Indications and usage     

Loperamide is an antidiarrheal which reduces bowel movements and secretions, causing a decrease of liquid stools.

This active ingredient is used for symptomatic treatment of occasional diarrhea.

Medizzine recommends consult your doctor if you are unsure why has prescribed loperamide.

Other ingredients of available commercial presentations:

Capsules/tablets:

Each capsule contains 2 mg of the active ingredient. The other ingredients (excipients) may include pregelatinized starch corn gluten-free, lactose monohydrate, colloidal anhydrous silica, magnesium stearate,gelatin, orange yellow S (E110), and titanium dioxide (E171). Tablets can contain mannitol.

Oral solution:

Each ml contains 0.2 mg of active ingredient. Excipients include sodium saccharin, raspberry flavor, currant aroma, red Ponceau 4R (E-124), methylparaben, propylparaben, citric acid, glycerol (E422) and purified water.

Oral lyophilisate:

This medicine contains 2 mg loperamide hydrochloride. The other ingredients (excipients) are gelatin, mannitol (E421), aspartame (E951), mint flavor and sodium hydrogen carbonate.

Important information     

Capsules:

This medicine contains lactose. If your doctor has told you that you have an intolerance to some sugars, contact him before taking it.

This form also contains orange-yellow dye (E110), which may cause allergic reactions. It can cause asthma, particularly in patients allergic to aspirin.

Oral solution:

This medicine contains red dye Ponceau (E124) as an excipient. It can cause allergic-type reactions including asthma, particularly in patients allergic to aspirin. This medication contains glycerol (E422) as excipient which may be harmful at high doses. It can cause headache, stomach upset and diarrhea.

This medicine contains methyl and propyl paraben, which can cause allergic reactions, possibly delayed.

In some countries, the loperamide oral solution contains ethanol.

Oral lyophilisate:

This medicine may be harmful to people with phenylketonuria since it contains aspartame, a source of phenylalanine.

Before taking the drug     

Do not use loperamide

If you are allergic (hypersensitive) to loperamide or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

The diarrhea treatment with this medicine is symptomatic only and should not be used if:

- You have blood in stools and high fever (above 38 ° C).
- You have acute ulcerative colitis or pseudomembranous colitis associatedwith broad-spectrum antibiotic treatment.
- If appears sudden constipation and/or abdominal distention.

Take special care with loperamide

If there is no clinical improvement within 48 hours or if occur fever, constipation and other symptoms, stop the treatment and consult your doctor.

Patients with diarrhea should drink plenty of fluids to prevent dehydration as evidenced by dry mouth, excessive thirst, decreased urine output, wrinkled skin, dizziness, lightheadedness. This is especially important in children and the elderly.

Since treatment of diarrhea with loperamide is symptomatic, the ailment should be treated from its cause when such treatment is available.

Drugs that prolong intestinal transit time can induce the development of toxic megacolon (a complication of other intestinal diseases, characterized by dilated colon, bloating and sometimes fever, increased heart rate, abdominal pain, and dehydration).

Patients with liver problems should consult their doctor before taking this medicine.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

If you are taking an antibiotic or a narcotic-type analgesic, consult your doctor or pharmacist before taking this medicine since loperamide can interact with them.

Concurrent use of loperamide and ritonavir or quinidine increases two to three times the plasma levels of loperamide, although the clinical significance of this interaction is unknown.

Simultaneous use with other opioid analgesics can increase the risk of severe constipation and central nervous system depression.

Loperamide can worsen or prolong diarrhea caused by broad-spectrum antibiotics.

Pregnancy and lactation

It is not established the safety of this drug for use in pregnant women.

The medicine use during pregnancy can be dangerous for the embryo or fetus and should be monitored by your doctor.

The use of loperamide is not recommended for women who are breastfeeding.

No dose adjustment is required.

Driving and using machines

Although are not expected undesirable effects in this sense, if occurs fatigue, dizziness or drowsiness, is recommended avoid driving vehicles or operate machinery.

How to use loperamide     

Follow carefully the instructions of use of loperamide provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given on this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Do not exceed the recommended dose for each age.

Adults and children over 12 years:

The initial dose is 4 mg loperamide (two capsules, two tablets, two lyophilized or 4 teaspoons 5 ml). Subsequently, you should take 2 mg (1 capsule, one tablet, one lyophilized or 2 teaspoons 5 ml) after every loose stool.

Do not take more than 16 mg loperamide a day. Nor take this medicine for more than 2 days.

Doses for children:

Children 8 to 12 years: Two 5 ml tablespoons 4 times a day.

Children under 8 years (4-8 years): One 5 ml teaspoon three or four times a day.

In some countries, this drug is not recommended in children aged under 4 years; in others, the lower limit is two years old.

Children 2 to 5 years:

0.4 ml/kg/day divided into three doses. You can increase, if necessary, up to 1.2 ml/kg/day.

Elderly patients:

No dose adjustment is required.

Kidney failure

No dose adjustment is required.

Liver failure

Although there is no pharmacokinetic available data for this disorder, this medication should be used with caution.

While you take loperamide     

If a doctor prescribes another medicine, tell you are taking loperamide.

If you feel unwell during treatment with loperamide, immediately consult your doctor.

If you take more than you should:

If you take more loperamide than you should, tell your doctor or go to the nearest hospital casualty department straight away. Take the medicine pack with you, even if it is empty.

In the case of overdose or accidental ingestion, may occur stupor, abnormal coordination, drowsiness, muscular hypertonia, respiratory depression, dry mouth, abdominal discomfort, nausea, vomiting, and constipation.

If you forget to take loperamide

Do not take a double dose to make up for missed doses.

Side Effects of loperamide     

Like all medicines, loperamide can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

- Uncommon side effects: Hypersensitivity reactions (rashes), urinary retention.
- Very rare side effects: Toxic megacolon (constipation, bloating, nausea and vomiting), paralytic ileus (persistent constipation), particularly, when recommendations for using loperamide have not appropriately complied.

It may also occur symptoms difficult to distinguish from those from diarrhea, such as

Abdominal pain and/or bloating, nausea and vomiting, tiredness, drowsiness or dizziness and dry mouth.
