Indications and usage     

Indomethacin is a drug belonging to the group of anti-inflammatory drugs (NSAIDs), which reduce pain and inflammation.

This medication is used to:

- Inflammatory diseases of the joints and surrounding tissues such as arthritis, osteoarthritis, degenerative joint disease, bursitis, tendinitis, synovitis, tenosynovitis, and capsulitis.
- Deformity and rigidity of the spine (ankylosing spondylitis)
- Muscle pain, back pain or pain after joint surgery
- Menstrual pains (dysmenorrhea)
- Gout

Medizzine recommends consulting your doctor if you are unsure (or you do not know) why you have been given indomethacin.

Other ingredients of available commercial presentations:

Injectable solution Each vial contains indomethacin sodium, equivalent to 1 mg of indomethacin. It contains no excipients or diluents.

Capsules: Besides indomethacin, contains starch, powdered cellulose, colloidal silicon dioxide and magnesium stearate. Capsules contain yellow iron oxide (E172), titanium dioxide (E171) and gelatin. The printing ink contains shellac and black iron oxide (E172).

Modified release capsules (retard): Besides the active principle, the retard capsules may contain sucrose, corn starch, lactose, povidone, talc, magnesium stearate, polymers of methacrylic acid, esters of acrylic and methacrylic acid. The cover contains titanium dioxide (E171), erythrosine (E127), indigo carmine (E132), yellow iron oxide (E172) and gelatin. The printing ink contains shellac glaze, titanium dioxide (E171) and iron oxide black (E172).

Suppositories: Each suppository contains 100 mg of indomethacin and vegetable fat as an excipient.

Important information     

Retard capsules may contain lactose and sucrose. If your doctor has told you that you suffer an intolerance to certain sugars, contact him before taking it. Diabetic patients should note sucrose content.

Before taking the drug     

Do not use indomethacin

If you are allergic (hypersensitive) to the active substance indomethacin, to any anti-inflammatory drug, to acetylsalicylic acid or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor take this medicine if:

- You have suffered asthma attacks, allergic skin reactions, sudden swelling, especially in the throat, rashes all over the body, or watery rhinorrhea while taking aspirin or other NSAIDs or COX-2 (celecoxib, for example)
- You have had previously or currently suffers from a stomach ulcer or bleeding from the digestive tract
- you have an important disease of kidney, liver or heart
- You have nasal polyps
- You are in the last trimester of pregnancy
- The patient is under twelve.

Take special care with indomethacin

Before taking indomethacin, check with your doctor or pharmacist if you have:

- Coagulation disorders or bleeding problems
- You suffer a psychiatric disorder, epilepsy, Parkinson's disease or related disorders.
- You have kidney problems
- Currently have an infection, treated or not, or you are getting a vaccine containing live microorganisms
- You suffer any liver or heart disease leading to fluid retention and high blood pressure
- you have asthma
- You have a bowel disease such as ulcerative colitis or Crohn's disease
- you have disseminated lupus erythematosus or other connective tissue diseases
- You will undergo surgery

Medications such as indomethacin are associated with a small increased risk of heart attack (myocardial infarction) or stroke (stroke). This risk is more likely if the treatment is prolonged or are used high doses. Do not exceed the recommended duration of treatment and the dose recommended by your doctor.

If you have heart problems, have suffered previously a cerebrovascular event (stroke) or think you may be at risk for these conditions (for example if you have high blood pressure, diabetes, you smoke or have high cholesterol level), you should talk to your doctor and express your concerns.

This medicine can alter the results of some laboratory tests on blood and urine. If you are visited by a doctor who is not your usual, you must inform that you are receiving indomethacin.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

Check with your doctor or pharmacist if you are taking any of the medication listed below:

- Aspirin or similar medication (salicylates)
- other NSAIDs, including diflunisal
- Quinolones, a group of drugs used to treat infections (norfloxacin, moxifloxacin, ciprofloxacin and other)
- Antidepressants reuptake inhibitors of serotonin (SSRIs) and/or noradrenalin (SNRI) (paroxetine and other)
- Sulfonylureas, used for the treatment of diabetes
- Medicines to treat high blood pressure (such as atenolol, captopril, doxazosin, hydralazine, losartan, nifedipine, propranolol or quinapril)
- Antiplatelet drugs or medicines to prevent blood clotting
- Lithium or haloperidol (antipsychotic drugs)
- antiretrovirals, such as zidovudine or ritonavir.
- cardiac glycosides, such as digoxin
- cyclosporine or tacrolimus
- Corticosteroids (anti-inflammatory)
- Methotrexate (used to treat some types of cancer, psoriasis or severe rheumatoid arthritis
- Desmopressin (antidiuretic hormone used to prevent nocturnal enuresis),
- Thiazide diuretics, furosemide, and triamterene
- Mifepristone (used to induce abortion)
- Baclofen (a muscle relaxant)
- Pentoxifylline, which is used for certain vascular problems,
- Probenecid (a drug for gout)
- Tiludronic acid used to treat bone problems.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Use with food and beverages

Indomethacin capsules should be swallowed whole, preferably with meals or milk.

Special physiological situations

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy and lactation

Indomethacin may cause you to have more difficulty becoming pregnant. Tell your doctor if you are planning to become pregnant or are having trouble getting pregnant.

If you are pregnant or breastfeeding your child, talk to your doctor before taking this medicine.

Children

It is not recommended for use in children.

Use in elderly

Driving and using machines

This medicine may make you feel drowsy, tired, dizzy, or may affect the vision. You must ensure that indomethacin does not affect you before driving or operating machinery.

How to use indomethacin     

Follow carefully the instructions of use of indomethacin provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given on this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Your doctor has recommended the dosage that best suits your situation. Always follow your doctor's choice over any other indication you can receive or read.

The usual doses range from 50 to 200 mg daily

Acute joint disease: 25 mg two or three times a day as initial treatment.

Chronic or subacute joint disease: 25 mg two or three times a day, gradually increasing in steps of 25 mg until reaching 150 mg daily.

In the case of an outbreak of acute inflammation of chronic articular process: can be increased the dose until 150-200 mg/day.

Severe muscle pain: 50 mg two or three times a day as initial therapy. Rarely, are required dose up to 200 mg. It can be kept no longer than 10 to 14 days.

Lumbago: 50 mg two or three times a day, depending on the importance of pain, keeping it about 5-10 days.

Gout: 50 mg three or four times daily until remission of symptoms.

After orthopedic surgical procedures: 100-150 per day in two or three doses.

Elderly: Your doctor may prescribe a different dose than above, according to the previous situation of each patient and make a closer follow.

Children: this medicine is not recommended for children.

If you think that the effect of the medicine is too strong or too weak, consult your doctor.

While you take indomethacin     

If you become pregnant, contact your doctor immediately.

If a doctor prescribes another medicine, tell you are taking indomethacin.

Check with your doctor regularly developments of disorder that motivates the use of indomethacin. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with indomethacin at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with indomethacin, immediately consult your doctor.

If you use more medicine than you should

If you take more medicine than you should, go to the nearest hospital. Carry the pack with the remaining medicine, even if empty, or contact your doctor immediately. You can also consult the Toxicology Information Service of your country.

If you forget a dose:

Do not take a double dose to make up for missed doses.

Side Effects of indomethacin     

Like all medicines, indomethacin can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

These side effects occur most frequently at the beginning of treatment.

The most common side effects are headaches, dizziness and indigestion. If you suffer from prolonged headache, consult your doctor.

If you develop a yellowing of the skin or the conjunctiva (the white of the eyes), immediately discontinue treatment with indomethacin and tell your doctor immediately.

Some patients may suffer aseptic meningitis, that causes headaches, fever, neck stiffness, significant tiredness, feeling very ill, sensitivity to light. This situation requires immediate medical attention.

Medications such as indomethacin may be associated with a small increased risk of heart attack (myocardial infarction) or stroke (stroke).

If you experience any of the following problems during treatment with indomethacin, you should discontinue treatment and seek medical advice immediately:

To present blood in the stool, vomiting blood or expel small blackish grains as coffee beans in the content of vomiting.

Stop taking indomethacin and tell your doctor immediately if you experience:

Indigestion or heartburn, abdominal pain or other abdominal symptoms.

Other possible side effects:

Blood side effects: Changes in the number and type of blood cells. If you develop nasal bleeding, sore throat, soreness or infections talk to your doctor. You may need a blood test.

Allergic reactions: Allergic skin rashes, swelling of the throat, difficulty breathing, asthma, and flooding of lungs.

Nervous system side effects: fatigability, drowsiness, vertigo, headache, speech problems, nervousness, fading, coma, marked fatigue, hallucinations, difficulty sleeping, depression, confusion, anxiety, mental disorders, involuntary movements, loss of coordination, lightheadedness and tingling, seizures or worsening of epilepsy and Parkinson's syndrome.

Eye side effects: Pain in and around eyes, blurred vision or double vision, inflammation of the optic nerve, deposits on the surface of the eye or effects on the retina.

Ear side effects: Ringing in the ears or other disorders of the ear (rarely, deafness).

Heart side effects: High or low blood pressure, swelling of lower limbs, chest pain, heart rhythm disturbances, and heart failure.

Gastric side effects: Feeling unwell, abdominal pain or discomfort, loss of appetite, constipation or diarrhea, flatulence, inflammation of the mouth or intestines or worsening of these conditions if you previously suffer any of these, ulceration of the intestine with possible bleeding, obstruction or perforation, worsening of Crohn's disease or ulcerative colitis.

Liver side effects: inflammation of the liver (hepatitis), jaundice.

Cutaneous side effects: Sensitivity to light, worsening of psoriasis.

Kidney side effects: inflammation, disease or kidney failure, blood, and protein in the urine, increased blood urea.

Other side effects: Increase in blood glucose, urinary glucose, increased blood potassium, vaginal bleeding, alterations in the breast (increased, hypersensitivity), breast development in males, hot flushes, profuse sweating, mouth ulcers, muscle weakness and worsening cartilage injuries.
