Diclofenac is used to treat the symptoms of:

- Strains, sprains and other injuries
- Pain and inflammation following surgery
- Dysmenorrhea (menstrual cramps),
- Renal colics,
- Acute gout attacks
Other painful conditions affecting the joints and muscles such as a backache, rheumatoid arthritis, osteoarthritis, ankylosing spondylitis, and pyrophosphate arthropathy.

This medicine can also be used to relieve the symptoms associated with migraine attacks in adults.

Diclofenac belongs to a group of medicines called non-steroidal anti-inflammatory drugs (NSAIDs), which are used to reduce pain and inflammation in the conditions listed above.

Ophthalmic solution:

The active ingredient is diclofenac sodium (1 mg/ml). The other ingredients are macrogol ricinoleate, glycerol, tromethamine, boric acid and water.

50 mg tablets

Each tablet contains 50 mg of active substance. The other ingredients include lactose (30 mg), sodium starch glycolate, povidone, microcrystalline cellulose, colloidal silica, magnesium stearate, corn starch, hydroxypropylmethylcellulose, polyethylene glycol, titanium dioxide, talc, iron oxide red, iron oxide yellow, methacrylic acid and triacetin.

50 mg orodispersible tablets

Each dispersible tablet contains 50 mg (equivalent to 46.5 mg of diclofenac sodium). The other ingredients include microcrystalline cellulose, sodium carboxymethyl starch potato without gluten, povidone, croscarmellose sodium, colloidal anhydrous silica, hydrogenated castor oil, and talc.

100 mg delayed-release tablets

Each tablet contains 100 mg of active substance. The other ingredients (excipients) include hydroxypropylmethylcellulose, microcrystalline cellulose, methacrylic acid and methyl methacrylate, ammonium methacrylate copolymer type B, talc, povidone, corn starch, magnesium stearate.

Solution for injection 75 mg/3 ml:

Each ampoule contains 75 mg of diclofenac sodium. Other ingredients are: For a 3 ml ampoule: benzyl alcohol 119 mg, 9 mg sodium metabisulfite, propylene glycol, 3.8 mg sodium hydroxide and water for injection.

100 mg suppositories:

Each suppository contains 100 mg sodium diclofenac. Other ingredients include a mixture of triglycerides of saturated fatty acids.

Ophthalmic solution:

This medicine in ophthalmic solution contains ricinoleate drug, which can cause contact dermatitis (skin reactions).

50 mg tablets

This medication may contain lactose. If your doctor has told you that you have an intolerance to some sugars, consult your doctor before taking it.

50 mg orodispersible tablets

This drug contains hydrogenated castor oil as an excipient, that can cause nausea, vomiting, colic, and at high doses, severe purging. Do not administer to patients with intestinal obstruction.

Solution for injection 75 mg/3 ml:

This medication contains benzyl alcohol as an excipient. By this reason, this drug is contraindicated in children under three years. This product contains sodium metabisulfite as excipient which may cause allergic-type reactions including anaphylactic reactions and bronchospasm in susceptible patients, especially those with asthma or allergic history.

You should not take this drug if you are allergic to diclofenac or some other anti-inflammatory drug. Allergy to one of these medications implies the allergy to the whole therapeutic group, including aspirin.

Many drugs used for minor ailments and frequently purchased without prescription may contain substances related chemically to diclofenac. Tell your doctor or pharmacist if you are allergic to aspirin or any anti-inflammatory and painkiller drug.

Allergy to NSAIDs or aspirin is potentially serious and may endanger the patient's life,although previous allergy symptoms have been mild.

Do not use diclofenac

If you are allergic (hypersensitive) to the active substance or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Also, you should not take this medicine:

-If you previously had a bleeding stomach or duodenum or suffered a perforation of the digestive system while taking an anti-inflammatory non-steroidal drug.
- If you currently have or have had on more than one occasion: an ulcer or bleeding of the stomach or duodenum.
- If you have an inflammatory bowel disease such as Crohn's disease or ulcerative colitis.
- If you have liver or kidney disease.
- If you have bleeding disorders.
- If you have severe heart failure.
- If you are in the third trimester of pregnancy
- If you have symptoms suggesting blood loss from the gastrointestinal tract (vomiting blood or if coffee grounds, rectal bleeding or black stools or bloody diarrhea).

Take special care with diclofenac

Tell your doctor if you have:

- Asthma.
- Arterial hypertension.
- Kidney, liver or heart disease.
- Gastrointestinal disorders or specific bowel diseases (Crohn's disease, etc.).
- Bleeding tendency.
- Other diseases, such as anemia or porphyria.

In the case of an active infection, concurrently with taking diclofenac, can be damped much or even be hidden some of the symptoms, such as fever or pain, so the worsening of your infection may be lower than really.

It is important to use the smallest dose that relieves pain and should not take this medicine for longer than necessary to control the symptoms.

These drugs can cause fluid retention, particularly in patients with heart failure and/or high blood pressure (hypertension).

Laboratory tests: Frequent liver and kidney function tests and monitoring of blood counts are necessary if taken for more than a few days.

Your doctor also will monitor renal function, especially in patients with impaired kidney or heart, elderly patients, patients on diuretics and patients before and after surgery.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Taking other medicines

Please tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including medicines obtained without a prescription. Especially:

- Medicines to treat diabetes. A dose adjustment of these medicines may be necessary as blood sugar may drop too low.
- Anticoagulants, (e.g. warfarin, acenocoumarol). These medicines may increase the risk of bleeding.
- Diuretics (water tablets). The effect of these may be decreased. Potassium-sparing diuretics may increase the potassium levels in the blood.
- Lithium. The blood levels of these medicines may be increased if taken with diclofenac.
- Cytotoxic medicines (e.g. methotrexate to treat cancers). These drugs should not be taken less than 24 hours before or after diclofenac potassium tablets - the blood levels of these medicines may be increased if taken with diclofenac.
- Cyclosporine. This drug taken with diclofenac may harm kidney function.
- Quinolones, a group of antiinfective medicines (e.g. ciprofloxacin or levofloxacin). The association can produce fits (convulsions).
- Corticosteroids. May increase the risk of bleeding in the stomach.
- Other NSAIDs, including aspirin. These may increase the risk of side effects.
- Medicines to treat high blood pressure (ACE inhibitors, beta blockers). Diclofenac can reduce the blood pressure lowering of these medicines.
- Mifepristone. Its effects may be reduced by NSAIDs.
- Cardiac glycosides, as digoxin, used to treat heart failure. The use of diclofenac with these medicines may worsen heart failure or increase blood levels of cardiac glycosides.
- Tacrolimus, an immunosuppressant. As in the case of cyclosporine may increase the risk of kidney damage.
- Zidovudine, an antiretroviral drug used to treat HIV infection. In combination with diclofenac, may increase the risk of blood disorders.

Use with food and beverages

Special physiological situations

Pregnancy and lactation

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

It is not recommended to take diclofenac during the first 6 months of pregnancy. However, the doctor may prescribe diclofenac during the first six months of pregnancy if he considers that the benefit for the mother outweighs the risk to the fetus. However, you must avoid taking diclofenac during the last 3 months of pregnancy since a damage to the fetus and a reduction of delivery labor may occur.

It should be noted that diclofenac passes into the milk, so it must be carefully assessed the risk for the baby against the potential benefits to the mother. May be administered diclofenac at low doses (50-75 mg/day) if needed. The improvement of symptoms usually begins few hours after administration, although the peak effect may take several days or even weeks, depending on the disease.

If you forget to take the medicine

If you have missed several doses, consult your doctor for instructions about dosage schedule to follow to reach your maintenance dose.

Do not take a double dose to compensate forgotten individual doses.

If you stop taking diclofenac

Consult your doctor about treatment duration. If you stop taking diclofenac earlier than prescribed by your doctor, you can feel the symptoms of the disease that motivate its administration.

If you suffer from any of the following at any time during your treatment, stop taking the medicine and seek immediate medical help:

- Blood in the feces,
- Black tarry stools,
- Vomit any blood or dark particles that look like coffee grounds,
- An allergic reaction such as itching, low blood pressure, swelling of the face, lips, tongue, mouth, and throat, which may cause shortness of breath or difficulty swallowing,
- A type of meningitis (aseptic) causing a combination of symptoms such as headaches, fever, stiff neck, tiredness, muscle pain, sore throat and disorientation.
- Yellowing of the skin or the whites of the eyes,
- Stomach pain, indigestion, heartburn, meteorism, nausea, vomiting or other abnormal stomach symptoms.

Stop taking the medicine and tell your doctor if you experience:

- Any type of fit or seizure
- An unexpected change in the amount of urine

Tell your doctor if you experience any of the following symptoms:

Common side effects:

- Headaches, dizziness, ’spinning’ sensation,
- Diarrhea, weight loss or poor appetite,
- Abnormal liver function tests,
- Skin rashes

Rare side effects:

- Tiredness,
- Stomach ulcers or bleeding,
- Hepatitis,
- Itching,
- Fluid retention (symptoms of which include swollen ankles).

Very rare side effects:

- Sensations of pins and/or needles, tremor, blurred or double vision, hearing disorders (loss or impairment), tinnitus (ringing in the ears), difficulty sleeping, nightmares, depression, irritability, anxiety, psychotic reactions, disorientation, loss of memory, numbness, hypersensitivity to light, taste disturbance,
- Constipation, inflammation of the tongue, mouth ulcers, ulcers of the gullet, lower gut disorders (including inflammation of the colon causing diarrhea and stomach pains), pancreatitis (inflammation of the pancreas), worsening of ulcerative colitis or Crohn’s disease,
- Palpitations (fast or irregular heart beat), chest pain, high blood pressure, inflammation of blood vessels (vasculitis),
- Inflammation of the lung (pneumonitis),
- Congestive heart failure,
- Blood disorders (including anemia, making you tired and more prone to minor infections or bleeding),
- Kidney or liver disorders, blood or proteins in the urine,
- Skin rash, itching, skin eruptions, eczema, erythema multiforme (round red patches on the skin), Stevens-Johnson-Syndrome (severe skin rash with flushing, fever, blisters and ulcers), or Lyell’s Syndrome (severe rash with reddening, peeling and swelling of skin that looks like severe burns), hair loss,
- Impotence (difficulty getting an erection).

Medicines such as diclofenac may be associated with a small increased risk of heart attack (e.g. myocardial infarction) or stroke.
