Indications and usage     

Etoricoxib is a nonsteroidal anti-inflammatory drug (NSAID). It is a member of a class of NSAIDs known as selective inhibitors of cyclooxygenase-2 (COX-2).

Cyclooxygenase is an enzyme responsible for the generation of prostaglandins (chemicals that occur naturally in the body which can produce pain and inflammation). The production of prostaglandins by COX-2 enzyme is increased in certain diseases such as arthritis. By blocking COX-2, etoricoxib reduces the amount of prostaglandins produced and reduces pain and inflammation.

Etoricoxib is not a narcotic.

Your doctor has prescribed these tablets to relieve symptoms of osteoarthritis, rheumatoid arthritis or gout.

Other ingredients of available commercial presentations:

The tablets contain 60, 90 or 120 mg of etoricoxib. The other ingredients are calcium hydrogen phosphate (anhydrous), croscarmellose sodium, magnesium stearate, microcrystalline cellulose (core) and carnauba wax, lactose monohydrate, hypromellose, titanium dioxide, glycerol triacetate. Tablets of 90 mg and 120 mg also contain yellow iron oxide (E172, coloring agent) and indigo carmine (E132, coloring agent) (tablet coating).

Important information     

These tablets contain lactose. If your doctor has told you that you suffer an intolerance to some sugars, contact him before taking it.

Before taking the drug     

Do not use etoricoxib

If you are allergic (hypersensitive) to etoricoxib or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you have an active ulcer or bleeding of the stomach or duodenum
- If you have severe liver disease
- If you have severe kidney disease
- If you have previously had any of the following symptoms after taking aspirin (acetylsalicylic acid) or other anti-inflammatory drugs, including COX-2 inhibitors:

Asthma (wheezing or difficulty to breath easily), nasal irritation, nasal polyps, or allergy symptoms such as swelling of the face, lips, tongue or throat (angioedema) and/or rash itchy

- If you are or may be pregnant, or are breastfeeding,
- If you are under 16 years of age
- If you have inflammatory bowel disease
- If your doctor has diagnosed you have heart problems including heart failure (moderate or severe types), angina (chest pain) or if you have had a myocardial infarction, surgical revascularization (bypass surgery), peripheral arterial disease (poor circulation in legs or feet due to narrow or blocked arteries) or any type of stroke (including transient stroke or transient ischemic attack).

Etoricoxib may slightly increase your risk of heart infarction and stroke and therefore it should not be used in patients who have or have had heart problems or stroke

- If you have high blood pressure that has not been controlled with treatment (consult your doctor or nurse if you are not sure whether your blood pressure is adequately controlled)

If you think you are in any of these circumstances, do not take the tablets until you have consulted with your doctor.

If are not sure if any of this is applicable to you, ask your doctor or pharmacist.

Take special care with etoricoxib

If any of the followings is applicable to you, etoricoxib may not be suitable for you or may need to be monitored regularly while taking it:

- You have a history of ulcers or stomach bleeding
- You are dehydrated, for example, following a prolonged bout of vomiting or diarrhea
- You have swelling due to fluid retention (edema)
- You have a history of heart failure, heart attack or any other form of heart disease
- You have a history of stroke or mini-stroke (transient)
- You have a history of high blood pressure
- You have a history of liver or kidney disease
- You are being treated for an infection. Etoricoxib can mask or hide a fever, which is a sign of infection.
- You are a woman trying to become pregnant
- You are elderly (ie. more than 65 years)
- You have diabetes or high cholesterol or are a smoker, as these can increase the risk of suffering heart disease.
- You have a history of allergies to other drugs. Some COX-2 inhibitors have been associated with an increased risk of skin reactions in patients with a history of any drug allergy.

The following are things you should know while taking etoricoxib:

1. If you are elderly or have a kidney, liver or heart diseases, your doctor will recommend regular checks.

2. If you develop any symptoms that could indicate liver problems, as a yellowing of the skin and eyes (jaundice), it is important to contact your doctor as soon as possible.

3. If you develop severe and persistent abdominal pain and/or your stools become black, should urgently seek medical advice.

4. If appear or begin to worsen any of the following symptoms: shortness of breath, chest pain or ankle swelling, discontinue treatment with etoricoxib and consult your doctor.

5. If you develop any symptoms that could indicate a severe allergic reaction such as an inability to breathe or skin reactions such as ulcers or blistering, should see a doctor urgently.

It is important you use the lowest dose possible which controls your pain. Should not take etoricoxib for longer than necessary. This is because the risk of heart attacks and strokes might increase after prolonged treatment, especially with high doses.

6. Etoricoxib can increase blood pressure in some people, especially in high doses, and this could increase the risk of heart attacks and strokes.

During treatment, your doctor will control potential risks, to ensure it is safe to continue treatment.

If you have any doubts about this, check with your doctor before taking etoricoxib.

If any of these circumstances is applicable to you, tell your doctor before taking it.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any medications, even those without a prescription or herbal remedies.

In particular, if you are taking any of these medicines, your doctor may want to monitor if your medicines are working properly, once you start taking etoricoxib:

- Blood thinners such as warfarin or other oral anticoagulants
- Rifampicin (an antibiotic)
- Methotrexate (a drug used to suppress the immune system and often used in rheumatoid arthritis)
- ACE inhibitors and angiotensin receptor antagonists (drugs for treating high blood pressure and heart failure)
- Lithium (a medicine used to treat some types of depression)
- Diuretics (water tablets)
- Cyclosporine or tacrolimus (medicines used to suppress the immune system)
- Digoxin (a medicine for heart failure and irregular heart rhythm)
- Minoxidil (a drug used to treat high blood pressure)
- Salbutamol (a medicine for asthma)
- Oral contraceptives
- Hormone replacement therapy (female hormones)

Etoricoxib can be taken with low-dose aspirin (acetylsalicylic acid). If you are currently taking low-dose aspirin to prevent heart attack or stroke, you should not stop treatment without consulting your doctor because etoricoxib can not replace aspirin (acetylsalicylic acid) for this use. You should be aware that the risk of stomach ulcers is greater if you take aspirin with etoricoxib and should not take anti-inflammatory drugs or higher doses of aspirin (acetylsalicylic acid) while you be taking etoricoxib.

Remember your doctor has prescribed this medicine only for you. Never give it to someone else.

Use with food and beverages

Special physiological situations

Pregnancy and lactation

Etoricoxib should not be taken during pregnancy. Do not take the tablets if you are pregnant, think you may be pregnant or are planning to become pregnant.

If you become pregnant, stop taking this medicine and see your doctor. Consult your doctor if you have questions or need more information.

It is not known whether etoricoxib is excreted in human milk. If you are breastfeeding or plan to, consult your doctor before taking etoricoxib. If you are using etoricoxib should not breastfeed your child.

Children:

Etoricoxib should not be used in children or adolescents under 16 years of age.

Elderly

Etoricoxib works equally well in young adults and elderly patients. If you are elderly, your doctor will want to keep a tighter control of your health. No dose adjustment is required in elderly patients

Driving and using machines

In some patients taking etoricoxib have been reported dizziness and drowsiness. Do not drive if you experience any of these symptoms nor use any tools or machines.

Consult your doctor or pharmacist before taking any medicine.

How to use etoricoxib     

Follow carefully the instructions of use of etoricoxib provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given on this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Remember to take your medicine. The doctor will indicate the duration of the treatment and how to increase the dose.

Take etoricoxib by mouth once a day. This medicine may be taken with or without food.

Does not exceed the recommended dose to treat your disease.

osteoarthritis

The recommended dose is 60 mg once a day.

Rheumatoid arthritis

The recommended dose is 90 mg once daily.

Gout

The recommended dose is 120 mg once a day which should only be used for the acute painful period, limited to a maximum of 8 days treatment.

Higher doses than recommended for each condition have either not demonstrated additional efficacy or have not been studied properly. Therefore, the doses stated above for each condition are the maximum recommended doses.

If you have a mild or moderate liver disease, you should not take more than 60 mg a day.

Your doctor will have decided what dose is appropriate for you, and it is important to take the tablets as your doctor has told you.

Do not change your dose or stop treatment without talking to your doctor.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take etoricoxib     

If a doctor prescribes another medicine, tell you are taking etoricoxib.

Check with your doctor regularly developments of disorder that motivates the use of etoricoxib. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with etoricoxib at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with etoricoxib, immediately consult your doctor.

If you take more than you should:

You should never take more tablets than your doctor recommends. If you take too many tablets etoricoxib, seek medical attention immediately.

If you forget to take etoricoxib

It is important to take the dose your doctor has prescribed. If you miss a dose, do not take an extra dose to make up and just resume the usual schedule the following day.

Side Effects of etoricoxib     

Like all medicines, etoricoxib can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

In clinical studies and in general use, the side effects occurred in patients treated with etoricoxib were as follows:

Common side effects (affect 1 in 10-100 patients)

Weakness and fatigue, dizziness, flu-like symptoms, diarrhea, indigestion (dyspepsia), stomach pain or discomfort, heartburn, flatulence, nausea, alteration of liver-related blood tests, headache, swelling of the legs and/or feet due to fluid retention (edema) and increased blood pressure, palpitations, hematomas.

Uncommon side effects (affect 1 in 100-1000 patients)

abdominal swelling, chest pain, swelling of the face, redness, feeling cold, high blood potassium levels, changes of kidney-related blood or urine tests, heart failure, myocardial infarction, cerebral infarction, mini-stroke (transient ischemic attacks), abnormal heart rhythm (atrial fibrillation), change in bowel movement pattern, constipation, dry mouth, altered taste, gastroenteritis, stomach ulcer, irritable bowel syndrome, inflammation of the esophagus, mouth ulcers, vomiting, blurred vision, eye irritation and redness, nose bleeding, ringing in the ears, vertigo, increased or decreased appetite, weight gain, cramps or muscle spasm, muscle pain or stiffness, inability to sleep, numbness or tingling, drowsiness, anxiety, depression, decreased mental acuity, short of breath, cough, skin rash or itching, urinary infection.

Rare side effects (affect 1 in 1.000-10.000 patients)

Low blood sodium level.

Very rare side effects (affect less than 1 in 10.000 patients)

Allergic reactions (which can be very serious and require immediate medical attention) included urticaria, swelling of the face, lips, tongue and/or throat. It may cause difficulty breathing or swallowing (angioedema), bronchospasm (wheezing or difficulty breathing), severe skin reactions, inflammation of the stomach or stomach ulcers that can become serious and may bleed, liver problems, severe kidney problems, severe increase in blood pressure, confusion, hallucinations.
