Indications and usage     

The active ingredient escitalopram is indicated for the treatment of depression (major depressive episodes) and anxiety disorders (such as panic or anguish disorder with or without agoraphobia, social anxiety disorder, generalized anxiety disorder and obsessive-compulsive disorder).

Escitalopram belongs to a group of antidepressants called selective reuptake inhibitors (SSRIs). These medicines act on the serotonin system in the brain by increasing the serotonin level. Disturbances in the serotonin system are considered an important factor in the development of depression and related diseases.

Medizzine recommends you consult your doctor if you do not know why has prescribed escitalopram.

Other ingredients of available commercial presentations:

Tablets

Each tablet contains 10, 15 or 20 mg escitalopram as oxalate. The other ingredients are microcrystalline cellulose, colloidal anhydrous silica, talc, croscarmellose sodium and magnesium stearate. The coating contains hypromellose, macrogol 400 and titanium dioxide (E171).

Oral drops 20 mg/ml:

1 drop contains 1 mg escitalopram. The other ingredients are: Propyl gallate, citric acid anhydrous ethanol 96%, sodium hydroxide and purified water

Warning:

Additional warning     

Oral drops: This medicine contains small amounts of ethanol (alcohol), less than 100 mg per dose. Each drop contains 4.7 mg ethanol.

Before taking the drug     

Do not use escitalopram

If you are allergic (hypersensitive) to the active substance escitalopram or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Also, you should not take this medicine:

If you take other medicines that belong to a group called MAO inhibitors, including selegiline (used to treat Parkinson's disease), moclobemide (used to treat depression) and linezolid (an antibiotic).

Do not take escitalopram if any of the above circumstances can be applied to you.

Take special care with escitalopram

Tell your doctor if you have any other condition or illness, as your doctor may need to take this into account.

Specifically, tell your doctor:

- If you have epilepsy. Escitalopram treatment should be stopped if seizures occur or there is an increase in seizure frequency.

- If you suffer liver or renal failure. Your doctor may need to adjust the dose.

- If you suffer diabetes. Treatment with escitalopram may alter glycemic control. May be needed adjusting the dose of insulin and/or oral hypoglycemic agents.

- If you have a decreased level of sodium in the blood.

- If you have bleeding or bruise easily.

- If you are receiving electroconvulsive therapy.

- If you have coronary disease.

Keep in mind:

- Some patients with manic-depressive illness may enter into a manic phase. This is characterized by a fast and uncommon change of ideas, inappropriate happiness and excessive physical activity. If you experience these symptoms, contact your doctor.

Symptoms such as restlessness or difficulty to sit or stand can also occur during the first weeks of treatment. Tell your doctor immediately if you experience these symptoms.

Thoughts of suicide and worsening of your depression or anxiety disorder

If you are depressed and/or have an anxiety disorder, you can sometimes have thoughts of hurting or kill yourself. These may gradually increase if you are taking antidepressants for the first time, since these medicines take a time to start its effects, usually about two weeks, although in some cases, may be longer.

You may be more likely to have these kinds of thoughts:

- If you have had thoughts about self-harm (including suicide).
- If you are a young adult. Information from clinical trials has shown an increased risk of suicidal behavior in adults younger than 25 years with psychiatric conditions who were treated with an antidepressant.

If at any time you have thoughts of harming or killing yourself, contact your doctor or go directly to a hospital.

It may be helpful to tell a relative or close friend that you are depressed or have an anxiety disorder and ask him to read the prospectus included in the package. You can ask if he thinks your depression or anxiety disorder has worsened or if he is worried about your changes in attitude.

If this situation is applicable to you or you are not sure, tell your doctor before taking this medicine.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription. It is important, especially:

Tell your doctor if you are taking any of these medicines:

- Non-selective monoamine oxidase inhibitors (MAOI), containing phenelzine, iproniazid, isocarboxazid, nialamide and tranylcypromine as active ingredients. If you have taken any of these medicines will need to wait 14 days before you start taking escitalopram. After finishing with escitalopram, you must wait 7 days before taking any of these medicines.

- Selective inhibitors of MAO-A reversible, containing moclobemide (used in treating depression).

- Lithium (used in the treatment of manic-depressive disorder) and tryptophan.

- Imipramine and desipramine (both used to treat depression).

- Sumatriptan and similar medicines (used to treat migraines) and tramadol (used against severe pain). These increase the risk of side effects.

- Cimetidine and omeprazole (used to treat stomach ulcers), fluvoxamine (antidepressant), and ticlopidine (used to reduce the risk of stroke). These may cause increased blood concentrations of escitalopram.

- Hypericum (St. John's wort), an herbal medicine used for depression.

- Acetylsalicylic acid and anti-inflammatory drugs (NSAIDs).

- Warfarin, acenocoumarol, dipyridamole and phenprocoumon (medicines used to reduce the risk of thrombosis) .Your doctor will probably check the INR during treatment with escitalopram to correct the dose of anticoagulant if needed.

- Mefloquine (used to treat malaria), bupropion (used to treat depression) and tramadol (used to treat severe pain) due to a possible risk of a lowered threshold for seizures.

- Neuroleptics (medicines used to treat schizophrenia, psychosis) due to a possible risk of a lowered threshold for seizures of antidepressants.

- Flecainide, propafenone and metoprolol (used in cardiovascular diseases), clomipramine and nortriptyline (antidepressants) and risperidone, thioridazine and haloperidol (antipsychotics). It may be necessary to adjust the escitalopram dose

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Use with food and beverages

Escitalopram may be taken with or without food. As with many drugs, the combination of escitalopram with alcohol is not advisable, although not expected any interaction with this substance.

Special physiological situations

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy and lactation:

Tell your doctor if you are pregnant or planning to become pregnant. Do not take escitalopram if you are pregnant unless you and your doctor have discussed the risks and benefits involved.

If you take escitalopram during the last three months of your pregnancy be aware that the following effects can be observed in the newborn baby:

Difficulty breathing, bluish skin, fits, body temperature changes, feeding difficulties, vomiting, low blood sugar, stiff or floppy muscles, vivid reflexes, tremor, jitteriness, irritability, lethargy, constant crying, sleepiness and sleeping difficulties. If your newborn baby has any of these symptoms, please contact your doctor immediately.

If you take escitalopram during pregnancy, you should never stop treatment abruptly.

Do not take escitalopram if you are breastfeeding unless you and your doctor have discussed the risks and benefits of this medication.

Use in children and adolescents under 18

Escitalopram should not normally be used in the treatment of children and adolescents under 18 years. Also, you should know that patients under 18 have an increased risk of adverse effects such as suicide attempts, suicidal thoughts and hostility (predominantly aggression, oppositional behavior and anger) when they take this class of medicines. Despite this, your doctor may prescribe escitalopram for patients under age 18 when he decides which is the most convenient for the patient.

If the doctor has prescribed escitalopram to a patient under 18 and you want to discuss this, please go back to talking to your doctor.

You should inform your doctor if any of the symptoms listed above develop or worsen when a patient under 18 are taking escitalopram. At the same time, the long-term effects of treatment with regard to the safety of use, growth, maturation and cognitive and behavioral development of patients of this age group have not yet been demonstrated.

How to use escitalopram     

Follow carefully the instructions of use of escitalopram provided by your doctor. Consult your doctor or pharmacist when in doubt. Adults

Depression

The usual recommended dose is 10 mg taken as an only daily dose. Your doctor may increase up to a maximum of 20 mg daily.

Anguish Disorder

Starting dose is 5 mg as an only daily dose for the first week, before increasing the dose up to 10 mg daily. Your doctor may then increase it up to a maximum of 20 mg daily.

Social anxiety disorder

The usual recommended dose is 10 mg taken as an only daily dose. Your doctor may reduce this dose up to 5 mg per day or increase it up to 20 mg a day, depending on how you respond to treatment.

Generalized Anxiety Disorder

The usual recommended dose is 10 mg taken as an only daily dose. This dose may be increased by your doctor up to a maximum of 20 mg daily.

Obsessive-compulsive disorder

The usual recommended dose is 10 mg taken as an only daily dose. This dose may be increased by your doctor up to a maximum of 20 mg daily.

Elderly (over 65)

Normal recommended starting dose is 5 mg taken as an only dose per day.

Children and adolescents (under 18)

Normally, escitalopram should not be given to children and adolescents.

You can take escitalopram with or without food. Swallow the tablets with water. Do not chew because it tastes bitter.

If necessary, you can divide the tablets by placing them on a flat surface with the slit upwards. The tablets may be broken by pressing down each end of the tablet, with the two forefingers .

Duration of treatment

It may take a couple of weeks before you start to feel better. Continue to take escitalopram even if you begin to feel better ahead of schedule.

Do not change the dose of medication without talking to your doctor.

Continue to take escitalopram as recommended by your doctor. If you stop too soon the medicine, the symptoms may reappear.

It is recommended to continue treatment for at least 6 months after you feel well again.

If you think that the effect of the medicine is too strong or too weak, consult your doctor.

While you take escitalopram     

If you become pregnant, contact your doctor immediately.

If a doctor prescribes another medicine , tell you are taking escitalopram.

Check with your doctor regularly development of disorder that motivates the use of escitalopram. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with escitalopram at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with escitalopram, immediately consult your doctor.

If you use more medicine than you should

If you take more dose of escitalopram than prescribed, immediately contact your doctor, go to the emergency room of the nearest hospital or contact the Poison Control Center in your country. Do it even if you do not observe signs of discomfort or poisoning.

Signs of overdose may include dizziness, tremor, agitation, convulsion, coma, nausea, vomiting, changes in heart rate, decreased blood pressure and changes in the body content of salt and water. Carry with you the medicine package and the unconsumed tablets if you go to the doctor or hospital.

If you forget to take escitalopram:

Do not take a double dose to compensate for missed doses. If you miss a dose, and you remember before you go to bed, take it straight away. The next day take your dose as usually. If you remember the night or the next day, skip the missed dose and continue as usual.

If you stop taking escitalopram

Do not stop treatment with escitalopram until your doctor tells you.

When you have finished your course of treatment, it is generally recommended that the dose of escitalopram be reduced gradually over several weeks.

When you stop taking escitalopram, especially if it is abrupt, you may feel withdrawal symptoms. These are common when treatment with escitalopram is suspended. The risk is greater when it has been used for a long time or in high doses or when the dose is reduced too quickly.

Most people find that these symptoms are mild and go away on their own within two weeks. However, in some patients may be severe or prolonged (2-3 months or more).

If you have severe withdrawal symptoms when stop taking escitalopram, contact your doctor. He may recommend you start taking your tablets again and reduce the treatment more slowly.

Withdrawal symptoms include:feeling dizzy (instability or loss of balance), tingling, burning sensation and (less commonly) electric shock, even in the head, sleep disturbances (vivid dreams, nightmares, inability to sleep), feeling of restlessness, headache, feeling sick (nausea), sweating (including night sweats), feeling restless or agitated, tremor (shakiness), feeling of confusion or disorientation, feelings of excitement or irritation, diarrhea (loose stools), visual disturbances, rapid pulse or palpitations.

Side Effects of escitalopram     

Like all medicines, escitalopram can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

The side effects usually disappear after a few weeks of treatment. You should be aware that many of the effects may be symptoms of your illness and therefore will improve when you start feeling better.

Consult your doctor if you have any of the following side effects during treatment:

Uncommon side effects:

Unusual bleeding, including gastrointestinal bleeding.

Rare side effects:

If you experience swelling of skin, tongue, lips or face (angioedema), or you have difficulty breathing or swallowing (allergic reaction), contact your doctor or go to hospital immediately.

If you have high fever, agitation, confusion, tremors and sudden contractions of muscles, may be signs of a rare condition called serotonin syndrome. If you feel like this, contact your doctor.

If you experience the following side effects, should contact your doctor or go to the hospital immediately:

- Difficulty urinating.
- Convulsions (seizures),
- Yellowing of skin and whites of the eyes (jaundice), are signs of impaired liver function or hepatitis.

In addition to the above, there have been reported the following side effects:

Very common side effects:

Feeling sick (nausea).

Common side effects:

- Clogging or a runny nose (sinusitis).
- Decreased or increased appetite.
- Anxiety, agitation, abnormal dreams, difficulty sleeping feel asleep, dizziness, yawning, tremors, itchy skin.
- Diarrhea, Constipation, vomiting, dry mouth.
- Increased sweating.
- Muscle and joint pain (arthralgia and myalgia).
- Sexual disorders (delayed ejaculation, problems with erection, decrease sexual impulse and women may experience difficulties achieving orgasm).
- Fatigue, fever.
- Weight gain.

Uncommon side effects:

- Urticaria, skin rash, itching (pruritus).
- Gnashing of teeth, agitation, nervousness, anguish attacks state of confusion.
- Sleeo disorders, taste disorders, fainting (syncope).
- Dilation of pupils (mydriasis), visual disturbance, ringing in the ears (tinnitus).
- Hair loss.
- Vaginal bleeding.
- Weight loss.
- Rapid heartbeat.
- Swelling of arms and legs.
- Nose bleeding.

Rare side effects:

- Aggression, depersonalization, hallucinations.
- Low heart rate.

Some patients have reported (frequency not known):

Thoughts of harming yourself or thoughts of killing yourself

Decreased levels of sodium in the blood (symptoms include feeling sick and unwell with weak muscles or confusion).

Dizziness when standing due to low blood pressure (orthostatic hypotension).

abnormal liver function tests (increase in liver enzymes in the blood).

Movement disorders (involuntary movements of the muscles).

Painful erections (priapism).

Bleeding disorders including skin and mucous bleeding (ecchymosis) and low levels of blood platelets (thrombocytopenia).

Sudden swelling of skin or mucosa (angioedema).

Increased the amount of urine excreted (inappropriate secretion of ADH).

Flow of milk in women who are not breastfeeding (galactorrhea).

Mania.

Other known side effects associated with drugs that act similarly to escitalopram are:
