Indications and usage ▲

Nifedipine belongs to a group of medicines known as calcium channel blockers. Drugs in this group dilate coronary arteries, increasing blood flow, improving oxygen supply to heart. Furthermore, nifedipine dilates peripheral arterial vessels, decreasing high blood pressure.

Tablets of type OROS own a prolonged release system that acts as an osmotic pump, releasing nifedipine through an orifice made by laser technology. Once all the active ingredient is released, the whole OROS tablet, undeformable, is excreted in feces.

This drug is indicated for treatment of chronic stable angina (exertional angina), vasospastic angina pectoris and Raynaud's syndrome.

OROS tablets are indicated for treatment of hypertension and chronic stable angina (exertional angina) in combination therapy.

Capsulas:

Besides active substance nifedipin, the other ingredients are: gelatin, polyethylene glycol 400, glycerin, peppermint flavor, distilled water, sodium saccharin, orange yellow S (E-110), titanium dioxide (E171).

Retard tablets:

Besides active substance nifedipin, the other ingredients are microcrystalline cellulose, corn starch, lactose, polysorbate 80, magnesium stearate, hypromellose, macrogol 4000, titanium dioxide (E171), red iron oxide (E172).

OROS tablets:

Besides active substance nifedipin, the other ingredients are polyethylene oxide, hypromellose, magnesium stearate, sodium chloride, red iron oxide, cellulose acetate, polyethylene glycol, hydroxypropylcellulose, propylene glycol, titanium dioxide (E171) and black iron oxide (E172 ).

Important information ▲

OROS tablets: If your doctor has prescribed you the maximum daily dose (120 mg of nifedipine) may result a daily intake of 2 mmol sodium, which should be considered if you need to follow a low sodium diet.

Capsules: This dosage form contains glycerol as excipient, which can be harmful at high doses. It can cause headache, stomach upset and diarrhea.

Capsules also contains orange yellow S (E110) as excipient. It can cause allergic reactions, including asthma, especially in patients allergic to aspirin.

Retard tablets: This medicine contains lactose. If your doctor has told you have an intolerance to some sugars, contact him before taking it.

Before taking the drug ▲

Do not use nifedipine

If you are allergic (hypersensitive) to the active substance nifedipine or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor take this medicine if:

- If you are being treated with rifampicin.
- If you are in the first 20 weeks of pregnancy or during breastfeeding.

You should not receive nifedipine. if you are in cardiovascular shock or suffer unstable angina or you have recently suffered an acute myocardial infarction.

Take special care with nifedipine

- if you have arterial hypotension (low blood pressure).
- if you suffer from gastrointestinal narrowing.
- if you are on dialysis and suffer malignant hypertension and a severe renal disease.
- If you have heart problems.
- If you have a liver disease.

Before starting treatment with this drug, tell your doctor:

- If you have diabetes.
- if you have a colostomy bags system.
- if you are having to do a radiologic examination since the test results may be altered by treatment with nifedipine OROS (GITS, nifedipine gastrointestinal therapeutic system).

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

Certain medicines can interact with nifedipine. In these cases, your doctor may adjust the dose or indicate to you stop taking any medications. This is especially important in the case of taking:

- Other medicines to lower high blood pressure; such as diltiazem;
- Cardiotonic, such as digoxin;
- Antiepileptics, such as phenytoin, phenobarbital, carbamazepine or valproic acid.
- Antiarrhythmic, such as quinidine;
- Immunosuppressive drugs, such as tacrolimus;
- Antibiotics, such as rifampicin, erythromycin, quinupristin or dalfopristin;
- Antidepressants, such as fluoxetine or nefazodone;
- Antifungals, such as ketoconazole, itraconazole or fluconazole;
- Antiretroviral drugs, such as indinavir, ritonavir, saquinavir, amprenavir, nelfinavir, delarvidine;
- Cimetidine;
- Cisapride.

It may be convenient to take special measures in case of radiologic examinationo.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Use with food and beverages

Do not drink alcohol while take nifedipine, nor drink grapefruit juice during treatment with nifedipine.

Special physiological situations

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy and lactation

This medicine is contraindicated in the first 20 weeks of pregnancy and during lactation.

Children

This drug is not recommended for use in children and adolescents.

Driving and using machines

Do not drive, because nifedipine may limit the ability to drive cars. Do not use any tools or machines, especially when starting treatment.

How to use nifedipine ▲

Follow exactly instructions of use of nifedipine provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Capsules:

Your doctor will determine the appropriate dose for your particular case. In general, the capsule should be swallowed unchewed using some liquid (not grapefruit juice).

Try to take the medication every day at the same time.

Your doctor will indicate the duration of treatment and starting dose of nifedipine. If you need to discontinue treatment, it is recommended to decrease the dose gradually.

Usually, the treatment regimen is a capsule three times a day (10 mg every 8 hours). If you need higher doses, your doctor will prescribe the dose required for your disorder.

Retard tablets:

Usually, the treatment regimen is one tablet twice a day (20 mg every 12 hours). If you need higher doses, your doctor will prescribe the appropiate dose.

OROS tablets:

Generally, the tablet should be swallowed without chewing with the help of a little liquid (not grapefruit juice).

The tablet should not be split because it would lose its sustained release properties. Try to take the medication every day at the same time.

Your doctor will indicate the duration of treatment with nifedipine and dose with which to start. If you require discontinuation of treatment, gradually dose reduction is recommended.

The usual dose is one tablet once a day (in the morning). If higher doses are needed, your doctor will prescribe the appropiate dose.

Use in elderly and in patients with liver failure:

If you become pregnant, contact your doctor immediately.

If a doctor prescribes another medicine , tell you are taking nifedipine.

Main symptoms of overdose include vomiting, cyanosis (bluish in distal areas, lips, hands, fingers, etc.), seizures, sweating and cardiocirculatory collapse.

If you forget a dose:

If you forget to take your medicine, you should take the dose as soon as you remember on the same day. Do not take a double dose to make up for missed doses.

Side Effects of nifedipine ▲

Like all medicines, nifedipine can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients

Common side effectss (affect 1 in 10-100 patients):

- Headache, dizziness.
- Edema (swelling), vasodilation.
- Constipation, nausea.
- General discomfort.

Uncommon side effects (affect 1 in 100-1.000 patients):

- Allergic reaction; allergic edema (swelling), angioedema, swelling of the larynx.
- Anxiety reactions, sleep disturbances, agitation, nervousness.
- Vertigo, migraine, tremor.
- Impaired vision.
- Tachycardia (rapid heart rate), palpitations, angina pectoris.
- Arterial hypotension, postural hypotension, syncope.
- Epistaxis (nosebleeds), nasal congestion, dyspnea (difficulty breathing), chest pain.
- Abdominal or gastrointestinal pain, dyspepsia (difficulty to digest), flatulence, dry mouth, diarrhea.
- Altered results of liver function tests .
- Rash, sweating.
- Muscle cramps, swollen joints.
- Increased secretion and excretion of urine.
- Erectile dysfunction.
- Unspecified pain, chills, asthenia.

Rare side effects (Affect less than 1 in 1.000 patients):

- Tingling in extremities, increased sensitivity to touch.
- Gingival hyperplasia (alteration of the gums)
- Abdominal distension, lack of appetite, vomiting.
- Muscle pain

Very rare side effects (affect less than 1 in 10.000 patients):

- Hypotension which can produce heart problems or ventricular fibrillation.
- Agranulocytosis, purpura (alteration of certain parameters of the blood),
- Hyperglycaemia (impaired blood glucose test)
- Exfoliative dermatitis and produced by photosensitivity, urticaria.
- Gynecomastia (breast growth in men)

Also, have been reported other possible adverse reactions, including severe allergic reactions, disnea (difficulty breathing), vomiting.

In dialysis patients with malignant hypertension and hypovolaemia can occur a significant decrease in blood pressure due to vasodilation.
