Ciprofloxacin hydrochloride is a broad spectrum antibacterial substance and oral administration. It belongs to the group of fluoroquinolones, with which it shares most of its properties and medical applications.

This medicine is indicated in the infections caused by bacteria sensitive to its action, especially:

Urinary tract infection, including pyelonephritis and other infections of the urinary tract, acute or chronic prostatitis.
Respiratory tract infections pneumonia, sinusitis, chronic and acute bronchitis, and exacerbations of the chronic bronchitis
Skin infections
Bone and joint infections: Osteomyelitis, osteoarthritis
Complicated infections of the abdominal cavity : Used in conjunction with other antimicrobial
Severe diarrhea: When indicated antibacterial drug treatment.
Sexually transmitted diseases: Gonorrhea in man and woman
Typhoid fever
Preventive treatment after exposure to Anthrax by inhalation
Indication for children: Complicated urinary tract infection. Only in very selected cases

The use in children under 18 should be considered exceptional, given the high risk of serious side effects, especially those related to the growth plate of long bones.

Do not use ciprofloxacin

If you are allergic (hypersensitive) to ciprofloxacin or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

If you suffer pain or inflammation of some sort in the calf area, paying particular attention to the Achilles' tendon. If this occurs, discontinue treatment, remain in complete rest and immediately tell your doctor.

If you have any alteration in the function of the kidneys, as your doctor will adjust the dose of ciprofloxacin.

If you are taking tizanidine

Take special care with ciprofloxacin

- If you have or have had epileptic seizures and/or convulsions; changes in blood flow or you have or have had thrombosis and/or cerebral hemorrhage. Ciprofloxacin can cause seizures. Talk to your doctor if he/she prescribes you this medicine.
- If you are over 65 years and are taking steroids (anti-inflammatory drugs, such as prednisone, deflazacort, dexamethasone or similar); a quinolone (the group of drugs to which belongs ciprofloxacin), can cause inflammation and rupture of tendons. At the first sign of pain or inflammation, treatment should be stopped, maintaining in rest the affected limb and consulting your doctor.
- If you develop severe or durable diarrhea, evidenced during or after treatment with ciprofloxacin, because it can be caused by a special type of colitis called pseudomembranous colitis, which can be severe. In this case, you should consult your doctor immediately.
- If you have a personal or family history of glucose-6-phosphate dehydrogenase deficiency (congenital or acquired metabolic disorder).
- During treatment with this medicine, it is advisable to avoid exposure to ultraviolet radiation and not expose yourself excessively to sunlight; if you do so, should protect your skin properly.
- If you have myasthenia gravis, since in these cases, the active ingredient ciprofloxacin should be used with caution.

Patients with impaired renal function

Although the drug is eliminated primarily by the kidney, it is not essential that the function of this organ be optimal, since it is partially eliminated through the liver and bile. If you have an impaired kidney function, tell your doctor before he prescribes ciprofloxacin.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

Some drugs can influence the action of ciprofloxacin, such as:

Antacids, mineral supplements, sucralfate :

Taking these drugs simultaneously with ciprofloxacin causes a decrease in the effect of ciprofloxacin. By this reason, it is recommended to take ciprofloxacin 1 to 2 hours before or 4 hours after taking these products.

Dairy products and beverages enriched with minerals:

It is recommended not to take these products simultaneously with ciprofloxacin because it can cause a decrease in the effect of this.

Theophylline, caffeíne, methylxanthine or pentoxifylline:

The blood levels of these drugs increase when taken together with ciprofloxacin, so the risk of adverse effects from these drugs also increases.

Nonsteroidal anti-inflammatory drugs (NSAIDs):

Taking these drugs (except acetylsalicylic acid) along with quinolones at very high doses may cause convulsions.

Cyclosporine

Cyclosporine is a drug used to prevent rejection in transplants. When taken together with ciprofloxacin may cause alterations in the function of the kidneys.

Other drugs whose effect may increase

- Warfarin(medicine used to thin blood and prevent clots),
- Glyburide (medicine to treat diabetes),
- Phenytoin (a medicine used in the treatment of epilepsy),
- Ropinirole (a medicine used in the treatment of Parkinson's disease),
- Methotrexate (a medicine used in the treatment of rheumatoid arthritis or cancer chemotherapy),
- Clozapine (a medicine used in the treatment of schizophrenia),
- Tacrine (drug used in the treatment of Alzheimer's disease),
- Mexiletine (a medicine used in the treatment of arrhythmias),
- Duloxetine (medicine to treat depression).

The effect of these drugs may increase when taken together with ciprofloxacin.

Drugs that increase the effect of ciprofloxacin

Probenecid (a medicine used in the treatment of gout), metoclopramide (a medicine used to treat nausea and vomiting): when taken together with ciprofloxacin, can increase its blood levels and therefore increase the effects of this drug.

Drugs that reduce the effect of Ciprofloxacin

Opioid premedication (medicines used for preoperative sedation) and omeprazole (a drug used for treating ulcers): when taken together with this drug can reduce blood levels of ciprofloxacin, and therefore decrease its effects.

Use with food and beverages

This medicine may be taken with meals or out of them. As with other fluoroquinolones, simultaneous administration of antacids, sucralfate, medicines containing calcium or iron, zinc or didanosine should be avoided because absorption of ciprofloxacin is reduced. If you must take these other drugs at the same time, the antibiotic can be taken two hours before or six hours later. For the same reason not be taken with dairy products or calcium-fortified juices.

Special physiological situations

Pregnancy and lactation:

Always consult your doctor or pharmacist before taking any medicine during pregnancy or breastfeeding.

Pregnancy:

It has not been established the safety of this drug in pregnant women, so it can not be ruled out a possible harm to the fetus. Ciprofloxacin belongs to the category C in the FDA classification.

Lactation:

It has not been established the safety of use in nursing mothers, so its use should be avoided.

Ciprofloxacin passes into the milk of women. It is unknown the exact amount of the drug that can ingest a baby, although should be discouraged the simultaneous breastfeeding the child and using this medicine due to the potential risks. Your doctor will advise you about which of these two alternatives should follow.

Children:

Ciprofloxacin is only indicated in patients under eighteen years of age in exceptional situations, already mentioned. In addition to the possible deleterious effect on growth, many children suffer more side effects than adults.

Elderly:

Elderly patients with good kidney function no special problems during treatment with ciprofloxacin. Ask your doctor if you have any impairment of renal function if he prescribes this medicine.

Driving and using machines

Ciprofloxacin may impair your ability to drive or use machinery. The risk increases during the first days of treatment, if you drink alcohol or when the drug dose is increased. Therefore, do not drive or use dangerous machinery if you are in some of these situations

The duration of treatment is usually 7 to 14 days, depending on the severity of the case. Some infections may require longer treatments (bone infections, prostatitis) .The usual adult dose is 500 mg every 12 hours. In the case of treatment or prevention of anthrax infection, the usual dose is 500 mg every 12 hours orally, with the usual duration of treatment or prevention (60 days).

The usual treatment period for acute infections is 5 to 10 days. However, treatment should be continued for 3 days after the disappearance of signs and symptoms. If you have improved without completing the treatment prescribed by your doctor, you should finish it as directed. Otherwise, the effectiveness of treatment can decrease and increase the risk that the bacteria will become resistant to ciprofloxacin.

Ciprofloxacin can be taken with meals or out of them . As with other similar drugs (fluoroquinolones), simultaneous administration of antacids, sucralfate, medicines containing calcium or iron, zinc or didanosine should be avoided because absorption of that is reduced. If you must take these other drugs at the same time, ciprofloxacin can be taken two hours before or six hours later. For the same reason, ciprofloxacin should not be taken with dairy products or "rich in calcium" juices.

Overall, the tablet should be swallowed whole without chewing, with the help of some liquid (eg a glass of water). We recommend drinking plenty of fluids during treatment with this medicine.

Elderly:

The dose is equivalent to that used in adults. Only needs to adjust in the case of an alteration in the renal function.

Children and adolescents (5-17 years):

The recommended dose is 20 mg/ kg every 12 hours orally; the maximum daily dose is 1.500 mg. For the treatment or prevention of anthrax infection, the recommended dose is 10-15 mg/kg every 12 hours orally .

Patients with impaired kidney function:

If you suffer any alteration in renal function, tell your doctor, as you may require a dose adjustment.

Patients with impaired liver function:

In adults, does not require dose adjustment.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

If a doctor prescribes another medicine, tell you are taking ciprofloxacin.

If you take more than you should:

Symptoms you may experience include dizziness, tremor, headache, fatigue, confusion, upset stomach and intestines and abnormalities in renal and hepatic function.

Consult immediately your doctor or pharmacist or consult a Poison Control Center in your country or go right away to the nearest hospital. Take the pack with the rest of unconsumed medicine.

If you forget to take ciprofloxacin

If you forget a dose, take it as soon as you remember and continue the prescribed treatment as usual. However, if is near the time for the next dose, it's better not take the missed tablet, taking the next as usual.

Do not take a double dose to make up for forgotten individual doses.

If you stop taking ciprofloxacin

You should not stop treatment without the advice of your doctor, as it could worsen and reproduce infection.
