Indications and usage     

Zonisamide is an antiepileptic medicine.

This medicine is used to treat adults who are already taking other antiepileptic but continue to have seizures that affect one part of the brain (partial seizure), which may or may not be followed by a crisis affecting the whole brain (secondary generalization).

Medizzine recommends consult your doctor if you are unsure (or you do not know) why you have been given zonisamide.

Other ingredients of available commercial presentations:

The active ingredient is zonisamide. The other ingredients present in the capsule contents are: microcrystalline cellulose, hydrogenated vegetable oil and sodium lauryl sulphate. The capsule shell contains: gelatin, titanium dioxide (E171), shellac, propylene glycol, potassium hydroxide and black iron oxide (E172). In addition, capsule's cover contains sunset yellow FCF (E110) and Allura Red (E129).

Important information     

This medicine contains a yellow dye called sunset yellow (E110) which may cause allergic reactions, possibly delayed.

Before taking the drug     

Do not use zonisamide

If you are allergic (hypersensitive) to the active substance zonisamide or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Take special care with zonisamide

- If you are elderly, as it may be necessary to adjust the dose of zonisamide and may be more likely to develop an allergic reaction or severe skin rash when taken
- If you have liver problems, as it may be necessary to adjust the dose of the medicine
- If you have kidney problems, as it may be necessary to adjust the dose.
- If you have previously suffered from kidney stones, as you may be at increased risk of developing more kidney stones. Reduce the risk of kidney stones drinking enough amount of water.
- If you live in a place or go on vacation to a place where it's warm. This medicine may make you sweat less, which can cause your body temperature to increase. You can reduce the risk of reaching an overheating by drinking sufficient water amount and keeping yourself cool.
- If you weigh less than 40 kg, or have lost a lot of weight, because zonisamide can cause you to lose more weight. Tell your doctor as it may be necessary to control it.

If any of these circumstances are applicable to you, tell your doctor before taking it.

Contact your doctor immediately if

- You have difficulty breathing, swelling of the face, lips or tongue (angioedema), or a severe rash as these symptoms may indicate you are suffering a severe allergic reaction.
- You have muscle pain or weakness, as this may be a sign of abnormal muscle breakdown which can lead to kidney problems.
- You have a sudden pain in your back or stomach, you have pain while urinating or you notice blood in the urine, as this may be a sign of kidney stones.

Contact as soon as possible with your doctor if:

- You suffer an unexplained skin rash, because it can become a more severe rash.
- You feel unusually tired or feverish, you have a sore throat, swollen glands or you see that come out bruise more easily, as this may mean you have a blood disorder.

Your doctor may decide that you should stop taking zonisamide.

If in doubt, consult your doctor.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any medications, even those without a prescription or herbal remedies.

The effect of zonisamide can be reduced by other medicines you are taking. This may require an adjustment of the dose of the drug.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Use with food and beverages

Zonisamide can be taken with or without food.

Special physiological situations

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy and lactation

Tell your doctor right away if you believe or are pregnant or planning to become pregnant.

Zonisamide should not be used if you are pregnant, planning to become pregnant or think you might be pregnant or you are breastfeeding. Women of childbearing age must use contraception while taking this medicine and for one month after.

Zonisamide may affect your concentration, ability to react or respond, and may make you feel sleepy, particularly at the beginning of treatment or after a dose increase. If taking the medication affects you in this way, take special care when driving or using machines.

How to use zonisamide     

Follow exactly instructions of use of zonisamide provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Capsules should be swallowed whole with water. Do not chew.

The drug should be taken once or twice daily, as directed by your doctor.

Taking it twice a day, take half the daily dose in the morning and half at night.

Usual adult dose :

The starting dose is 50 mg per day divided into two equal doses of 25 mg.

It can be increased up to 100 mg at intervals of one to two weeks. The usual daily dose is between 300 mg and 500 mg.

Some people respond properly to lower doses. The dose may be increased more slowly if you experience unwanted side effects or if you have kidney or liver problems.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take zonisamide     

If a doctor prescribes another medicine , tell you are taking zonisamide.

Check with your doctor regularly developments of disorder that motivates the use of zonisamide. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with zonisamide at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with zonisamide, immediately consult your doctor.

If you take more than you should:

If you have taken more medicine than you should, let it be known right away to your carer (relative or friend), your doctor or pharmacist, or contact the emergency department of the nearest hospital, carrying your medicine with you. You may have drowsiness and could lose consciousness.

Do not attempt to drive.

If you forget to take zonisamide

If you forget to take a dose, do not worry: take the next dose at the usual time. Do not take a double dose to make up for missed doses.

If you stop taking this medicine

Zonisamide is intended to be taken as long term medication. Do not reduce your dose or stop taking medication unless directed by your doctor.

If your doctor advises you to stop treatment, will reduce gradually the dose to decrease the risk of further crises.

Side Effects of zonisamide     

Like all medicines, zonisamide can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Zonisamide belongs to a group of medicines (sulphonamides) that can cause severe allergic reactions, severe skin rashes, and blood disorders, which, very rarely, can be fatal .

Contact your doctor immediately if:

You have difficulty breathing, swelling of face, lips, or tongue (angioedema) or severe rash as these symptoms may mean you are having a severe allergic reaction.

You have muscle pain or you are weak, as this may be a sign of abnormal muscle breakdown which can lead to kidney problems (rhabdomyolysis).

You suffer a sudden pain in your back or stomach, you have pain while urinating or you notice blood in urine, as this can be a sign of kidney stones.

Contact as soon as possible with your doctor if

- you suffer an unexplained rash, because it can become a more severe disorder.
- You feel unusually tired or feverish, have a sore throat, swollen glands or notice that you bruise more easily, as this may mean you have a blood disorder.

Your doctor may decide you should stop taking this drug.

Side effects reported most often are mild. Are more frequently during the first month of treatment, but decrease often with continued treatment.

Common side effects (affect 1 in 10-100 patients)

- Agitation, irritability, confusion, depression, insomnia, unusual or strange thoughts, feeling anxious or emotional instability.
- Poor muscle coordination, dizziness, bad memory, slow thinking, drowsiness, loss of concentration, abnormalities in speech, abnormal skin sensation (tingling), tremor.
- Double vision, involuntary eye movements.
- Kidney stones.
- Rashes, allergic reactions, fever, fatigue, flu-like symptoms.
- Ecchymosis (a small bruise caused by blood leaking from broken blood vessels in the skin).
- Loss of appetite, weight loss, decreased bicarbonate levels (a substance that prevents blood becomes acidic).
- Nausea, indigestion, stomach aches, diarrhea, constipation.

Uncommon side effects (affect 1 in 100-1.000 patients)

- Anger, fury, aggressiveness, suicidal thoughts, suicide attempt.
- Vomiting.
- Inflammation of the gallbladder or gallstones.
- Urinary stones.
- Pneumonia and urinary tract infections.
- Low levels of potassium in the blood and convulsions / seizures.

Very rare side effects (affect less than 1 in 10.000 patients):

- Hallucinations, memory loss, coma, neuroleptic malignant syndrome (inability to move, sweating, fever, incontinence), status epilepticus (prolonged or repeated seizures).
- Respiratory disorders, difficulty breathing, inflammation of lungs.
- Inflammation of the pancreas (severe stomach pain or back)
- liver problems, renal failure, Increased levels of creatinine (a waste product that your kidneys should normally remove).
- Severe rashes (at the same time may feel unwell and have fever, itching).
- Abnormal muscle breakdown (you may feel pain or weakness) which can lead to kidney problems.
- Swollen glands, blood disorders (reduction in the number of blood cells, which can make it more likely to have infections, feel tired, pale and feverish and having bruise more easily).
- Decreased sweating, Excessive body temperature.
