Indications and usage     

Ketorolac tromethamine is a painkiller belonging to a group of medicines called nonsteroidal antiinflammatory drugs (NSAIDs).

It is indicated for the short-term treatment post-operative pain, mild to moderate.

The injectable form is also indicated for treating pain caused by renal colic.

Tablets:

Besides ketorolac, excipients include: Lactose, microcrystalline cellulose, magnesium stearate, hydroxypropyl methylcellulose, titanium dioxide, polyethylene glycol.

Injectable solution:

Each ml solution contains 30 mg ketorolac tromethamine. The other ingredients are: sodium chloride, ethanol, sodium hydroxide and water for injection.

Important information     

Tablets:

This medicine contains lactose. If your doctor has told you have an intolerance to some sugars, contact him before taking it.

Injectable solution:

This medicine contains 12.25% ethanol (alcohol); this small amount is equivalent to 98 mg/ampoule . This product contains less than 23 mg sodium (1 mmol) per ampoule, which is considered essentially sodium free.

Before taking the drug     

Do not use ketorolac

If you are allergic (hypersensitive) to ketorolac to any NSAID or any of the other ingredients of the medicinal product prescribed to you. It is possible that this medicine may have cross-sensitivity to acetylsalicylic acid (aspirin) and other prostaglandin synthesis inhibitors, which may induce severe allergic reactions. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- Having active peptic ulcer
- Having a history of ulceration, gastrointestinal bleeding or perforation
- Having a full or partial syndrome of nasal polyps, angioedema or bronchospasm.
- Having asthma.
- Suffering severe heart failure.
- Suffering moderate to severe renal impairment.
- Having suffered considerable blood loss or dehydration.
- Being prone to bleeding and clotting disorders. It should not be used in patients with cerebral hemorrhage.
- Be subjected to surgical interventions with high risk of bleeding or incomplete hemostasis.
- Concomitant treatment with other NSAIDs, including acetylsalicilic acid (aspirin) and selective inhibitors cyclo-oxygenase-2 (COX-2).
- Be subjected to anticoagulant therapy at therapeutic doses.
- Using antiplatelet.
- Concomitant treatment with probenecid, lithium salts and pentoxifylline.
- Pregnancy, childbirth or breastfeeding.
- Patients under 16.
- In the immediate preoperative and during surgical procedure.

Take special care with ketorolac

- If you experience gastrointestinal pain (irritation, ulceration or perforation), rectal bleeding or vomiting, you should immediately discontinue the treatment and consult to doctor immediately.
- Take special care if you are older than 65 years.
- When this drug is administered to elderly or debilitated patients should be handled carefully and use the minimum effective dose.
- If you take medicines which increase the risk of ulcer or gastrointestinal bleeding, such as heparin, antiplatelet drugs, corticosteroids,thrombolytics or selective serotonin reuptake inhibitors (antidepressants) (SSRI) and/or norepinephrine (SNRI).
- If you have had inflammatory bowel disease (ulcerative colitis, Crohn's disease).
- If you have kidney disease, your doctor may need to perform certain tests during treatment with this medicine.
- If you experience any renal disoder (lower back pain, bloating, blood in urine or other sign of kidney inflammation).
- Ketorolac can cause fluid retention, use extreme caution if you have hypertension or cardiac decompensation.
- If you are being treated with anticoagulants (warfarin or heparin).
- If you experience widespread manifestations of liver dysfunction as itching or reddening of the skin during treatment, treatment should be discontinued immediately and should consult to doctor as soon as possible..
- If you have difficulty getting pregnant or being treated for infertility.
- If you experience skin redness, swelling of the oral mucosa, shortness of breath, dizziness or any other sign of hypersensitivity or anaphylactic reaction should discontinue treatment and immediately consult your doctor.
- If you are taking other medications, such as methotrexate, because ketorolac may modify or enhance the effect of these.

Taking other medicines

You should not use ketorolac with:

Other NSAIDs (including aspirin) , anticoagulants, antiplatelet (such as aspirin, ticlopidine, or clopidogrel), pentoxifylline , probenecid and lithium.

Take special care if you are being treated with any of the following medications::

Corticoesteroids, thrombolytic, (drugs to help eliminate blood clots), antidepressants, methotrexate, antihypertensive o furosemide (a diuretic used to increase urine output).

Special physiological situations

Pregnancy and lactation

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Ketorolac is contraindicated in pregnancy and childbirth.

You should not use this medicine if you are breastfeeding.

Children

Ketorolac should not be used in patients under 16 years.

Elderly

Doses should be adjusted according to the instructions provided below.

Driving and using machines

Caution is advised when driving or operating machinery, especially at the beginning of treatment, since to ketorolac may cause drowsiness, dizziness, insomnia, vertigo and depression.

How to use ketorolac     

Follow carefully the instructions of use of ketorolac provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given on this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Your doctor will indicate the duration of treatment. Do not stop treatment before.

Treatment with ketorolac should be initiated in hospital.

Administration of the injection solution:

The maximum duration of treatment must not exceed two days. After these two days, is required switching to oral treatment, but the total duration of treatment with ketorolac may not exceed 7 days.

In the treatment of pain caused by renal colic a single dose of 30 mg is recommended for intramuscular or intravenous.

The dose of ketorolac tromethamine should be adjusted according to the severity of pain and the patient's response in order to administer the lowest possible effective dose.

The recommended ketorolac starting dose via intramuscular or intravenous is 10 mg followed by 10-30 mg every 4 to 6 hours, as needed for pain control. In cases of severe or very severe pain, the recommended starting dose is 30 mg ketorolac.

The maximum recommended daily dose is 90 mg for adults and 60 mg in elderly patients.

In patients who are receiving intravenous ketorolac and pass to oral treatment, the total combined daily dose of both oral and parenteral forms should not exceed 90 mg in adults and 60 mg in elderly patients.

Tablets:

The oral treatment duration should not exceed 7 days. If the patient have received previously ketorolac in the post-operative period, the total duration of treatment with this medicine may not exceed 7 days..

The recommended oral dose is 10 mg every 4 to 6 hours and should not exceed 40 mg/day.

Patients with mild renal insufficiency (serum creatinine=170-442 mmol/l) should receive lower doses (half the recommended dose), without exceeding 60 mg as total daily dose, and should be performed periodic renal function tests.

Patients who receive intravenous ketorolac and pass to oral treatment, the combined total daily dose should not exceed 90 mg in adults and 60 mg in elderly patients during the day of change.

When morphine is administered in combination with ketorolac, the required daily dose of morphine is considerably reduced.

While you take ketorolac     

If a doctor prescribes another medicine, tell you are taking ketorolac.

Check with your doctor regularly developments of disorder that motivates the use of ketorolac. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with ketorolac at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with ketorolac, immediately consult your doctor.

If you take more than you should:

If you have taken more ketorolac than you should, you may experience abdominal pain, nausea, vomiting, hyperventilation, peptic ulcer, erosive gastritis and renal dysfunction.

If you take more ketorolac than you should, tell your doctor or go to the nearest hospital casualty department straight away. Take the medicine pack with you, even if it is empty.

If you forget to take ketorolac

Do not take a double dose to make up for missed doses.

Side Effects of ketorolac     

Like all medicines, ketorolac can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Gastrointestinal side effects:

Adverse effects seen more requently are gastrointestinal: peptic ulcer, perforation or gastrointestinal bleeding, sometimes fatal, particularly in the elderly. If you develop any symptoms that could indicate a serious gastrointestinal reaction, should consult your doctor immediately.

Other side effects may include: nausea, vomiting, diarrhea, flatulence, constipation, abdominal pain or discomfort, inflammation of the stomach, blood in stools, blood in vomiting, mouth ulcers, worsening of ulcerative colitis and Crohn's disease, belching, feeling of fullness, inflammation of the esophagus, inflammation of the pancreas, altered taste, dry mouth.

Metabolism and nutrition side effects:

Anorexia (lack of appetite), increased potassium, increased sodium, increased thirst.

Neurologic side effects:

Meningitis, seizures, dizziness, drowsiness, headache, hyperactivity y decreased ability to concentrate, insomnia, tingle and numbness.

Psychiatric side effects:

Abnormal dreams, abnormal thinking, anxiety, depression, euphoria, hallucinations, psychotic reactions, nervousness.

Renal and urinary side effects:

Acute renal failure, kidney pain, increased urinary frequency, urinary retention, decrease in urine output or other signs of kidney inflammation.

Cardiovascular side effects:

Swelling (edema, fluid retention), arterial hypertension, heart failure, arrhythmias, hot flushes, palpitations, hypotension, chest pain, pallor.

Reproductive system side effects:

infertility.

Respiratory side effects:

Asthma, respiratory difficulty, pulmonar congestion, bronchospasm, nasal bleeding, pulmonary edema (fluid in the lungs).

Hepatobiliary side effects:

Abnormal liver tests, hepatitis, jaundice, liver failure.

Musculoskeletal side effects:

Muscle pain.

Cutaneous side effects:

Very rarely may occur allergic vesiculobullous skin reactions (including toxic epidermal necrolysis (Lyell disease) and Stevens-Johnson syndrome (skin and mucous membrane lesions). Other skin reactions may include scaly dermatitis, rash with macules (spots) or papules (bumps), itching, urticaria, facial erythema.

Immune system side effects:

Allergic reactions, anaphylaxis, swelling of the larynx, angioneurotic edema (angioedema).

If you develop any symptoms that could indicate a serious allergic reaction, you should see your doctor immediately. Anaphylactoid reactions, including anaphylaxis, can become fatal.

Blood side effects:

Blood spots on the skin (purpura), decreased platelet count, hemolytic uremic syndrome (anemia and blood in stool).

Eye side effects:

Impaired vision.

Ear and labyrinth side effects:

Ringing in the ears, decreased hearing, vertigo.

General side effects:

Asthenia, weight gain, swelling, reactions at the administration point, fever.

Laboratory tests:

Elevated levels of urea and creatinine, elevation of potassium levels, increased time of bleeding.

Injury, poisoning and procedural complications:

Bruising, postoperative bleeding.
