Indications and usage     

Moxifloxacin is an antibiotic belonging to the family of fluoroquinolones. This substance works by killing bacteria that cause infections, if such bacteria are susceptible to the active ingredient moxifloxacin.

Moxifloxacin is used in adults (not for children) to treat the following bacterial infections:

- Sudden worsening of chronic bronchitis (acute exacerbation of chronic bronchitis).
- Lung infection (community-acquired pneumonia), except severe cases.
- Acute infection of the sinuses (acute bacterial sinusitis).

Medizzine recommends consult your doctor if you are unsure (or you do not know) why you have been given moxifloxacin.

Other ingredients of available commercial presentations:

Each film-coated tablet contains 400 mg moxifloxacin as hydrochloride. The other ingredients are:

Core: microcrystalline cellulose, croscarmellose sodium, lactose monohydrate and magnesium stearate.

Coating: hypromellose, Macrogol 4000, iron oxide (E172) and titanium dioxide (E171).

Important information     

This medicine contains lactose. If your doctor has told you have an intolerance to some sugars, contact him before taking it.

Before taking the drug     

Consult your doctor if you are unsure of belonging to any of groups of patients described below.

Do not use moxifloxacin

If you are allergic (hypersensitive) to the active substance moxifloxacin, to other quinolones or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor take this medicine if:

- If you are pregnant or breastfeeding.
- If are a patient in growth period.
- If you have a history of tendon diseases or injuries related to treatment with fluoroquinolones.
- If you have a severe liver disease or increased levels of liver enzymes (transaminase) 5 times above the upper limit of normal.
- If you have any hereditary disease or a related illness with certain changes in the electrocardiogram,
- If you have salt imbalance in blood, especially low blood potassium levels (hypokalemia) which are currently not corrected by treatment,
- If you have a very low heart rate (bradycardia) or you have a weak heart (heart failure),
- If you have a history of abnormal heart rhythms (arrhythmias), or you are taking other medications that cause certain disorders on the electrocardiogram.

The main reason for most of these contraindications is because moxifloxacin may cause changes in the ECG as a prolongation of the QT interval ie delayed conduction of electrical signals.

Take special care with moxifloxacin

Moxifloxacin can modify your heart's ECG. If you are currently taking any medication that can cause a decrease in blood potassium levels, consult your doctor before taking it.

If you experience palpitations or irregular heart beat during treatment, inform your doctor immediately, who can perform an ECG to measure your heart rate.

If you suffer from epilepsy or another disorder that can cause you to have convulsions, consult your doctor before taking this medicine

If you or a relative have glucose-6-phosphate dehydrogenase deficiency (rare hereditary disease), inform your doctor, whom will indicate whether moxifloxacin is right for you .

During treatment with moxifloxacin:

- The risk of cardiac abnormalities may increase with increasing dose. Therefore, you should take the recommended dose.

- Rarely, you may experience a severe, sudden allergic reaction (reaction or anaphylactic shock) even with the first dose, and develop the following symptoms: chest tightness, feeling dizzy, feeling sick or faint, or feel dizzy when standing. If any of these symptoms occur, stop taking moxifloxacin and consult your doctor immediately.

- This substance can cause rapid and severe liver inflammation, which can lead to liver failure that endangers patient's life. Consult your doctor before continuing treatment if you develop symptoms such as a sudden feeling of discomfort and/or discomfort associated with yellowing of skin or the whites of the eyes (jaundice), dark urine, itching, bleeding tendency or brain disease produced by liver disorder (symptoms of reduced liver function or severe inflammation) .

- You can develop diarrhea during or after taking antibiotics including moxifloxacin. If diarrhea is severe or persistent, or if you notice blood or mucus in the stool, stop taking the drug immediately and consult your doctor. In these situations, you should not take medicines that can stop or decrease bowel movement.

- Occasionally, this active ingredient can cause pain and inflammation of tendons, especially in elderly or in whom are being treated with corticosteroids. At the first sign of pain or inflammation you should stop taking moxifloxacin, keep rest the affected limb and consult your doctor immediately.

- If you are elderly and suffer from kidney problems, be sure that your fluid intake is sufficient because dehydration may increase the risk of kidney failure.

- If your vision gets worse or if you have any other eye disturbances while taking moxifloxacin, consult an eye specialist immediately.

- Quinolones can increase skin sensitivity to sunlight or UV light. Must avoid prolonged exposure to sunlight or direct sunlight and should not use tanning beds or any UV lamp during treatment with moxifloxacin.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

While taking moxifloxacin, you should consider the following:

If you are taking moxifloxacin and other medicines that affect the heart, there is an increased risk of disorders of heartbeat. Therefore you should not take at the same time moxifloxacin with the following drugs:

- Drugs belonging to the class named antiarrhythmic (e.g., quinidine, hydroquinidine, disopyramide, amiodarone, sotalol, dofetilide, ibutilide),
- Neuroleptics (e.g. phenothiazines, pimozide, sertindol, haloperidol, sultopride),
- tricyclic antidepressants
- Some antimicrobial drugs (e.g. sparfloxacin, erythromycin administered intravenously, pentamidine, antimalarial, in particular, halofantrine)
- some antihistamines (terfenadine, astemizole, mizolastine)and other drugs (e.g., cisapride, intravenous vincamine, bepridil and diphemanil).
- Any medicine containing magnesium or aluminum, such as antacids for indigestion, or any medicine containing iron or zinc, drugs containing didanosine or drugs containing sucralfate to treat gastrointestinal disorders, since may reduce the action of moxifloxacin. Therefore, you should take the tablets 6 hours before or after taking other medications.

The oral intake of medicinal charcoal while moxifloxacin tablets reduces its action. Therefore, it is recommended not to use these drugs simultaneously.

If you are currently taking oral anticoagulants (e.g., warfarin) your doctor may need monitor the clotting time of blood (INR).

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Use with food and beverages

The effect of moxifloxacin is not altered by food including dairy products.

Special physiological situations

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy and lactation

Do not use this medicine during pregnancy or if you are breast-feeding.

Driving and using machines

The active ingredient moxifloxacin may cause dizziness or vertigo. If you suffer from these symptoms, do not drive or operate machinery.

Consult your doctor or pharmacist before taking any medicine.

How to use moxifloxacin     

Follow exactly instructions of use of moxifloxacin provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

The usual dose in adults is one film-coated tablet of 400 mg once a day orally.

Take the tablet whole (to mask the bitter taste) and with plenty of fluids. It can be taken with or without food. It is recommended to take tablets at the same time of day.

No dose adjustment is required in elderly patients, in patients with a low bodyweight or in patients with kidney problems.

Duration of treatment depends on the type of infection. Unless your doctor tells you otherwise, the duration of treatment with moxifloxacin is as follows:

- Sudden worsening of chronic bronchitis (acute exacerbation of chronic bronchitis): 5 to 10 days.

- Lung infections (pneumonia) acquired in the community, except in severe cases: 10 days.

- Acute infections of the sinuses (acute bacterial sinusitis): 7 días

It is important to take the full course, even if you begin to feel better after a few days. If you stop treatment too soon, the infection may be not completely healed, may return or your condition may worsen and you may also create a bacterial resistance to the antibiotic.

Do not exceed the recommended dose and duration of treatment.

If you become pregnant, contact your doctor immediately.

If a doctor prescribes another medicine , tell you are taking moxifloxacin.

Check with your doctor regularly developments of disorder that motivates the use of moxifloxacin. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with moxifloxacin at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with moxifloxacin, immediately consult your doctor.

If you use more medicine than you should

If you take more than the recommended, consult your doctor immediately and, if possible, take any remaining tablets, the packaging or package leaflet and show to the doctor or pharmacist what you have taken.

If you forget a dose:

If you forget a dose, you should take the tablet as soon as you remember on the same day. If you do not take your tablet a day, take your normal dose (one tablet) on the next day. Do not take two tablets to make up the missed dose.

If you stop taking moxifloxacin

Side Effects of moxifloxacin     

Like all medicines, moxifloxacin can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

During treatment with moxifloxacin, have observed the following side effects:

Infection:

Common: Infections caused by resistant bacteria or fungi, e.g. oral and vaginal infections caused by Candida.

Disorders of the blood and lymphatic system:

Uncommon: Low red blood cell count, low white blood cell count, low levels of certain white blood cells (neutrophils), decrease or increase in blood cells necessary for blood clotting (platelet), increase in certain white blood cells (eosinophils), decrease of blood coagulation.

Very rare: increase of blood coagulation.

Allergic reactions:

Uncommon: Allergic reactions.

Rare: Severe, sudden generalized allergic reaction, including Very rarely, anaphylactic shock that can endangering life (e.g. difficulty breathing, decrease in blood pressure, fast heartbeat), sudden swelling of the face, lips, tongue, vocal cords, including possible swelling of the airways that endangers life (angioedema).

Changes in laboratory tests:

Uncommon: Increased blood lipids (fats)

Rare: Increased blood glucose, increased uric acid in blood.

Psychiatric disorders:

Uncommon: Anxiety, restlessness or agitation.

Rare: Emotional instability, depression (very rarely, leading to self-harm), hallucinations.

Very rarely: Feeling of loss of personality (not being yourself), dementia (possibly leading to self-harm).

Nervous system:

Common: Headache, dizziness.

Uncommon: Tingling (like pins and needles) and/or numbness, altered taste (In very rare cases, loss of taste), confusion and disorientation, Sleep disorders (predominantly insomnia), tremors, vertigo, spinning sensation, drowsiness.

Rare: Disturbances of cutaneous sensitivity, alteration of smell (including loss of smell), abnormal dreams, balance disorders and lack of coordination (due to dizziness), seizures, concentration disorders, speech impairment, total or partial loss of memory.

Very rare: Increased skin's sensitivity.

Eyes:

Uncommon: Vision disorders, including double and blurred vision.

Ears:

Rare: Ringing in the ears.

Cardiovascular system:

Common: Marked alteration of electrical activity of heart (ECG) in patients with low blood potassium.

Uncommon: Marked alteration of electrical activity of heart (ECG), palpitations, lirregular and fast heartbeats, serious heart rhythm disturbances, angina pectoris, flush.

Rare: Abnormally fast heart rate, fainting, arterial hypertension, hypotension.

Very rare: abnormal heart rhythm nonspecific, irregular heartbeat (Torsades de Pointes), cardiac arrest.

Respiratory system:

Uncommon Respiratory distress including status asthmaticus.

Digestive apparatus:

Common: Nausea, vomiting, abdominal or stomach pain, diarrhea.

Uncommon: Loss of appetite, flatulence and constipation, upset stomach (indigestion / heartburn), stomach inflammation, increase of certain digestive enzyme in blood (amylase).

Rare: Difficulty swallowing, inflammation of the mouth, severe diarrhea with blood and/or mucus (Antibiotic-associated colitis even pseudomembranous colitis) that, in very rare circumstances, can lead to life-threatening complications.

Liver:

Common: Increase in certain liver enzymes in blood (transaminases).

Uncommon: Abnormal liver function (including blood LDH increase, a liver enzyme), increased bilirubin, blood increase in liver enzymes (gamma glutamyl transferase and/or alkaline phosphatase).

Rare: Jaundice (yellowing of the whites of the eyes or skin), liver inflammation.

Very rare: Fulminant liver inflammation with danger to life by liver failure.

Skin:

Uncommon: Itching, rash, urticaria, dry skin.

Very rare: Alterations of skin and mucous membranes (painful blisters in mouth, nose or penis/vagina) or Stevens-Johnson syndrome (toxic epidermal necrolysis, Lyell's disease) with possible risks to your life.

Locomotor system:

Uncommon: Joint and muscle pain.

Rare: Pain and inflammation of tendons (tendinitis) , muscle cramps, muscle spasms.

Very rare: Tendon rupture, joint inflammation, muscle stiffness.

Kidney:

Uncommon: dehydration.

Rare: Alterations in kidney (including increased laboratory tests for kidney, as urea and creatinine), renal failure.

General adverse effects:

Uncommon: discomfort (mainly, weakness or tiredness), pain in: back, limbs, pelvis and thorax, sweating.

Rare: Swelling of hands, feet, ankles, lips, mouth and throat (angioedema).

In addition, there have been observed rare cases of the following side effects reported following treatment with other quinolone antibiotics and possibly also may occur during treatment with moxifloxacin:

Transient loss of vision, Increased levels of sodium and calcium in blood, increased degradation of red blood cells (hemolysis) and increased sensitivity of the skin to sunlight or UV.
