Erythromycin is an antibiotic belonging to the macrolide group.

This medicine is primarily used to treat throat infections, nose, ears and lower respiratory tract. It is also used to treat skin infections ant to prevent infections in people with a history of a rheumatic disease, congenital heart disease or other acquired valvular heart disease and who are allergic to penicillin antibiotics .

Tablets:

Besides the active ingredient, the tablets also contain the following inactive ingredients:

Calcium hydrogen phosphate anhydrous, maize starch, sorbic acid, povidone, purified talc, sodium starch glycollate, magnesium stearate, Opadry Pink OY-B-34901 (containing titanium dioxide), and iron oxide red.

The tablets also may contain sucrose, sodium citrate, polacrilin potassium, hydroxypropyl cellulose, propylene glycol, macrogol 400, macrogol 8000, allura red AC, brilliant scarlet 4R (also called red Ponceau 4R), indigo carmine, and sunset yellow FCF.

Suspension/granules/mixture:

Besides the active ingredient, these presentations also contain sorbitol, propylene glycol alginate, sodium citrate, colloidal anhydrous silica, aspartame, sodium benzoate, erythrosine, cherry flavor (contains preservative).

The granules also may contain carmellose sodium, sodium citrate, aluminum magnesium silicate, erythrosine, citric acid anhydrous, flavor cherry imitation, sucrose, saccharin sodium, and sodium cyclamate.

Capsules:

The capsules contain 250 mg erythromycin as the active ingredient. It also contains lactose, povidone, cellacephate, diethyl phthalate, gelatin, shellac, potassium phosphate, erythrosine, indigo carmine, sunset yellow, and carbon black.

Do not use erythromycin

If you are allergic (hypersensitive) to erythromycin to other macrolide antibiotics or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Also, you should not take this medicine if:

- You are taking allergy medicines (antihistamines) as terfenadine or astemizole (medicines retired in most countries)
- You are pregnant, you may be pregnant or planning to become pregnant
- You are breastfeeding your baby
- You are taking cisapride (medicine already retired in most countries)
- You are taking dihydroergotamine or ergotamine (migraine drugs)
- You are taking pimozide or sertindole for psychiatric problems
- You are taking simvastatin (a drug for cholesterol)
- You are taking theophylline or derivatives (a medicine for asthma and bronchitis)
- You are taking mizolastine (an allergy medicine)

Take special care with erythromycin

- If you have a liver disease
- If you suffer from myasthenia gravis, a muscle disease.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

There are no known interactions of erythromycin with other medicines.

Especially:

- Cholesterol lowering drugs (atorvastatin, lovastatin, simvastatin, etc)
- Drugs for Parkinson's disease (bromocriptine, cabergoline)
- Anxiety medicines, as buspirone
- Epilepsy drugs (carbamazepine, hexobarbital, phenytoin, valproate)
- Anti-infective drugs (chloramphenicol, clindamycin, lincomycin)
- Medications for circulation, as cilostazol
- Antiulcer drugs (cimetidine) or other disorders (cisapride)
- Medications for psychiatric disorders, as quetiapine or clozapine
- Medicines for heart disease (digoxin, disopyramide, quinidine)
- Medications for high blood pressure (felodipine)
- Antituberculosis drugs (rifabutin)
- Drugs for erectile dysfunction (sildenafil, tadalafil)
- Drugs for breathing problems as theophylline
- Drugs for insomnia, as zopiclone
- Migraine medicines (eletriptan)
- Drugs to prevent blood clots (acenocoumarol or warfarin)
- Analgesics and sedatives medications, as alfentanil midazolam or triazolam

Special physiological situations

Pregnancy and lactation:

If you are breastfeeding your baby, you should not take this medicine unless your doctor recommends doing so.

Driving and using machines

It is unlikely that erythromycin alters the ability to drive or operate machinery. However, on rare occasions, erythromycin may affect these abilities, so you should not drive or operate machinery until verifying that the medication does not affect you.

Follow carefully the instructions of use of erythromycin provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given on this website, which may be different. Adults

Your doctor will determine the dose depending on the severity and type of the infection The usual dose is 1 or 2 grams a day, divided into 3 or 4 shots. This dose may be increased up to 4 g or more per day, depending on the severity of the infection.

Children:

Your doctor will determine the dose depending on the child's weight, age and severity of the infection. Usually, the dose in children is 30-50 mg/kg per day, divided into 3 or 4 doses, but your doctor can prescribe a different dosage. For more serious infections, your doctor may set a higher dose.

Alternatively, the total daily dose may be divided only into two doses (every 12 hours).

The drug should be administered before or during the meal and in the case of tablets or capsules, swallowed with a glass of water.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

If a doctor prescribes another medicine, tell you are taking erythromycin.

If you or your child takes more erythromycin what has been prescribed or accidentally receive an overdose, immediately go to the nearest hospital carrying the medicine package with you

If you forget to take erythromycin

If you miss a dose, take the next one as soon as you remember. Do not take a double dose to make up the missed dose.

If you stop taking erythromycin

You should not stop treatment with this antibiotic, as it may worsen your condition. Take the medication as long as your doctor has told you.

Like all medicines, erythromycin can cause side effects although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients

Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

The most common side effects of oral erythromycin are located in the digestive system, manifested by abdominal pain, frequently dependent on the doses used.

Uncommon side effects:

Nausea, vomiting, diarrhea and anorexia (loss of appetite)a.

Rare side effects:

Rarely it has been observed the appearance of pseudomembranous colitis.

During repeated treatment, it is possible the growth of bacteria or fungi resistant to the antibiotic. If this occurs,your doctor will advise you to discontinue treatment and the appropriate therapy.

Have been described allergic reactions as urticaria, moderate skin rashes, and anaphylactic phenomena.

Have been observed cases of reversible loss of hearing, more often in patients with renal insufficiency and in those receiving high doses of erythromycin.

Very rarely have been described some transient adverse effects in patients receiving erythromycin therapy, although no link has been established between the effect and cause. This include:

Confusion, hallucinations, convulsions (seizures), vertigo, cardiac arrhythmias, ventricular tachycardia, chest pain and palpitations.
