in the leaflet that accompanies the medication.

Ofloxacin for otic application:

The most common side effect during treatment of external otitis is discomfort after application, which occurs in 7% of patients. If the symptom becomes important, you must stop treatment and consult your doctor again. Other side effects include itching, earache and dizziness.

In middle ear infections, the most common side effect is the bitter taste produced by the passage of fluid from the middle ear to the back of the throat. It occurs in 7% of patients. This side effect is not serious and is not cause for discontinuation. Other side effects in these patients include ear pain, abnormal sensations in the ear, itching, rash and dizziness.

Ofloxacin for ocular application:

Common side effects are transient burning and eye discomfort.

Other side effects include eye redness, itching, burning, eye pain, blurred vision, photophobia, feeling of eye dryness, foreign body sensation, tearing, inflammation and eye dryness, conjunctivitis or chemical keratitis (by ofloxacin), dizziness nausea, swelling of the face and general or local allergic reactions.

Although it rarely happens, ofloxacin for application to the eye or ear may lead to general side effects.
