Indications and usage     

This medication is used to treat:

In adults:

- Major depressive episodes
- Obsessive-compulsive disorder
- Bulimia nervosa: Fluoxetine is indicated as a complement of psychotherapy for the reduction of bingeing and purging.

In children over 8 years and adolescents:

- For depressive episodes moderate or severe, if no response to psychological therapy after 4-6 sessions. Fluoxetine should be offered to children or young people with moderate or severe depression only if combined with psychological therapy.

Other ingredients of available commercial presentations:

Capsules: Besides fluoxetine, contains gelatin, patent blue V (E 131), yellow iron oxide (E172), titanium dioxide (E 171). The components of the edible ink are shellac, propylene glycol, ammonium hydroxide, black iron oxide (E 172) (formulation 1) or shellac, soya lecithin, antifoam DC 1510, black iron oxide (E172).

Dispersible tablets: The other ingredients are microcrystalline cellulose, sodium saccharin, mannitol, sorbitol, anise flavor, peppermint flavor, colloidal anhydrous silica, pregelatinized corn starch, sodium stearyl fumarate, crospovidone.

Oral solution: The other ingredients are: benzoic acid, sucrose, glycerol, peppermint flavor (containing 0.23% alcohol) and purified water.

Important information     

Oral solution contains sucrose (sugar) that can harm teeth. If your doctor has told you that you have an intolerance to some sugars, contact him before taking it.

Before taking the drug     

Do not use fluoxetine

If you are allergic (hypersensitive) to fluoxetine or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

If you are taking any medicine belonging to another class of drugs that are also used to treat depression and are known as non-selective inhibitors of monoamine oxidase (also called reversible inhibitors of monoamine oxidase type A or MAO-A), because it can cause serious or even fatal reactions.

Treatment with fluoxetine should only be started after two weeks of finishing treatment with an irreversible MAOI (for instance tranylcypromine). However, you can start treatment with fluoxetine the following day after treatment with certain reversible MAOIs called reversible MAO inhibitors (MAO-A inhibitors) (eg moclobemide).

Do not take any MAOIs for at least five weeks after stopping fluoxetine treatment. If your doctor prescribed fluoxetine for an extended period of time and/or high dose, he or she may consider the need for a greater interval before taking a MAOI. Examples of MAOIs include nialamide, iproniazid, selegiline, moclobemide, phenelzine, tranylcypromine, isocarboxazid, and toloxatone.

Take special care with fluoxetine

- If you develop a rash or other allergic reactions (like itching, swollen face or lips or shortness of breath), stop taking fluoxetine and contact your doctor immediately.
- If you have epilepsy or have had seizures previously. If you have experienced seizures or increased frequency of these, contact your doctor immediately, you may need to stop treatment with fluoxetine.
- If you have suffered from mania in the past. If you have a manic episode, contact your doctor immediately as you may need to stop treatment with fluoxetine.
- If you have diabetes, your doctor may need to adjust the dose of insulin or another antidiabetic drug.
- If you have liver problems, since your doctor may need to adjust the dose of fluoxetine,
- If you have heart disease.
- If you are taking diuretics, especially if you are an elderly patient.
- If you are receiving electroconvulsive therapy (ECT).
- If you have a history of bleeding disorders or if you have unusual bruising or bleeding.
- If you are taking drugs that affect blood clotting.
- If you develop a fever, muscle stiffness or tremor, changes in your mental state like confusion, irritability or extreme agitation, you may have a condition called serotonin syndrome or neuroleptic malignant syndrome. Although this syndrome appears rarely, can put your life at risk, so should tell your doctor immediately

- If you have suicidal or self-harm thoughts: Depression is associated with an increased risk of suicidal thoughts, self-harm and suicide (suicide-related events).

This risk may persist until an improvement of the disease occurs. After starting treatment with fluoxetine, it may take 3-4 weeks until you experience an improvement, so your doctor will carefully monitor your evolution when you begin treatment. Other psychiatric conditions for which fluoxetine is prescribed can also be associated with an increased risk of suicide-related events. Therefore they should follow the same precautions when treating patients with other psychiatric disorders.

Use in children and adolescents aged 8 to 18 years:

Patients under 18 have an increased risk of adverse effects such as suicide attempts, suicidal thoughts and hostility (predominantly aggression, oppositional behavior and anger) when they take this class of drugs. Fluoxetine should only be used in children and adolescents aged 8-18 years for the treatment of depressive episodes of moderate to severe in combination with psychological therapy, and should not be used for other indications in this age group.

Additionally, there exists only limited information regarding the long-term safety of fluoxetine on growth, puberty, and cognitive, emotional and behavioral development in this age group. However, your doctor may prescribe fluoxetine for patients under 18 for the treatment of depressive episodes of moderate to severe in combination with psychological counseling if thinks this is the most convenient for the patient.

If your doctor prescribes fluoxetine for a patient under 18 and you want to know more about this question, consult your doctor. You should tell your doctor if any of the symptoms listed above appears or worsen when a patient under 18 is taking fluoxetine.

Fluoxetine should not be used in the treatment of children under 8 years old.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

Fluoxetine may affect the way other medicines work (interaction). Interactions can occur with:

- Monoamine oxidase inhibitors (MAOIs) (medicines used to treat depression). Fluoxetine should not be used with non-selective MAO inhibitors and inhibitors known as MAO inhibitors type A (moclobemide) since may occur serious or even fatal reactions. Fluoxetine can be used jointly with MAO inhibitors known as MAO-B inhibitors (selegiline) as long as your doctor closely monitor treatment.
- Lithium, tryptophan: there is an increased risk of serotonin syndrome when these drugs are given along with fluoxetine. When administered in combination with lithium your doctor will carry out medical exams more often.
- Phenytoin (a drug used for epilepsy), fluoxetine can increase blood levels of this drug, so your doctor may need to introduce more controls phenytoin level when administered with fluoxetine.
- Clozapine (used to treat some mental disorders), tramadol (for pain) or triptans (for the treatment of a migraine), there is an increased risk of hypertension.
- Flecainide or encainide (to treat heart problems), carbamazepine (for the treatment of epilepsy), tricyclic antidepressants (e.g. imipramine, desipramine and amitriptyline), because fluoxetine can change the blood levels of these drugs. Your doctor may need to lower their dose when administered with fluoxetine.
- Warfarin, acenocoumarol or other drugs that have effects on coagulation of blood; Fluoxetine may alter the effect of these drugs on the blood. Your doctor will need to make additional checks of INR if starts fluoxetine treatment while taking oral anticoagulants.
- Should not take medicinal plants containing St. John's Wort (also called Hypericum perforatum) while being treated with fluoxetine because it can lead to increased side effects. If when starting treatment with fluoxetine you are taking St. John's wort, tell your doctor.

Use with food and beverages

Fluoxetine can be taken with or without food, as desired. Is not advisable to drink alcohol while taking this medicine.

Special physiological situations

Pregnancy and lactation

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy

Available information to date does not indicate an increased risk when this drug is used during pregnancy. However, caution should be exercised when used during pregnancy, especially during late pregnancy or just before giving birth since have been reported the following effects in newborn infants: irritability, tremor, muscle weakness, persistent crying, difficulty in sucking or sleeping.

The use of this medicine is not recommended during pregnancy.

Lactation

Fluoxetine is excreted in breast milk and may cause side effects in babies. You should continue breastfeeding only if absolutely necessary. If breastfeeding is continued, your doctor may prescribe a lower dose of fluoxetine.

The use of fluoxetine is not recommended during breastfeeding. If necessary, your doctor will decide whether you should using this medicine.

Driving and using machines

This drug may impair judgment or coordination. Do not drive or use machinery without advice from your doctor or pharmacist.

How to use fluoxetine     

Follow carefully the instructions of use of fluoxetine provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given on this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Swallow the capsule with a little water. Do not chew.

Usual doses:

Depression:

The recommended starting dose is 20 mg. If necessary, your doctor may review and adjust your dosage, during the following 3-4 weeks after starting treatment.

When deemed necessary, the dose can be increased gradually to a maximum of 60 mg. It should increase the dose carefully to ensure that you receive the lowest effective dose.

You may not feel better immediately when you start taking your medicine for depression. This is common since the improvement in depressive symptoms takes a few weeks after starting treatment.

Patients with depression should be treated for a period of at least 6 months.

Bulimia nervosa:

The recommended dose is 60 mg per day.

Obsessive-compulsive disorder:

The recommended dose is 20 mg daily. Your doctor may review and adjust the dose after two weeks of treatment. When necessary, the dose can be gradually increased up to 60 mg.

If no improvement is observed within first 10 weeks, treatment with fluoxetine should be reconsidered.

Children and adolescents aged 8 to 18 years with depression:

Treatment should be initiated and supervised by a specialist. The initial dose is 10 mg/day (2.5 ml oral solution).

After one or two weeks the doctor may increase the dose up to 20 mg/day. The dose should be increased carefully to ensure that the patient is maintained at the lowest effective dose.

Underweight children may need lower doses.

Your doctor should assess the need for continued treatment after 6 months. If you have not improved, treatment should be reconsidered.

Elderly: your doctor will increase the dose more carefully. The daily dose should generally not exceed 40 mg. The maximum dose is 60 mg per day.

If you have a liver problem or are using other medication that might influence on fluoxetine's effect, your doctor may decide to prescribe a lower dose or may instruct you to use this medication every other day.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take fluoxetine     

If a doctor prescribes another medicine, tell you are taking fluoxetine.

Check with your doctor regularly developments of disorder that motivates the use of fluoxetine. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with fluoxetine at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with fluoxetine, immediately consult your doctor.

If you take more than you should:

If you take more fluoxetine than you should, tell your doctor or go to the nearest hospital casualty department straight away. Take the medicine pack with you, even if it is empty.

If you forget to take fluoxetine

Do not worry if you miss a dose. Take your next dose the next day at the usual time. Do not take a double dose to make up for missed doses.

Taking your medicine at the same time each day may help you remember to take it regularly.

If stop taking fluoxetine

Do not stop taking fluoxetine unless directed by your doctor. It is important to keep taking your medicine.

Do not stop taking your medication without first asking your doctor even if you begin to feel better. Be sure not to run out of medication.

You may notice the following effects when you stop taking fluoxetine:

Dizziness, tingling sensations like pins and needles, sleep disturbances (vivid dreams, nightmares, insomnia), restless or agitation, unusual tiredness or weakness, anxiety, nausea and/or vomiting, tremor (shakiness) and headache.

Most patients notice that symptoms that appear when stopping treatment with fluoxetine are mild and self-limiting and disappear in a few weeks. If you have these symptoms when stopping treatment, consult your doctor.

To discontinue treatment with fluoxetine, your doctor will provide guidance on how gradually reduce the dose in 1-2 weeks; this will help you to reduce the chance of suffering withdrawal effects.

Side Effects of fluoxetine     

Like all medicines, fluoxetine can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Your doctor has a complete list of these effects. Tell your doctor promptly about these symptoms or any other unusual.

If you have a rash or allergic reactions such as itching, swelling of face or lips (angioedema) or shortness of breath (asthma attack, bronchospasm), stop taking this medication and tell your doctor immediately.

If you feel restless and feel like you can not remain sitting or stand still, you may suffer from something called akathisia; if you feel so, see your doctor.

Consult your doctor immediately if your skin starts to turn red or presents any type of reaction or you have blistering or if your skin starts to peel (Stevens-Johnson syndrome, Lyell's disease) This happens very rarely.

Some patients have had:

- A combination of symptoms known as serotonin syndrome including unexplained fever with increased heart rate, sweating, muscle stiffness or tremor, confusion, extreme agitation or sleepiness (only rarely)
- Weakness, drowsiness or confusion mostly in elderly and in people treated with diuretics (elderly)
- Prolonged and painful erection (priapism),
- Irritability and extreme agitation

If you have any of the side effects described above should contact your doctor immediately.

If you have any of the symptoms described below and they bother you, or last for some time, tell your doctor:

General side effects:

- Chill,
- Sensitivity to light,
- Lack of appetite,
- Weightloss,
- Tiredness.

Digestive side effects:

- Diarrhea,
- Stomach upset, indigestion,
- Vomiting,
- Difficulty swallowing,
- Dysgeusia (altered taste),
- Dry mouth,
- Rarely, abnormal liver tests and very rare cases of hepatitis.

Neuropsychiatric side effects:

- Headache,
- Disturbed sleep or abnormal dreams,
- Dizziness,
- Euphoria,
- Uncontrollable movements,
- Seizures,
- Extreme restlessness, confusion, agitation, anxiety, nervousness, inability to concentrate and think clearly,
- Hallucinations,
- Behavior disorders (uncontrolled, uninhibited)
- Panic attacks,
- Suicidal or self-harm thoughts.

Genital and urinary side effects:

- Difficulty urinating, increased frequency of urination,
- Sexual dysfunction,
- Prolonged and painful erection (priapism),
- Galactorrhea (breast milk production).

Respiratory side effects:

- A sore throat,
- Difficulty breathing.
- Have been reported rarely lung disorders, including inflammatory processes and other processes such as fibrosis.

Other side effects:

- Hair loss,
- Yawns,
- Blurred vision,
- Bruising or bleeding without grounds,
- Heavy sweating,
- Hot flushes,
- Feeling dizzy when you stand up,
- Muscle or joint pain,
- Low blood sodium levels.

Most of these side effects tend to disappear with continued treatment.

In addition, in children and adolescents (8-18 years), fluoxetine may slow growth or possibly may delay sexual maturity.
