Indications and usage ▲

Bisoprolol is a drug belonging to the family of beta-blockers. Its therapeutic activity is mainly based on its effect on the heart, reducing the frequency of beats.

This medicine is used to treat arterial hypertension (high blood pressure) and stable chronic angina.

This substance is also used to treat stable chronic heart failure.

Bisoprolol is used in combination with other suitable medicines for this condition (such as ACE inhibitors, diuretics and cardiac glycosides).

Before taking the drug ▲

Do not use bisoprolol

If you are allergic (hypersensitive) to bisoprolol or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you have any of the following heart diseases: acute heart failure or heart failure decompensation requiring specific treatment, cardiogenic shock, AV block second or third degree and you do not carry a pacemaker, sick sinus syndrome, sinoatrial block, bradycardia (low heart rate) or low blood pressure (hypotension).
- If you have severe asthma or lung disease.
- If you suffer from circulation in the legs (such as occlusive diseases of blood vessels and Raynaud's Syndrome).
- If you suffer metabolic acidosis.
- If you have pheochromocytoma and are not being treated.
- If you are taking floctafenine and sultopride.

Take special care with bisoprolol

If you have any of the following diseases, always consult your doctor before taking this medicine:

- Asthma and other obstructive respiratory diseases. Your doctor will inform you and decide whether you should take any other medicine.
- Poorly controlled diabetes.
- If you suffer allergic reactions, bisoprolol can cause or worsen these reactions. You should also be careful if you receive certain allergy treatments (desensitization therapy).
- Heart disease as a first-degree atrioventricular block or the Prinzmetal angina.
- Impaired blood circulation in the extremities such as Raynaud's syndrome or intermittent claudication.
- Psoriasis or a history of psoriasis.
- Thyroid diseases.
- If you have heart failure, your doctor will evaluate which is the best way to treat the disease.
You should not stop treatment abruptly. The dose should be slowly reduced
- If you are to undergo anesthesia.
- In patients with pheochromocytoma.
- During strict fasting.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

Is strictly contraindicated the concomitant use of bisoprolol and floctafenine (anti-inflammatory) and/or sultopride.

Be especially careful if you are taking any of these medicines:

- anti-inflammatory drugs (NSAIDs)
- medicines for migraine (ergotamine derivatives).
- drugs for treating cardiovascular diseases (eg. verapamil, diltiazem, bepridil, clonidine, amiodarone, disopyramide, quinidine, dihydropyridine derivatives, digitalis).
- monoamine oxidase inhibitors (except MAO-B inhibitors).
- drugs for treatment of diabetes (insulin and oral antidiabetic since bisoprolol may increase hypoglycemia).
- eye drops for treating glaucoma.
- medicines to relieve nasal congestion.
- anesthetics, tacrine, barbiturates and antidepressants, phenothiazines and other antihypertensive agents.
- baclofen.
- Iodinated contrast agents.
- Corticosteroids.
- rifampicin (to treat infections) and mefloquine (for malaria).

Special physiological situations

Pregnancy and lactation

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Bisoprolol has pharmacological properties that may cause harmful effects on pregnancy and/or neonate. If you are pregnant or plan to become pregnant you should tell your doctor, who will indicate whether you can take this drug during pregnancy.

It is unknown whether bisoprolol passes in to breast milk. Therefore, breastfeeding is not recommended during treatment with bisoprolol.

Children

There is not enough available clinical studies at this age, so its use is not recommended.

Driving and using machines

This medicine may affect your ability to drive or operate machinery. This occurs mainly at starting treatment or when changing the dose

How to use bisoprolol ▲

Follow carefully instructions of use of bisoprolol provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Bisoprolol in tablets is for oral administration.

Bisoprolol is taken in the morning and can be ingested with food. The tablets should be swallowed whole with some liquid and without chewing.

The usual dose for treatment of hypertension or angina is 5 mg once a day. If your doctor considers it necessary, he or she may increase your dose up to a daily maximum of 10-20 mg bisoprolol once daily or can also indicate a lower dose.

When applied for heart failure, bisoprolol should be started at a low dose and gradually increase.

Your doctor will decide how to increase the dose, and this usually will be done as follows:

- 1.25 mg once a day for a week.
- 2.50 mg once a day for a week.
- 3.75 mg once a day for a week.
- 5 mg once a day for four weeks.
- 7.5 mg once a day for four weeks.
- 10 mg once daily as maintenance therapy (continuously).

This scheme can be modified at the discretion of your doctor.

In patients with severely impaired kidney or liver maximum daily dose is 10 mg.

Bisoprolol is generally used as long-term treatment. Your doctor will tell you how long to take it. Never stop treatment before or abruptly, as it may cause a worsening of your disease.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take bisoprolol ▲

If a doctor prescribes another medicine, tell you are taking bisoprolol.

Check with your doctor regularly developments of disorder that motivates the use of bisoprolol. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with bisoprolol at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with bisoprolol, immediately consult your doctor.

If you take more than you should:

If you take more bisoprolol than you should, tell your doctor or go to the nearest hospital casualty department straight away. Take the medicine pack with you, even if it is empty.

If you forget to take bisoprolol

If you forget a dose, take it as soon as you remember it unless it is nearly time for your next dose. Then continue taking the medicine as usual

Do not take a double dose to make up for a forgotten dose.

If you stop taking bisoprolol

Keep taking bisoprolol until your doctor tells you to stop. Your doctor will indicate the duration of treatment with bisoprolol. Do not stop treatment early, even if you feel better . If you need to stop treatment, it must be done gradually to avoid side effects

Side Effects of bisoprolol ▲

Like all medicines, bisoprolol can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Common side effects

Circulation: Cold feeling or numbness in extremities, Raynaud's disease, increase of existing intermittent claudication.

Neurologic side effects Tiredness, exhaustion, dizziness, headache (especially at start of therapy, usually mild and disappear within 1-2 weeks).

Gastrointestinal side effects: Nausea, vomiting, diarrhea, abdominal, pain and constipation

Uncommon side effects

General disorders: Muscular weakness and cramps, arthropathy.

Circulation: decreased or altered pulse, worsening heart failure, decrease in blood pressure when standing up.

Neurologic side effects: Sleep disorders, depression.

Respiratory side effects: difficulty breathing in asthmatic patients or with obstructive pulmonary diseases.

Rare side effects

Neurologic disorders: nightmares, hallucinations.

Skin disorders: itching or redness, rashes, blush.

Liver disorders: Increased levels of hepatic enzymes, hepatitis.

Metabolic disorders: increased triglycerides, hypoglycemia (low blood sugar).

Urogenital disorders: disorders of sexual potency

Otorhinolaryngologic disorders: hearing loss, inflammation of the inner lining of the nose (allergic rhinitis).

Eye disorders: decreased tearing (this should be considered in patients who wear contact lenses)

Immune system disorders: Lupus-like syndrome that disappears after cessation of treatment.

Case reports

Eye: Conjunctivitis.

Skin: cause or worsen psoriasis or inducir erupción del tipo psoriasis, hair loss.
