Indications and usage     

The active ingredient ketoconazole belongs to the group of imidazole antifungal.

It is used for the treatment of infections caused by fungi and yeasts, such as:

- infections of the skin, hair and nails.
- persistent vaginal infections
- infections of stomach, intestines and other internal organs (or several organs at once).

Ketoconazole may also be prescribed to prevent fungal infections in people whose natural defenses are seriously damaged.

Other ingredients of available commercial presentations:

Tablets:

Each tablet contains as active ingredient ketoconazole. The other ingredients are lactose, corn starch, polyvinylpyrrolidone, microcrystalline cellulose, colloidal anhydrous silica and magnesium stearate.

Oral suspension:

Each ml contains as active ingredient ketoconazole (INN) 20 mg. The other ingredients are sucrose, sodium saccharin, cellulose microcrystalline, sodium carboxymethylcellulose, sodium benzoate, cream cherry flavor, sodium lauryl sulfate, citric acid monohydrate, erythrosine and purified water.

Additional warning     

This medicine contains lactose (tablets) or sucrose (suspension). If your doctor has told you that you have an intolerance to certain sugars, contact him before taking it. Diabetic patients should take into account the sucrose content of the suspension.

Before taking the drug     

Do not use ketoconazole

If you are allergic (hypersensitive) to the active substance ketoconazole, or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor should take this medicine if:

- you have liver problems.
- you are pregnant or think you may be pregnant. Consult your doctor.
- you are breastfeeding your child.

Nor should take in the case of taking:

- some medicines for allergies (terfenadine, astemizole, and mizolastine);
- certain drugs for sleep (midazolam and triazolam);
- cisapride and domperidone, medicines used for certain digestive problems;
- certain drugs used to lower cholesterol levels (for example, simvastatin and lovastatin);
- pimozide (a medicine used for psychotic disorders);
- certain medicines used to treat irregular heartbeat rhythm, mainly quinidine and dofetilide.

Take special care with ketoconazole

If you have liver problems:

You should not take ketoconazole if you have acute or chronic liver disease. During treatment with this medicine may occur liver problems, sometimes serious.

If you have any of the following symptoms, stop taking ketoconazole and consult your doctor immediately: dark urine, pale stools, yellowish skin, abnormal fatigue, nausea, vomiting, loss of appetite, abdominal pain and/or fever.

If you must take ketoconazole, your doctor will need to perform regular blood tests in order to rule out any liver disorder and make sure that you can continue treatment.

Tell your doctor if you have Addison's disease (impaired function of the adrenal gland). If you have Addison's disease or in periods of high stress, such as major surgery, monitoring the function of your adrenal glands may be necessary.

Tell your doctor if you have porphyria, a metabolism disorder characterized by the removal of porphyrin in the urine and feces.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription. It is important, especially if you are taking:

Some medicines should not be taken simultaneously and others need to adjust their dose.

Examples of drugs that should not be taken while taking ketoconazole:

- Some allergy medicines (terfenadine, astemizole, and mizolastine);
- certain drugs for sleep (midazolam and triazolam);
- cisapride and domperidone, certain drugs used for digestive problems;
- certain drugs used to lower cholesterol levels (eg simvastatin and lovastatin);
- pimozide (a medicine used for psychotic disorders)

The combination with certain other medications may require dose adjustment of either ketoconazole or the other medications. Examples are:

- certain drugs for migraine headaches (ergotamine and dihydroergotamine).
- certain drugs for tuberculosis (eg: rifampicin, rifabutin, isoniazid) and medicines for epilepsy (eg: carbamazepine and phenytoin);
- some drugs acting on the heart and blood vessels (digoxin and certain drugs called calcium channel blockers);
- drugs that decrease blood clotting;
- methylprednisolone, a drug administered orally or in injectable for inflammatory processes;
- cyclosporine, tacrolimus and rapamycin (= sirolimus) drugs that are typically used after organ transplantation;
- certain HIV protease inhibitors;
- certain drugs used in cancer treatment;
- certain drugs used for anxiety or sleeping (tranquilizers), mainly buspirone, alprazolam, and brotizolam;
- sildenafil
- trimetrexate.
- Ebastine, a drug allergy;
- reboxetine, a drug used to treat depression.

If any of these situations affects you or have doubts, consult your doctor before taking ketoconazole.

Use with food and beverages

Ketoconazole should always be taken with food, to be absorbed properly by the body. The tablets should be swallowed with a little liquid.

Some people feel dizzy if they drink alcohol during treatment with ketoconazole. It is therefore, advisable not to drink alcohol.

To ensure proper absorption by the body of ketoconazole, should have enough acidity. Therefore, drugs that neutralize stomach acid should not be taken until have passed two hours since taking ketoconazole.

For the same reason, if you are taking any medications that inhibit the production of acid in the stomach, you should take ketoconazole with some cola beverage.

Special physiological situations

Pregnancy & lactation

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

If you are pregnant or think you might be talking to your doctor, he will decide if you can or can not take ketoconazole.

If you are taking ketoconazole it is not recommended breastfeeding as it may be small amounts of the drug in milk.

Driving and using machines

How to use ketoconazole     

Follow carefully the instructions of use of ketoconazole provided by your doctor. Consult your doctor or pharmacist when in doubt. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Your doctor will tell you how long your treatment with ketoconazole. Do not stop treatment before.

The amount of ketoconazole and how long you have to take it depends on the type of infection and its location. Your doctor will tell you exactly how to take. The duration of treatment may vary from 5 days to 12 months.

Adults and children weighing more than 30 kilos:

Usually, the daily dose is one tablet once daily with food. Sometimes, the doctor may be increased the dose to two tablets once a day.

Suspension: Normally, two teaspoons (10 mL) oral suspension once a day. Sometimes, this dose may be increased up to 4 teaspoons (20 ml), once a day, with meals.

Children weighing 15 to 30 kilos:

Usual daily dose: half a tablet once a day with food or a full teaspoon (5 mL) oral suspension once daily, with meals.

Children weighing less than 15 kilos:

Recommended dose: 1 ml oral suspension three times a day with meals.

If you think that the effect of the medicine is too strong or too weak, consult your doctor.

While you take ketoconazole     

If you become pregnant, contact your doctor immediately.

If a doctor prescribes another medicine , tell you are taking ketoconazole.

Do not restart treatment with ketoconazole at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with ketoconazole, immediately consult your doctor.

If you use more medicine than you should

Tell your doctor immediately if you have taken an excessive amount of ketoconazole. He will do the most appropriate treatment.

In case of overdose or accidental ingestion, consult the Toxicology Information Service of your country.

If you forget to take ketotifen

If you forget to take one dose, take this dose as soon as you remember. Then, continue normally. Do not change the prescribed dose yourself.

Do not take a double dose to make up for a forgotten dose.

If you stop taking the medicine

If you interrupt your treatment with ketoconazole, symptoms may come back. Therefore, do not stop taking this medicine unless your doctor tells you

Side Effects of ketoconazole     

Like all medicines, ketoconazole can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

This medication is usually well tolerated.

Sometimes, occur the following side effects:

Stomach upset, nausea, vomiting, abdominal pain and diarrhea, sometimes occur but are not serious. Also can occur: headache, dizziness, sensitivity to bright light, tingling in the fingers of hands or feet, hair loss, erectile dysfunction, or menstrual disorders.

Hypersensitivity to ketoconazole is rare. It is mainly manifested by rashes, itching, shortness of breath and / or swelling of the face (angioedema). In this case, stop treatment and visit your doctor.

Los liver problems are also rare. Symptoms are: loss of appetite, nausea, vomiting, abdominal pain, dark urine, pale stools, yellowish skin, unusual tiredness and/or fever. If this happens stop taking ketoconazole and see your doctor immediately.
