Indications and usage     

Allopurinol belongs to a group of drugs called enzymatic inhibitors; these substances act by controlling the speed to which occur a given chemical process in the body; in this case, allopurinol produces a lowering of uric acid level in plasma and urine

Allopurinol is used to prevent the onset of gout and other conditions caused by excess uric acid in the body, such as kidney stones and certain types of renal or metabolic diseases.

Before taking the drug     

Do not use allopurinol

If you are allergic (hypersensitive) to allopurinol or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Take special care with allopurinol

- If you have an acute gout attack.
- if you are pregnant, might be pregnant or are breastfeeding your child.
- if you have or have had a kidney or liver disease.
- if you are taking or will begin taking a medication for a heart disease or for high blood pressure.
- If you notice that you bruise more easily than before or if it appears a sore throat or other signs of infection.
- If appears a rash, peeling skin, blisters or sores on the lips or in the mouth, wheezing, palpitations or chest tightness, unconsciousness: These symptoms may mean you are ALLERGICto allopurinol. Discontinue treatment and immediately contact your doctor.

You should stop treatment and contact your doctor as soon as possible if, while you are taking allopurinol, present any of the symptoms described below:

- High fever.
- Joint pain and painful swelling of the groins, armpit or neck.
- Jaundice (yellowing of the skin and eyes).
- Nausea or vomiting (which may be bloody).
- A general feeling of malaise.
- Weakness, numbness or loss of consciousness.
- Headaches, drowsiness, dizziness, impaired vision.
- Chest pain, high blood pressure or pulse slowing.
- Swelling (edema) of the ankles.
- Thirst, fatigue and weight loss.
- The aAppearance of boils.
- The appearance of blood in urine.

It is possible that, when starting treatment with allopurinol, occurs an acute attack of gout. Your doctor will recommend the use of certain medications to prevent it.

If a gout attack occurs, it is not necessary to stop treatment with allopurinol, but is required simultaneously a suitable anti-inflammatory drug.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

Especially, tell your doctor or pharmacist if you are taking:

6-mercaptopurine, azathioprine, adenine arabinoside (vidarabine), salicylates (aspirin and other), uricosuric agents (probenecid), chlorpropamide, theophylline, phenytoin, ampicillin, amoxicillin, cyclophosphamide, doxorubicin, bleomycin, procarbazine, mechlorethamine, cyclosporine, coumarin anticoagulants (acenocoumarol, warfarin) and didanosine.

Use with food and beverages

Usually, allopurinol is taken once a day, after meals

Special physiological situations

Pregnancy and lactation

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Allopurinol is not recommended if you are pregnant or while breastfeeding.

Elderly

The doctor will prescribe the lowest dose that produces a satisfactory effect. Caution should be exercised in yhe case of impaired renal function.

Driving and using machines

Because of its side effects (drowsiness, dizziness, and impaired coordination), allopurinol may impair your ability to drive.

How to use allopurinol

This medicine is given only orally

Swallow the tablet whole with the help of a little water.It is usually taken once a day, usually after meals.

Usual doses:

Adults:

The usual dose of allopurinol to begin treatment is 100 to 300 mg, which may be increased up to 900 mg a day, depending on each patient. In this case and, if stomach upset occurs, it is advisable to divide the total dose into several doses per day and accompany each one with a little food.

Children under 15 years:

The usual dose of this medicine in children is 100 to 400 mg daily.

Elderly:

Your doctor will recommend the lowest possible allopurinol dose for reducing levels of uric acid and control your symptoms.

Use in patients with impaired liver or kidney function:

If you have liver or kidney problems, your doctor will prescribe the lowest possible allopurinol dose which successfully reduces uric acid levels and control your symptoms.

In the case of renal failure, your doctor may advise taking less than 100 mg per day or to take a single dose of 100 mg at longer intervals than one day.

. If you are on dialysis 2 or 3 times a week, your doctor may prescribe a dose of 300-400 mg immediately after each dialysis session.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take allopurinol     

If a doctor prescribes another medicine, tell you are taking allopurinol.

Do not restart treatment with allopurinol at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with allopurinol, immediately consult your doctor.

If you take more than you should:

If you take more allopurinol than you should, tell your doctor or go to the nearest hospital casualty department straight away. Take the medicine pack with you, even if it is empty.

If you forget to take allopurinol

If you forget a dose, take it as soon as you remember.Then continue taking the medicine as usual

Do not take a double dose to make up for a forgotten dose.

If you miss several doses, it is best to contact your doctor so he decides what to do you from that point.

If you stop taking allopurinol

Keep taking allopurinol until your doctor tells you to stop. Your doctor will indicate the duration of treatment with allopurinol. Do not stop treatment early, even if you feel better .

Side Effects of allopurinol     

Like all medicines, allopurinol can cause side effects, although not everybody gets them.

The likelihood of side effects is greater when there are kidney and/or liver disorders.

Infections and infestations

Very rare: Infection of the hair follicle

Disorders of blood and lymphatic system

Very rare: Decreased number of white blood cells (increased risk of infection), red blood cells (may make you tired, fatigue), platelets (cells involved in clotting blood, appearing bruising or bleeding more easily than usual)

Immune system disorders

Uncommon:Hypersensitivity reactions (allergy: possible appearance of skin rash, peeling skin, blisters or sores on the lips or in the mouth)

Rare: Serious hypersensitivity reactions associated with exfoliation, fever, joint pain or painful swelling of the groins, armpit or neck, jaundice (yellowing of the skin and eyes), and other disorders of the skin and subcutaneous tissues (see below).

Very rare: Angioimmunoblastic lymphadenopathy (swelling in armpits, neck, groins)

Very rarely may occur seizures, wheezing, palpitations, chest tightness or unconsciousness.
