Indications and usage     

Imipramine is in a group of drugs called tricyclic antidepressants. These drugs alter levels of chemicals in the brain, relieving symptoms of depression.

Imipramine is used for:

- Treat symptoms of depression
- Treat childhood enuresis (involuntary urination during sleep)

Medizzine recommends consulting your doctor if you are unsure (or you do not know) why you have been given imipramine.

Other ingredients of available commercial presentations:

Tablets:

Besides the active principle imipramine, the medicament contains excipients as lactose and sucrose. It may also contain excipients such as sunset yellow (E 110), amaranth (E 123; a red substance banned in the United States), propyl hydroxybenzoate (E 216) and methyl hydroxybenzoate (E 218).

Important information     

This medicine contains lactose and sucrose. If your doctor has told you that you have an intolerance to some sugars, contact him before taking it. In diabetic patients should be taken into account this circumstance.

Sunset yellow dye (E-110) may cause allergic reactions in patients allergic to aspirin (aspirin and other salicylates). Hydroxybenzoate may cause abdominal discomfort and delayed allergy.

This medicine contains methylparaben and propylparaben, which may cause allergic reactions, possibly delayed.

Before taking the drug     

Do not use imipramine

If you are allergic (hypersensitive) to imipramine, to other tricyclic antidepressants or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you suffer from cardiac disorders such as irregular heartbeat (arrhythmia), heart block or have recently had a heart attack
- If you have episodes of mania
- If you have a severe liver disease
- If you have porphyria (a genetic disorder of hemoglobin in blood which causes blistering rash, abdominal pain, and nervous system disorders)
- If you can not swallow water
- If you have increased eye pressure (glaucoma)
- If you are taking monoamine oxidase inhibitors (MAOIs) or have been taking any of these drugs during the previous two weeks
- If the patient is younger than six years

Take special care with imipramine

Suicidal thoughts or worsening of depression or anxiety disorder

If you are depressed or have an anxiety disorder may have sometimes thought of harming or killing yourself. These side effects may be increased when you start treatment with antidepressants since these medicines take a time up to be effective, usually two weeks, although this period may be longer.

You may suffer more likely these thoughts:

- If you have previously had these thoughts of harming or killing yourself
- If you are a young adult.

The information from clinical trials has shown an increased risk of suicidal behavior in young adults (under 25 years) with psychiatric conditions who were treated with antidepressants.

If you have thoughts of harming yourself or suicidal, contact your doctor or go to the hospital immediately.

It may be useful to tell a relative or friend that you are depressed or have anxiety and ask them to read the leaflet that accompanies the medication container.

You can ask to tell you whether they think your depression or anxiety is getting worse or if are worried about your behavior.

Before taking imipramine, check with your doctor if you or your child (if he is the patient):

- You have any psychiatric disorder such as schizophrenia or a manic depressive psychosis
- You are trying or have recently gotten give up alcohol or you are in the process of gradua1 withdrawal an antiepileptic treatment.
- You have or have had glaucoma or prostatic hypertrophy.
- You have an overactive thyroid or are being treated for any thyroid disease.
- You have a history of epilepsy or brain damage.
- You have low blood pressure or poor blood circulation.
- Have a severe kidney disease
- Have a tumor of the adrenal glands, such as pheochromocytoma or neuroblastoma.
- Suffer panic attacks.
- Have longstanding constipation.
- You use contact lenses.
- You are receiving electroconvulsive treatment.
- You should be subjected to any surgery, including dental surgery involving anesthesia.

Laboratory tests

While taking this medication, your doctor will monitor regularly your liver function or blood cell levels.

Dental checkups

It is advisable regularly checking the condition of teeth by your dentist since imipramine can cause dental problems.

Consult your doctor, even if any of the above circumstances have only happened once.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

Tell your doctor especially if you are taking:

- Medicines to treat epilepsy, as barbiturates, phenytoin, carbamazepine, etc.
- Benzodiazepines, as diazepam, nitrazepam, oxazepam, alprazolam,
- Antidepressant medications including selective inhibitors of serotonin reuptake [SSRIs] (fluoxetine, fluvoxamine, etc)
- Disulfiram for the treatment of alcohol addiction.
- Treatment with nicotine for smoking cessation.
- Methylphenidate used to treat attention deficit and hyperactivity disorder,
- Anticoagulant drugs (warfarin)
- Antihistamines (medicines to treat allergies),
- Altretamine (to treat some cancers)
- Apraclonidine and brimonidine (for the treatment of glaucoma)
- Baclofen (a muscle relaxant),
- Analgesics as tramadol, codeine, dihydrocodeine.
- Some medicines for treating a heart disease, such as diltiazem, verapamil, labetalol, propranolol or quinidine
- Medicines to treat angina, applied by sprays or dissolved under the tongue, as nitroglycerin or isosorbide dinitrate.
- Drugs for treatment of arterial hypertension as guanethidine, debrisoquine, methyldopa, reserpine, clonidine or diuretics
- Medicines to treat some mental illnesses, such as thioridazine or chlorpromazine
- Cimetidine, medicine to treat stomach ulcers
- Entacapone or selegiline used to treat Parkinson's disease,
- Oral contraceptives or hormone replacement therapy
- Appetite-slowing
- Sympathomimetic drugs as adrenaline, ephedrine, isoprenaline, noradrenaline, phenylephrine and phenylpropanolamine (this last may be present in medicines sold without prescription for cold symptoms).
- Ritonavir, a medicine for treatment of AIDS

Use with food and beverages

You should not consume alcohol with this medication.

Special physiological situations

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy and lactation

This medicine should not be taken during pregnancy or breastfeeding.

If you take imipramine during the last three months of pregnancy, the baby may be born with breathing difficulties, lethargy, colic, irritability, changes in blood pressure, tremors and spasms. If its administration is considered necessary during pregnancy, it should be discontinued seven weeks before the expected date of delivery.

Use in children

Not for use in children under six.

Driving and using machines

Imipramine can alter alertness and cause drowsiness and blurred vision. These symptoms can be aggravated by alcohol intake.

Before driving or operating machinery should make absolutely sure that imipramine does not affect you for these tasks.

How to use imipramine     

Follow carefully the instructions of use of imipramine provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Depression:

Adults

25 mg three times a day, gradually increasing to 150 mg-200 mg per day in two or three doses.

In severe cases, treated in hospital, the dose may be increased up to 100 mg three times daily.

The usual maintenance dose is 50 to 100 mg per day in two or three doses.

Patients over 60 years:

Starting dose: 10 mg. It can be increased up to 30-50 mg daily

Nocturnal enuresis:

Children should only take the medicine at night, before go to bed, for a maximum of three months and a maximum dose 75 mg daily.

Children older 11 years (35-54 kg): 50-75 mg per day
8-11 years (25-35 kg): 25-50 mg per day.
6-7 years (20-25 kg): 25 mg per day
Children under 6 years: It is not recommended.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take imipramine     

If a doctor prescribes another medicine, tell you are taking imipramine.

Check with your doctor regularly developments of disorder that motivates the use of imipramine. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with imipramine at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with imipramine, immediately consult your doctor.

If you take more than you should:

If you (or your child) take more than you should, see a doctor immediately or go to the emergency department of the nearest hospital. Take the container with you, even if it is empty.

Alternatively, you can consult the Toxicology Information Service in your country.

Symptoms of overdose include tachycardia (fast heart rate) or irregular heartbeat, low blood pressure, drowsiness, fever, seizures, coma, agitation, muscle stiffness or impression of suffering a serious ill.

If you forget to take imipramine

Do not take a double dose to make up for a forgotten. If you miss a dose, take another as soon as you remember and the next on the usual schedule.

If you stop taking imipramine

Talk to your doctor before stopping treatment and follow his advice, as you may experience withdrawal symptoms if stopped abruptly.

Side Effects of imipramine     

Like all medicines, imipramine can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Discontinue treatment and consult immediately with your doctor

If you have any of the following allergic reactions: pneumonitis (fever, chills, cough, shortness of breath, unusual weight loss, feeling of being ill), skin rash (which can be itchy), sensitivity to sun or sun lamps, swelling of the face or tongue, which is potentially serious if it causes breathing difficulty (angioedema), shock and collapse.

Tell your doctor if you experience any of the following side effects:

Blood side effects: Reduction of some type of blood cell. You may experience a sore throat, mouth ulcers, pain or frequent bleeding and recurrent infections.

Metabolic and endocrine side effects: Disorders of sexual function or sexual desire, swelling of the breasts (gynecomastia), both women and men, overproduction of milk in lactating women (galactorrhoea), changes in blood glucose, loss or weight gain, syndrome of inappropriate secretion of antidiuretic hormone.

Nervous side effects: disorientation, vertigo, tiredness, weakness, headache, difficulty concentrating, confusion, agitation, mood changes, aggressiveness, difficulty sleeping, hallucinations, anxiety, restlessness, pricking sensation, tremor, muscle spasms or lack of muscle control, speech disorder and seizures.

Anticholinergic side effects: Dry mouth, blurred vision or double vision, constipation, sweating, hot flushes, difficulty urinating or urinary retention, pupil dilation, glaucoma and intestinal paralysis.

Cardiovascular side effects: Dizziness upon standing, low or high blood pressure, tachycardia, palpitations, arrhythmia (irregular heartbeat), electrocardiographic changes.

Digestive side effects: gastrointestinal discomfort, loss of appetite, inflammation of the oral mucosa, tongue lesions.

Liver side effects: Impaired liver function, jaundice, alterations in liver function tests.

Other: Hair loss, petechiae

Withdrawal symptoms: feeling unwell, upset stomach, diarrhea, difficulty sleeping, nervousness, anxiety, headaches, irritability
