Indications and usage     

Ibandronic acid belongs to a group of medicines called bisphosphonates. It contains no hormones.

This substance inhibits increased loss of calcium from bone (bone resorption) and returning to normal the high calcium blood levels. It also prevents bone complications and fractures related to the spread of cancer cells in bone.

This medicine is used to treat osteoporosis in patients who are at high risk for fractures. It has been shown to reduce the risk of vertebral fractures but not the risk of hip fractures. It is also indicated for treat pathologically high serum calcium levels (hypercalcemia) as a result of tumors.

It is also indicated for the prevention of skeletal events (pathological fractures, bone complications requiring radiotherapy or surgery) in patients with breast cancer and bone metastases.

Ibandronic acid prevents loss of bone from osteoporosis and helps to rebuild bone. Therefore, the drug reduces the chances of fracture. Healthy habits also facilitate the favorable effects of treatment. These include a balanced diet rich in calcium and vitamin D; walking or any other weight-bearing exercise; not smoking and not drinking too much alcohol.

Other ingredients of available commercial presentations:

Tablets:

Each tablet contains 50 or 150 mg of ibandronic acid. The other ingredients are:

Tablet core: lactose monohydrate, povidone, microcrystalline cellulose, crospovidone, purified stearic acid and colloidal anhydrous silica.

Tablet coat:

hypromellose, titanium dioxide, talc and macrogol 6000.

Injectable solution: Each vial contains 1 mg ibandronic acid (1.12 mg monohydrate monosodium salt) per ml solution. The other ingredients are sodium chloride, acetic acid, sodium acetate, and water for injections.

Warning:

Important information     

The tablets contain lactose. If your doctor has told you that you have an intolerance to sugars, contact him before taking it.

Before taking the drug     

Do not use ibandronic acid

If you are allergic (hypersensitive) to ibandronic acid or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you have low levels of calcium in the blood or think you may have them.
- If you are pregnant, you think you might be pregnant, or you are breastfeeding your child.

Ibandronic acid should not be given to children or teenagers.

Take special care with ibandronic acid

Some people need special care during treatment with ibandronic acid. Consult your doctor:

- If you have any disturbance of mineral metabolism (such as vitamin D deficiency).
- If you can not stand or sit for an hour straight.
- If you have any kidney problems.

- If you have ever experienced problems with your esophagus (the tube that connects the mouth to the stomach). Esophageal problems can cause severe symptoms such as chest pain, severe pain after swallowing food or drinking, severe nausea or vomiting. If you have any of these symptoms, tell your doctor right away.

If you are being treated by the dentist or you will undergo dental surgery, tell your dentist that you are being treated with ibandronic acid.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

If in doubt, consult your doctor.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription or any supplement containing calcium, magnesium, iron, or aluminum.

Aspirin and other nonsteroidal anti-inflammatory drugs (NSAIDs) They can irritate the stomach and intestine; as bisphosphonates (including ibandronic acid). Be therefore careful if you take painkillers or anti-inflammatories while you are taking this drug. (among NSAIDs are ibuprofen, diclofenac, and naproxen).

After swallowing the daily (or monthly) ibandronic acid tablet, wait for 1 hour before taking any other medicines, including medicines tablets for indigestion, calcium supplements, and vitamins.

Use with food and beverages

Do not take Ibandronic acid with solid or liquid foods:

Ibandronic acid loses effectiveness if taken with food.

Special physiological situations

Pregnancy and lactation:

Always consult your doctor or pharmacist before taking any medicine during pregnancy or breastfeeding.

Do not take Ibandronic acid if you are pregnant or breastfeeding. If you are breastfeeding may need to stop to breastfeed your child if you take Ibandronic acid.

Driving and using machines

It is very unlikely that ibandronic acid affects your ability to drive and use machines

Consult your doctor or pharmacist before taking any medicine.

How to use ibandronic acid     

Follow carefully the instructions of use of ibandronic acid provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given on this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

150 mg tablets:

The usual dose is one tablet per month.

Choose the day of the month that will be easier to remember. You can choose the same date (either the first day of each month) or the same day (as the first Sunday of each month). Choose what best fits your routine.

50 mg tablets:

The usual dose of ibandronic acid is one tablet a day.

For the two dosage forms:

It is important that the tablet reaches the stomach quickly and cause less irritation.

Take the tablet after a minimum of 6 hours after the last meal or drink (except water) you can only drink water during this six-hour period.

Take the tablet upon waking, before eating breakfast or drink liquids (on an empty stomach).

Swallow the tablet with a glass of tap water (at least 200 ml). Do not take your tablet with mineral water, fruit juice or other beverages.

Swallow the tablet whole. do not chew, crush or dissolve in the mouth.

For the next hour (60 minutes) after taking the tablet:

Do not lie down; if you do not stay upright (standing or sitting), some of the medicine could leak back into the esophagus.

Do not eat or drink anything (except tap water, if you need it). Do not take any other medicine.

After waiting 1 hour, you can have breakfast and the first drink of the day. Once you've eaten, you can, if you wish, lie down and take other medication you need.

Do not take this medicine at bedtime or before getting up in the morning,

It is important that you take Ibandronic acid every month (or every day) until directed by your doctor. This medicine is useful to treat osteoporosis only while you are taking it.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take ibandronic acid     

If a doctor prescribes another medicine, tell you are taking ibandronic acid.

Check with your doctor regularly developments of disorder that motivates the use of ibandronic acid. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with ibandronic acid at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with ibandronic acid, immediately consult your doctor.

If you take more than you should:

If you have taken by mistake too many tablets, drink a full glass of milk and talk to your doctor immediately. Do not induce vomiting nor lie down because it could injure the esophagus.

If you forget to take ibandronic acid

If you miss one or more doses of ibandronic acid, do not take more tablets to make up nor take another tablet later. Take the dose the next day when you wake up.

Side Effects of ibandronic acid     

Like all medicines, ibandronic acid can cause side effects although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Common side effects:

Heartburn (burning that rises from the stomach into the mouth), indigestion and diarrhea. Other possible side effects are rash and pain in muscles and joints.

These effects usually disappear after some time taking the drug. It is very likely that you can tolerate it. However, tell your doctor if these effects worsen or last long time.

Ibandronic acid can irritate the esophagus although you can avoid this by taking your dose according to the instructions. If you suffer symptoms such as severe pain in the chest, severe pain after swallowing food or drink, severe nausea or vomiting, tell immediately to your doctor.
