Indications and usage     

Diazepam belongs to a group of drugs called benzodiazepines which acting on the central nervous system, being the most important effects of these drugs: sedation, hypnosis, decreased anxiety, muscle relaxation, and anticonvulsant activity.

Doctors prescribe diazepam to suppress symptoms caused by anxiety, agitation, and psychic tension. It can also be useful for relief of acute agitation, tremors and hallucinations if you have an alcohol withdrawal syndrome.

Doctors can also prescribe diazepam as adjuvant treatment for relief of musculoskeletal pain (inflammation of muscles or joints, trauma, etc.). Can also be used to combat diseases caused by spasticity due to disorders from both spinal as supraspinal origin, such as cerebral palsy and paraplegia as well as athetosis and generalized stiffness syndrome.

Diazepam may be used as adjuvant treatment of seizure disorders. In these cases, your doctor should periodically evaluate the usefulness of this drug.

Treatment should start with the lowest dose. Do not exceed the maximum dose.

Diazepam is indicated for the treatment of febrile seizures in children over 1 year (about 10 kg) and epileptic seizures; in states of anxiety, tension, and anguish or as a sedative in minor surgery and diagnostic or endoscopic procedures.

Other ingredients of available commercial presentations:

Tablets

Each tablet contains diazepam. The other ingredients (excipients) are microcrystalline cellulose, calcium hydrogen phosphate, sodium starch glycolate, magnesium stearate and white talc.

Ampoules

Each 2 ml (1 ampoule) contain 10 mg diazepam. The other ingredients are ethanol (170.5 mg), benzyl alcohol (31.4 mg), propylene glycol, 0.1N sodium hydroxide, benzoic acid, sodium benzoate and water for injection.

Oral drops

Each ml of solution contains 2 mg of diazepam. The other ingredients are sucrose, sodium saccharin, ethanol (400 mg) and purified water.

Micro-enema

In addition to the active ingredient, the excipients include benzoic acid (E-210). ethanol, 250 mg per dose, propylene glycol, sodium benzoate (E-211), benzyl alcohol, 37.5 mg per dose and purified water.

Important information     

Ampoules:

This medicine contains 8.5% ethanol. Each ampoule contains 0.17 g of ethanol, which can be a risk in children, pregnant women, and patients with liver disease, alcoholism, epilepsy, brain damage or injuries and can modify or enhance the effect of other drugs.

By containing benzyl alcohol as an excipient, this drug is contraindicated in children under three years. By containing benzoic acid and sodium benzoate as excipients can irritate the eyes, skin and mucous slightly. It can increase the risk of yellowing of the skin (jaundice) in newborns.

Oral drops

Children

This medicine contains 50% ethanol in the final volume. Each ml contains 0.4 g of ethanol which may constitute a risk if is administered to children, since ethanol may modify or increase the effect of other drugs.

Adults

This medicine contains 50% ethanol final volume. Each ml contains 0.4 g of ethanol. You should exercise caution and not take it if you are pregnant, have liver disease, epilepsy, alcoholism or brain damage or injury, since ethanol may modify or enhance the effect of other drugs. In addition, you can suffer a decrease in responsiveness, so caution should be exercised if you are driving or operating machinery.

Micro-enema

By containing benzoic acid and sodium benzoate as excipient can irritate the eyes, skin and mucous slightly. It can increase the risk of yellowing of the skin in newborns (jaundice).

By containing benzyl alcohol should not be given to premature or newborn babies. It may cause toxic reactions and allergic reactions in children under 3 years of age.

This medicine contains 12% ethanol (alcohol); that means up to 250 mg per dose, equivalent to 2.5 ml wine per dose. It can be harmful to those patients suffering from alcoholism and should be taken into account in pregnant or nursing women, children and high-risk groups such as patients with liver disease or epilepsy.

By containing propylene glycol can produce similar symptoms to those of alcohol, so it can reduce the ability to drive or operate machinery.

Before taking the drug     

Do not use diazepam

If you are allergic (hypersensitive) to diazepam, other benzodiazepines or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you experience severe muscle or liver problems

Take special care with diazepam

Tolerance

If you are using this medicine continuously for a few weeks, may detect a certain degree of loss of efficacy with respect to hypnotic effects.

Dependence

Taking this medicine may cause the development of physical and psychic drug dependence. The risk of dependence increases with dose and the time you are taking this medicine.

This risk of dependence is also greater if you have a history of drug or alcohol abuse.

Once physical dependence has developed, if you stop abruptly the medication, may experience withdrawal symptoms such as restlessness, anxiety, insomnia, poor concentration, headaches, and flushing.

To minimize the risk of dependence, you should take these precautions:

Amnesia

Taking other medicines

Pregnancy and lactation

How to use diazepam

The usual recommended doses are as follow:

Adults:

Anxiety's symptoms:

2-10 mg, 2 to 4 times a day, depending on the doctor's instructions

Symptomatic relief in acute alcohol withdrawal:

10 mg, 3 or 4 times during the first 24 hours, reduced to 5 mg. 3 or 4 times daily, as needed.

Adjuvant for relief of musculoskeletal spasm:

2 to 10 mg 3 or 4 times daily.

Adjuvant in anticonvulsant therapy::

2 to 10 mg, 2 or 4 times daily.

Elderly or patients with debilitating diseases:

2 to 2.5 mg, 1 or 2 times a day, then will gradually increase as needed and tolerated.

In the treatment with diazepam in patients with liver or kidney disorders, special attention should be paid to individualizing the dose.

Children:

2 to 2.5 mg, 1 or 2 times a day, gradually increasing according to needs and tolerance; as general rule 0.1-0.3 mg/kg daily.

Because of the wide variety of response of children to drugs acting on the CNS, it should begin treatment with the lowest dose, being increased as required.

Do not use in children under 6 months of age.

Remember to take the medicine. Diazepam is administered orally.

Take the medicine swallowing the tablet without chewing, with water or another non-alcoholic beverage.

Each individual dose should not exceed the limits indicated and the total daily dose either, unless your doctor may prescribe a higher dose.

Your doctor will tell you how long your treatment with diazepam, which will be as short as possible. Do not stop treatment earlier, as you could not get the expected beneficial results .

In general, the total duration of treatment should not exceed 8-12 weeks, including the time needed for the gradual withdrawal of diazepam.

Never change by yourself the dose that you have been prescribed.

Micro-enemas for rectal administration:

Dose:

The dose for children weighing 10-15 kg (approximately 1-3 years) is 5 mg. In this case, it is recommended to use a dosage form containing 5 mg.

The dose for children over 16 Kg (about 3 years) is 10 mg.

The dose for adults is 10 mg in most cases.

The recommended dose in elderly and patients with hepatic failure is 5 mg. In this case, it is recommend using a dosage form of 5 mg.

While you take diazepam

In the case of overdose or accidental ingestion; consult the poison control center in your country.

If you forget to take diazepam

If you miss a dose you should never try to correct it by taking a double dose the next time, but on the contrary, must continue with the usual schedule.

If you stop taking the medication abruptly, you may experience withdrawal symptoms such as restlessness, anxiety, insomnia, poor concentration, headaches, and flushing.

Side Effects of diazepam     

Like all medicines, diazepam can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Most patients tolerate diazepam, but some, especially early in treatment, are a bit tired or sleepy during the day.

Other adverse reactions such as gastrointestinal disturbances, changes in libido or skin reactions, can occur occasionally.

You can develop an anterograde amnesia, ie you do not remember what happened while you were awake after taking the drug, therefore you need to ensure that you can rest for at least seven hours without being disturbed.

Very rarely, this medicine may cause sudden anxiety, hallucinations, excitement and sleep disorders.
