Indications and usage     

Meloxicam belongs to the group of medicines called nonsteroidal anti-inflammatory drugs (NSAIDs).

This drug is indicated for adults and children over 15 years suffering:

- Symptomatic short-term treatment of acute attacks of osteoarthritis (a chronic degenerative joint disease).
- Symptomatic long-term treatment of rheumatoid arthritis (inflammation of the joints, usually including hands and feet, resulting in swelling and pain) or ankylosing spondylitis (inflammation affecting the joints of the spine).

Medizzine recommends consulting your doctor if you are unsure (or you do not know) why you have been given meloxicam.

Other ingredients of available commercial presentations:

Tablets: Each tablet contains either 7.5 or 15 mg meloxicam. The other ingredients (excipients) are sodium citrate, microcrystalline cellulose, lactose monohydrate, povidone, colloidal anhydrous silica, magnesium stearate, sodium carboxymethyl potato starch (gluten free).

Capsules: Each capsule contains either 7.5 mg or 15 mg of the active ingredient meloxicam. In addition, each capsule also contains lactose monohydrate, maize starch, sodium citrate, and magnesium stearate.

Suppository: Each suppository contains 15 mg of meloxicam. The other ingredients are solid semisynthetic glycerides (Suppocire BP) and macrogol glycerol hydroxy stearate 40 (Cremophor).

Oral suspension: This dosage form contains 7.5 mg/5 mL meloxicam. The other ingredients are silicon dioxide, hydroxyethyl cellulose, sorbitol, xylitol, sodium phosphate monobasic hydrate, saccharin sodium, sodium benzoate, citric acid monohydrate, and water.

Injectable IM solution: Each ampoule contains 15 mg of meloxicam (10 mg/ml). The other ingredients are meglumine, glycofurol, poloxamer 188, sodium chloride, glycine, sodium hydroxide and water for injections.

Warning:

Important information     

The tablets and the capsules contain lactose. If your doctor has told you that you have an intolerance to sugars, contact him before taking it.

The oral suspension contains sorbitol. If your doctor has told you that you have an intolerance to sugars, contact him before taking it.

Before taking the drug     

Do not use meloxicam

If you are allergic (hypersensitive) to meloxicam, to any other nonsteroidal anti-inflammatory drug or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you are in the third trimester of pregnancy.
- If you have had an ulcer or a bleeding from the stomach or the duodenum or you have suffered a perforation of the digestive system.
- If you have a severe liver disease.
- If you have a severe kidney disease not on dialysis.
- If you have a bleeding of any kind.
- If you have a severe impairment of the heart.

Take special care with meloxicam

- If you have had or are suffering an ulcer, bleeding or perforation in the stomach or duodenum (which can be manifested by an intense or persistent abdominal pain or even black stools without previous warning symptoms).
- This risk is higher when are used high doses and prolonged treatment, in patients with a history of peptic ulcer and elderly. In these cases, your doctor will consider the possibility of associating a protective medicine stomach.
- If you suffer skin disorders or mucous membranes, you should immediately discontinue treatment.
- If changes occur in kidney function tests or liver (determined in a blood or urine test), treatment should be discontinued.
- If you suffer liver cirrhosis (tissue destruction of normal liver, with the formation of scar tissue nonfunctioning), either a kidney disease or you are being treated with diuretics, it is necessary to monitor kidney function during treatment (urine volume, blood tests, urinalysis).
- If you have Crohn's disease or ulcerative colitis, since the meloxicam-type drugs can worsen these conditions.
- Medicines such as meloxicam can be associated with a small increased risk of heart attack ("myocardial infarction") or stroke. This risk is more likely to occur when high doses are used and during prolonged treatments. Do not exceed the dose or duration of treatment recommended by your doctor.
- If you have heart problems, history of stroke, or think you might be at risk to suffer these pathologies (eg, you have high blood pressure, diabetes, increased cholesterol, or are a smoker), you should consult this treatment with your doctor or pharmacist.
- Also, these drugs can cause fluid retention, especially in patients with heart failure and/or high blood pressure.
- Elderly patients, debilitated or sensitive people may be more susceptible to the adverse effects of the drug, therefore, should be carefully monitored during treatment.
- If you have an infection, this medicine may mask symptoms.
- Women who have difficulties conceiving or undergoing infertility testing should consult their doctor before using this medicine.
- In the case of insufficient effect, you should not increase the dose or start using another anti-inflammatory drug at a time.

Consult your doctor, if any of the above circumstances had ever happened.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

Tell your doctor if you take simultaneously medications that alter blood clotting such as corticoids, oral anticoagulants or antiplatelets derived from acetylsalicylic acid.

You should also tell about the use of other drugs that may increase the risk of bleeding such as corticosteroids and antidepressants selective inhibitors of serotonin reuptake.

Tell your doctor if you are taking any of the following medications:

- Diuretics (medicines that help the kidneys remove salt and water)
- Drugs to lower blood pressure
- Cyclosporine
- Lithium
- Methotrexate
- Cholestyramine

Other interactions to consider:

- IUDs (intrauterine devices)

Use with food and beverages

The tablets should be taken with a glass of water during a meal

Special physiological situations

Pregnancy and lactation:

Always consult your doctor or pharmacist before taking any medicine during pregnancy or breastfeeding.

Because the administration of drugs such as meloxicam has been associated with an increased risk of congenital anomalies or abortions, its administration is not recommended during the first and second trimester of pregnancy unless it is considered absolutely necessary. In these cases, the dose and duration will be limited to a minimum.

In the third quarter the administration of meloxicam is contraindicated.

For patients of childbearing age should be taken into account that the meloxicam-type drugs have been associated with a decreased ability to conceive.

NSAIDs pass into breast milk. Therefore, meloxicam should not be given to women during lactation.

Children

It is not established the efficacy and safety of this medicine for children under 15 years, so it is not recommended for use in this age group.

Driving and using machines

Do not drive nor operate machinery if appear visual disturbances or drowsiness, vertigo or other central nervous system disorders.

How to use meloxicam     

Follow carefully the instructions of use of meloxicam provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given on this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Adults and children over 15 years:

Administer 7.5 mg to 15 mg of meloxicam a day, according to medical prescription. Do not exceed the dose of 15 mg a day.

Elderly patients with rheumatoid arthritis and/or ankylosing spondylitis:

The recommended dose for long-term treatment is 7.5 mg daily.

Patients with a severe renal disease on dialysis:

It should not be given more than 7.5 mg daily.

Patients with mild or moderate renal disease:

It is not necessary to reduce the dose.

Patients with mild or moderate liver disease:

It is not necessary to reduce the dose.

Children

The safety and efficacy of meloxicam have not been established in children under 15 years.

Patients with an increased risk of adverse reactions:

Should start treatment with 7.5 mg daily.

This drug, when administered orally, should be taken with a glass of water during a meal, in a single daily dose.

Suppositories

The usual dose for the treatment of rheumatoid arthritis and ankylosing spondylitis is 15 mg (1 suppository) per day.

Injectable solution:

This medicine is only for intramuscular use. Injection should only be administered by experienced medical staff. Treatment should be limited to an injection once a day at the start of treatment, with a maximum duration of 2 to 3 days in exceptional cases (eg. when the oral or rectal route is not possible). Do not exceed the dose of 15 mg/day.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take meloxicam     

If a doctor prescribes another medicine, tell you are taking meloxicam.

Check with your doctor regularly developments of disorder that motivate the use of meloxicam. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with meloxicam at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with meloxicam, immediately consult your doctor.

If you take more than you should:

If you have taken more meloxicam than you should, immediately consult your doctor or pharmacist or the Toxicological Information Center in your country. However, if the amount ingested is important, see a doctor or go to the emergency room of the nearest hospital. Take the pack and the rest of unconsumed medicine.

Symptoms due to an overdose usually include drowsiness, nausea, vomiting and pain in the upper stomach, which disappear if supportive treatment is applied. Gastrointestinal bleeding may also occur.

In cases of severe overdosage, increased blood pressure, impaired kidney or liver, decreased breathing, coma, convulsions, cardiovascular collapse and cardiac arrest may occur. Furthermore, similar to allergic reactions may occur.

If you forget to take meloxicam

Do not take a double dose to make up for missed doses.

Side Effects of meloxicam     

Like all medicines, meloxicam can cause side effects although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Disorders of the blood and lymphatic system

Common side effects:

Anemia (decrease of red blood cells).

Uncommon side effects:

Disorders in the blood cells count leukopenia (reduction in the number of leukocytes) thrombocytopenia (reduced platelet count) agranulocytosis (no granulocytes).

immune system disorders:

Rare side effects:

Anaphylaxis/anaphylactoid reactions (allergic or similar).

Psychiatric disorders:

Rare side effects:

Mood disorders, insomnia, and nightmares.

Nervous system disorders:

Common side effects:

Feeling dazed headaches.

Uncommon side effects:

Vertigo, tinnitus (perception of abnormal noises), drowsiness.

Rare side effects:

Confusion.

Ocular disorders:

Visual disturbances including blurred vision.

Cardiovascular disorders:

Palpitations increased blood pressure hot flashes.

Medications such as meloxicam may be associated with a moderate increase in the risk of heart attack (myocardial infarction) or stroke.

Respiratory, thoracic and mediastinal disorders:

Rare side effects:

asthma attacks in some patients allergic to aspirin and other NSAIDs (nonsteroidal anti-inflammatory drugs).

Gastrointestinal disorders:

The most common side effects that occur with drugs such as meloxicam are gastrointestinal peptic ulcers, gastrointestinal bleeding perforations (in some cases fatal), especially in the elderly. They have also been observed nausea, vomiting, diarrhea, flatulence, constipation, heartburn, abdominal pain, blood in the stool, mouth ulcers worsening of ulcerative colitis and Crohn disease. Less frequently, have been observed the occurrence of gastritis.

Hepatobiliary disorders:

Medications such as meloxicam may be associated with liver disorders which can cause yellowing of skin and eyes (jaundice), sometimes with a high fever or swelling and tenderness of the upper abdomen.

If you develop any of the following effects: yellowing eyes or skin discoloration, discontinue treatment and IMMEDIATELY tell your doctor.

Uncommon side effects:

transient alterations of liver function tests (eg elevated transaminases or bilirubin).

Rare side effects:

Hepatitis.

Disorders of the skin and subcutaneous tissue:

Common side effects:

Pruritus (itching) rash (skin spots).

Uncommon side effects:

Urticaria (skin rash).

Rare side effects:

Angioedema (inflammation, usually of allergic origin, affecting the subcutaneous tissue, mainly in eyes, lips, throat, hands and feet), bullous reactions such as erythema multiforme photosensitivity reactions (increased sensitivity of the skin to sunlight).

Very rare side effects:

Medications such as meloxicam may be associated very rarely to bullous reactions very serious such as Stevens-Johnson Syndrome and toxic epidermal necrolysis.

Renal and urinary disorders:

Uncommon side effects

Alteration of tests to examine renal function (eg Increased creatinine or urea).

Rare side effects:

Kidney failure.

General disorders:

Common side effects

Edema including edema of lower limbs.
