Indications and usage ▲

Candesartan cilexetil belongs to a group of medicines called angiotensin II receptor antagonists.(ARBs), which work by blocking the effects of the hormone angiotensin II. This medicine causes relaxation of blood vessels and thus a decrease in blood pressure and also facilitates to heart pumping blood to all parts of the body.

This medicine is used to treat hypertension (high blood pressure). It is also indicated for the treatment of heart failure, besides medications called ACE inhibitors or when these are not tolerated.

Other ingredients of available commercial presentations:

The active ingredient is candesartan cilexetil (INN). Each tablet contains 4, 8, 16 or 32 mg candesartan cilexetil. The other ingredients are carmellose calcium, hydroxypropylcellulose, lactose monohydrate, magnesium stearate, corn starch, oxide reddish-brown iron (E172) and macrogol.

This medicine contains lactose. If your doctor has told you that you have an intolerance to some sugars, contact him before taking it.

Before taking the drug ▲

Do not use candesartan

If you are allergic (hypersensitive) to candesartan or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

– If you are pregnant, you think you may be pregnant, planning to become pregnant or you are breastfeeding.
– If you have a severe liver disease.

Take special care with candesartan

Consult your doctor or pharmacist before taking this medicine:

– If you are taking other medicines to lower blood pressure, especially diuretics.
– If you are taking any other medicine including those obtained without a prescription.
– If you have heart, liver or kidney problems.
– If you are diabetic.
– If you are taking lithium.
– If you are taking potassium-sparing diuretics, potassium supplements, salt substitutes containing potassium or other drugs such as heparin.
– If you are vomiting or have diarrhea.

Patients having changes in liver function should tell to the doctor before starting candesartan treatment. In this case, the physician may prescribe a lower starting dose.

In patients with heart failure, should regularly monitor kidney function, mainly creatinine and potassium levels in elderly from 75 years and patients with impaired renal function.

In patients with very severe renal impairment or end-stage renal disease, including hemodialysis patients, the dose should be carefully adjusted, controlling blood pressure.

In patients with risk of hypotension (eg with intravascular volume depletion) is recommended a starting dose of 4 mg.

Black patients may need to increase the candesartan's doses since the effect of this drug is less in these persons.

The active substance candesartan is not recommended in children and adolescents (under 18).

Anesthesia and surgery:

If you are going to have surgery, tell your doctor or dentist you are taking candesartan. This substance, combined with some anesthetics, may cause a drop in blood pressure.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

- Administration of other drugs with antihypertensive properties, although these have been prescribed for other use, as they may increase the antihypertensive effect of candesartan.

- Concomitant use of potassium-sparing diuretics, potassium supplements, salt substitutes containing potassium or other drugs that can cause increased potassium levels, such as heparin, can lead to an increase in serum potassium.

- Have been reported a reversible increase in serum lithium concentrations during concomitant candesartan administration.

- The antihypertensive candesartan effect may be diminished by non-steroidal anti-inflammatory drugs (NSAIDs) such as indomethacin.

- When taking candesartan with ACE inhibitor (another medicine for heart failure) may increase the risk of adverse reactions, especially kidney disease and increased blood potassium.

Use with food and beverages

No matter take it with or without food or beverages.

Special physiological situations

Pregnancy and lactation

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Do not take candesartan during pregnancy and/or during lactation. If you become pregnant during treatment with this medicine, consult your doctor as soon as possible.

Children

The active substance candesartan is not recommended in children and adolescents (under 18)

Driving and using machines

Treating high blood pressure can cause fainting or dejection in some patients. Therefore, you must ensure that you do not experience these symptoms before driving or using machinery.

How to use candesartan ▲

Follow carefully the instructions of use of candesartan provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Your doctor will tell you how long your treatment with candesartan. Do not stop the treatment before your doctor tells you. Do not take higher doses than indicated by your doctor.

Method and route of administration

The dose should be individualized. Carefully follow your doctor's instructions. It is important to take the medicine every day as directed by your doctor.

Usual doses:

Hypertension

The recommended starting dose and usual maintenance dose is 8 mg once daily. Your doctor may increase the dose to reduce blood pressure to the desired level. This increase can be up to 32 mg once daily (1 tablet of 32 mg). It is best to take the tablet every day at the same time eg morning.

If you have impaired liver function, your doctor may prescribe a starting dose of 2 mg once a day.

If you have impaired kidney function, including hemodialysis patients, the initial dose is 4 mg.

In patients at risk for hypotension (for example, with an intravascular volume depletion) it is recommended a starting dose 4 mg candesartan

In black patients may need to increase the dose.

Heart failure:

The usual starting dose is 4 mg once daily. The dose may be doubled at intervals of at least two weeks up to 32 mg or highest tolerated dose.

This medicine may be given along with other treatments such as ACE inhibitors, beta blockers, diuretics or combinations thereof.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take candesartan ▲

If a doctor prescribes another medicine, tell you are taking candesartan.

Check with your doctor regularly developments of disorder that motivates the use of candesartan. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with candesartan at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with candesartan, immediately consult your doctor.

If you take more than you should:

If you take more candesartan than you should, tell your doctor or go to the nearest hospital casualty department straight away. Take the medicine pack with you, even if it is empty.

If you forget to take candesartan

If you forget a dose, just continue with the next dose as it has been prescribed, do not take a double dose to make up the missed dose.

If you stop taking candesartan

Keep taking candesartan until your doctor tells you to stop. Your doctor will indicate the duration of treatment with candesartan. Do not stop treatment early, even if you feel better.

Side Effects of candesartan ▲

Like all medicines, candesartan can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Common side effects include dizziness, headache, cold or flu symptoms.

Very rare side effects : swelling of the face, lips, tongue and/or throat (angioedema), nausea, joint or muscle pain, back pain, rash, urticaria, itching, jaundice (yellowish eyes or skin) or increase in liver enzymes y hepatitis (inflamed liver).

Also, have been described changes in renal function (especially in patients with previous kidney or heart problems). May occur changes in potassium or sodium levels and in red blood cell counts or leukocytes. Such changes are usually detected by a blood test.

If you notice any of these symptoms, or any other unusual or unexpected reaction occurs, consult your doctor or pharmacist.

If you notice swelling of the face, lips, tongue, eyes and/or throat or jaundice, contact your doctor immediately.
