Indications and usage     

Metoclopramide is in a group of drugs that act on nausea and vomiting. The drug is indicated for the treatment of symptoms in case of nausea and vomiting, in functional disorders of the digestive motility and the prevention and treatment of nausea and vomiting caused by radiation therapy, cobalt therapy, and cancer chemotherapy. It is also used in the preparation of radiographic examinations of the digestive tract.

Other ingredients of available commercial presentations:

Oral drops: Excipients include saccharin sodium, methylparaben, propylparaben, purified water. 100 ml of solution contains 260 mg metoclopramide hydrochloride.

Oral solution: Excipients include hydroxyethylcellulose, sodium cyclamate, sodium saccharin, methylparaben, propylparaben, citric acid monohydrate, oil of sweet orange, apricot aroma and purified water. 100 ml of the solution contains 100 mg metoclopramide hydrochloride.

Tablets: The excipients include lactose monohydrate, microcrystalline cellulose, corn starch, colloidal anhydrous silica and magnesium stearate. Each tablet contains 10 mg metoclopramide hydrochloride.

Injectable solution: The ingredients include sodium chloride and water for injections. Each ampoule (2 ml) contains 10 mg metoclopramide hydrochloride.

Important information     

Oral solution and drops:

Because these dosage forms contain methylparaben, propylparaben, may cause allergic reactions (possibly delayed).

In some countries, the oral solution contains sorbitol. If your doctor has told you to suffer an intolerance to sugars, contact your doctor before taking this dosage form of metoclopramide.

Tablets:

This medicine contains lactose. If your doctor has told you that you suffer an intolerance to sugars, contact him before taking it.

Before taking the drug     

Do not use metoclopramide

If you are allergic (hypersensitive) to metoclopramide or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you have a gastrointestinal hemorrhage, mechanical obstruction or gastrointestinal perforation, since stimulation of gastrointestinal motility constitutes a risk.
- If you have suffered in the past a previous history of incoordination of voluntary movements caused by drugs that act on the central nervous system or metoclopramide.
- If it has been confirmed or suspected the existence of a tumor of the adrenal medulla, due to the risk of severe episodes of hypertension.
- If you are taking levodopa or other drugs that cause extrapyramidal reactions (Parkinson's syndrome, muscle spasms of the face, neck and tongue, incoordination of voluntary movements and difficulty staying still) as phenothiazines and butyrophenones.

Metoclopramide is not recommended in patients with epilepsy because the class of drugs to which belong this drug may promote the development of seizures.

As with other groups of drugs that act on the central nervous system, neuroleptic malignant syndrome (NMS) may occur.

NMS is characterized by fever, extrapyramidal disorders (Parkinson's syndrome, muscle spasms of the face, neck and tongue, incoordination of voluntary movements and difficulty to keep still), nervous autonomic instability, and an increase of muscle breakdown markers.

Therefore, caution should be exercised if fever, one of the symptoms of NMS, and should be discontinued if treatment with metoclopramide if are suspected NMS.

If methemoglobinemia occurs (whose symptoms are a bluish color of the skin and mucous membranes, nausea, headache, dizziness, agitation, tachycardia, and somnolence) treatment with metoclopramide should be immediately and permanently withdrawn and will have to inform your doctor.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

It has to be done with caution combining metoclopramide with the following medications:

- Anticholinergics and morphine derivatives: They have the opposite effect of the metoclopramide on digestive tract motility.
- Levodopa
- Drugs that act by depressing the central nervous system (morphine derivatives, sleep-inducing drugs, anti-anxiety drugs, antiallergic, antidepressants, sedatives, barbiturates, clonidine, and derivatives): They potentiate the sedative effect of the Central Nervous System depressants and metoclopramide
- Drugs acting on the central nervous system: Metoclopramide may enhance its effects when administered with drugs that act on the central nervous system, causing extrapyramidal disorders (Parkinson's syndrome, muscle spasms of the face, neck and tongue, incoordination of voluntary movements and difficulty staying still).
- Digoxin: metoclopramide reduces the blood concentration of digoxin. Comprehensive monitoring of blood digoxin concentration is recommended.
- Cyclosporine: metoclopramide increases the blood concentration of cyclosporine. Comprehensive monitoring of cyclosporine concentration in the blood is recommended.
- Sertraline and fluoxetine: metoclopramide Injectable concomitant administration of both drugs may increase the risk of onset of symptoms of extrapyramidal character (Parkinson's syndrome, facial muscle spasms, neck and tongue, incoordination of voluntary movements and difficulty to keep still).

Use with food and beverages

Avoid combination with alcohol, which enhances the sedative effect of metoclopramide.

Special physiological situations

Pregnancy and lactation

You should not take this medicine during the first trimester of pregnancy, even if have not been proven the adverse effects that could affect the fetus. In the case of pregnancy, you should tell about your situation to the doctor.

Metoclopramide hydrochloride is excreted in breast milk. Therefore, breastfeeding is contraindicated during treatment with this medicine. In the case of being lactating, you must inform to your doctor.

Driving and using machines

During treatment, situations requiring special alertness such as driving or operating dangerous machinery should be avoided, since metoclopramide hydrochloride may produce drowsiness

How to use metoclopramide     

Follow carefully instructions of use of metoclopramide provided from your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

The duration of this treatment is limited. Your doctor will tell you how long your treatment with metoclopramide. Do not stop treatment earlier.

Adults:

The usual dose is 1 tablet, 3 times a day (1 tablet every 8 hours) which means a daily maximum of 3 tablets (30 mg metoclopramide a day).

Oral solution: the usual dose is 5-10 ml, 3 times a day (one for every 8 hours) which means a daily maximum of 6 teaspoons (30 mg of metoclopramide hydrochloride).

In patients with hepatic or renal insufficiency, it is recommended to decrease the dosage.

If vomiting occurs, do not take the medication until eight hours have elapsed since the last dose.

Children:

The usual dose for a child up to 3 years is 5 drops/kg/day (corresponding to 0.5 mg/kg/day metoclopramide). For children over three years, a solution oral format is available. The usual dose for a child over 3 years is ½ - 1 teaspoon dosing (5 ml) 3 times a day (one for every 8 hours), which means a daily maximum 3 teaspoons (15 mg metoclopramide hydrochloride).

In acute or very stubborn cases, your doctor may prescribe to you or your child the injectable form for parenteral (i.m. or I.V.) (pediatric dose = 1/2 ampoule).

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take metoclopramide     

If a doctor prescribes another medicine, tell you are taking metoclopramide.

Check with your doctor regularly developments of disorder that motivates the use of metoclopramide. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with metoclopramide at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with metoclopramide, immediately consult your doctor.

If you take more than you should:

Immediately consult your doctor or pharmacist or go to the emergency department of the nearest hospital. Take the container with you, even if it is empty. .

If recommended doses are exceeded, especially in children, adolescents and elderly, extrapyramidal reactions may occur (Parkinson's syndrome, muscle spasms of the face, neck and tongue, incoordination of volunteers movements, and difficulty to keep still).

By overdose, drowsiness and disorientation may also appear. These symptoms usually disappear after discontinuation of treatment.

Extrapyramidal reactions (Parkinson's syndrome, muscle spasms of the face, neck and tongue, incoordination of voluntary movements and difficulty staying still) are controlled by benzodiazepines in children and/or anticholinergic antiparkinsonian drugs in adults.

If you forget to take metoclopramide

Do not take a double dose to make up the missed dose.

It is best to administer the missed dose as soon as possible, keeping a 8-hour interval for each new administration.

If you stop taking metoclopramide

When stopping treatment, no effect is detected.

Side Effects of metoclopramide     

Like all medicines, metoclopramide can cause side effects although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

This medicine may cause drowsiness, fatigue and diarrhea.

Can occur extrapyramidal symptoms (Parkinson syndrome, muscle spasms of the face, neck and tongue (tardive dyskinesia), incoordination of voluntary movements and difficulty staying still (akathisia)), especially in children and adolescents, even after administration of a single dose of the drug.

These adverse reactions disappear upon discontinuation. Symptomatic treatment may be necessary.

Rarely can occur:

- Allergic reactions.
- a tendency to depression.
- Metabolic disorders, during prolonged treatment, relative to an increase of blood lactogen hormone concentration (eg lack in the appearance (or disappearance) of menstruation, breast milk secretion (galactorrhea), abnormal growth of male mammary gland (gynecomastia).
- methemoglobinemia (whose symptoms are bluish discoloration of the skin and mucous, nausea, headaches, dizziness, agitation, tachycardia and drowsiness, especially in newborns.
- Epilepsy, Neuroleptic malignant syndrome (characterized by an increase in body temperature, extrapyramidal disorders, and nervous instability).
