Indications and usage     

Naproxen belongs to a group of drugs called nonsteroidal anti-inflammatory drugs (NSAIDs), with anti-inflammatory properties (against inflammation), analgesic (pain) and antipyretic effect (reduction of fever).

Naproxen is indicated for the treatment of painful and inflammatory processes in the joints, muscles, tendons and ligaments as rheumatoid arthritis, osteoarthritis and ankylosing spondylitis. It is also indicated for the treatment of acute attacks of gout, rheumatoid syndromes and period pains (dysmenorrhea).

Other ingredients of available commercial presentations:

Sachets: The active substance is naproxen. Each sachet contains 500 mg of Naproxen. The other ingredients are: sodium saccharin, sucrose, mannitol, sodium chloride, sodium dioctylsulfosuccinate, peppermint powder, licorice essence composed and polyvinylpyrrolidone.

Each tablet or capsule, in addition of naproxen, may contain some of the following ingredients: croscarmellose sodium, povidone, magnesium stearate, iron oxide (E172), microcrystalline cellulose, polyvinylpyrrolidone, talc, hydroxypropyl methylcellulose, polyethylene glycol, titanium dioxide (E-171), indigotin lacquer (E-132), gelatin (capsules) and purified water.

Note: Some commercial presentations include lactose, potato starch, povidone K-90, glycerol, or Primojel sodium starch glycolate, magnesium stearate, silicone antifoam 1510, Eudragit L 12.5 (SS) and Eudragit L 30 D (SS), triacetin, talc and polyethylene glycol 600.

Additional warning     

Some brands or generic medicines with naproxen contain glycerol as an excipient, which can be harmful in high doses because it can lead to headache, stomach upset and diarrhea.

Some medicines containing naproxen incorporate lactose or sucrose among excipients. If your doctor has told you that you have diabetes or any kind of intolerance to sugars, contact the doctor who has prescribed the drug

Before taking the drug     

It is important that you use the lowest dose that alleviates or control pain and should not take this medicine for longer than necessary to control symptoms.

Do not use naproxen

If you are allergic (hypersensitive) to the active substance naproxen, to acetylsalicylic acid (aspirin) or other NSAIDs that have caused an allergic reactions or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Also, you should not take this medicine:

- If you previously have had a stomach or duodenal bleeding or suffered a perforation of the digestive system while taking an anti-inflammatory nonsteroidal drug (NSAIDs) .
- If you have a disease of the intestine.
- If you suffer a severe liver or kidney disease.
- In children under 2 years.
- If you have severe heart failure.
- If you are in the third trimester of pregnancy.

Take special care with naproxen

- If you have had or develop an ulcer, bleeding or perforation in the stomach or duodenum, which may be manifested by an intense or persistent abdominal pain and/or black stools, or even without previous warning symptoms.

This risk is increased when are used high doses and prolonged treatment in patients with a history of peptic ulcer disease and the elderly. In these cases, your doctor will consider the possibility of associating a protective stomach medicine.

- If you have asthma or an allergic disorder, since this drug may cause difficulties in breathing (bronchospasm).

- If you have a severe disorder in kidney, liver or heart, consult your doctor.

Medicines such as naproxen may be associated with a small increased risk of heart attacks (myocardial infarction) or brain (brain stroke). This risk is more likely to occur when are used high doses and prolonged treatment.

Do not exceed the dose or recommended duration of treatment.

- If you have heart problems, history of stroke, or think you might be at risk to suffer these conditions (for example,high blood pressure, diabetes, elevated cholesterol, or are a smoker), you should consult this treatment with your doctor or pharmacist.

Also, these drugs can produce fluid retention, especially in patients with heart failure and/or high blood pressure (arterial hypertension).

- If you are taking other anti-inflammatory, consult your doctor.

- If you have or suspect you have an infection, since naproxen may mask the usual signs and symptoms of infectious processes, talk to your doctor.

- If you experience vision disorders during treatment, consult your doctor or pharmacist.

Tell your doctor:

If simultaneously you are taking medications that alter blood clotting or increase the risk of ulcers, as oral anticoagulants, or antiplatelet agents as acetylsalicylic acid. You should also tell your doctor if you take other medicines that may increase the risk of bleeding such as corticosteroids and antidepressants selective inhibitors of serotonin reuptake.

- If you have Crohn's disease or ulcerative colitis since anti-inflammatory drugs naproxen type may worsen these conditions.

Taking other medicines

- Antacids o cholestyramine, because these can delay the action of naproxen,
- Hydantoins (medicines used mainly for epilepsy), as phenytoin,
- Sulfonylurea type medicines such as glyburide or glipizide (diabetes medication),
- Sulfonamides (a type of drug used to increase urine output), such as hydrochlorothiazide
- Methotrexate (an immunosuppressant),
- Probenecid (a medicine used in patients with gout),
- Furosemide (a type of drug used to increase urine output),
- Lithium (antipsychotic, to treat depression and bipolar disorder),
- Angiotensin converting enzyme inhibitors (to lower blood pressure) (ACEI),
- Beta-blockers (to treat high blood pressure), such as atenolol, bisoprolol or metoprolol.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Use with food and beverages

It is advisable to take this drug with meals.

Special physiological situations

Pregnancy & lactation

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Naproxen should not be given during pregnancy or childbirth or while breastfeeding.

Because the administration of naproxen has been associated with an increased risk of congenital abnormalities or abortions, its administration is not recommended during the first and second trimester of pregnancy unless it is considered absolutely necessary. In these cases the dose and duration will be limited to a minimum. In the third trimester of pregnancy, administration of naproxen is contraindicated.

Patients of childbearing age should take into account that the type of drugs to which belongs naproxen have been associated with a decreased ability to conceive.

Children

Naproxen should not be used in children under 2 years old.

Driving and using machines

Naproxen should be used with caution in patients whose activities require attention and have observed vertigo or visual disturbances during treatment with this medicine .

How to use naproxen     

Follow exactly instructions of use of naproxen provided by your doctor. Consult your doctor or pharmacist when in doubt. Rheumatoid arthritis, osteoarthritis and ankylosing spondylitis:

The usual starting dose is 500 to 1000 mg of naproxen twice daily (morning and evening). In patients with severe night pain and/or morning stiffness, patients changing from other anti-inflammatory to naproxen and patients suffering osteoarthritis with pain as the predominant symptom, it is recommended to start therapy with doses of 750-1000 mg daily for several weeks.

As maintenance therapy can be administered a single daily dose of 500-1000 mg in the morning or evening.

Acute gout:

It is recommended an initial dose of 750 mg, followed after 8 hours of 500 mg and then continue with 250 mg every 8 hours until attack be resolved

Dysmenorrhea:

The recommended starting dose is 500 mg followed by 250 mg every 6-8 hours.

Acute musculoskeletal processes:

The recommended starting dose is 500 mg followed by 250 mg every 6 or 8 hours.

In acute crises, if patient lacks an history of gastrointestinal disease, the dose may be increased to 1500 mg for no longer than 2 weeks.

Children:

In juvenile rheumatoid arthritis is recommended a dose of 10 mg/kg/day, divided into two doses, at intervals of 12 hours .

Special dosing patients:

In elderly or patients with heart, liver or kidney diseases, the dose should be reduced.

This drug is for oral administration . Swallow the medicine with a sufficient amount of liquid; preferably during meals.

If you think that the effect of the medicine is too strong or too weak, consult your doctor.

While you take naproxen     

If you become pregnant, contact your doctor immediately.

Symptoms of overdose include:

Dizziness, drowsiness, abdominal pain, indigestion, heartburn, nausea, transitory liver disorders, prothrombin deficiency, kidney disorders, excessive acidity of the blood, temporary suspension of breathing, disorientation or vomiting.

If you forget to take naproxen

Do not take a double dose to compensate missed doses.

Side Effects of naproxen     

Like all medicines, naproxen can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Adverse effects of naproxen are generally very rare (affect less than 1 in 10,000 patients):

Disorders of the blood and lymphatic system:

Agranulocytosis (decrease in certain white blood cells), aplastic or hemolytic anemia (decrease of the number of red cells, white cells and platelets in blood), eosinophilia (increase of certain white blood cells in blood) , leukopenia (decreased number of white blood cells in blood), thrombocytopenia (low platelet counts).

Immune system disorders:

Anaphylactoid reactions (acute allergic reaction), angioneurotic edema (inflammation in the skin, mucosa and viscera).

Metabolism and nutrition disorders:

Hypercalcemia (increased concentration of calcium in the blood).

Psychiatric disorders:

Difficulty concentrating, depression, sleep disorders.

Nervous system disorders:

Dizziness, sleepiness, headache, vertigo, cognitive dysfunction (impaired ability of knowledge), aseptic meningitis (inflammation of the meninges), seizures, insomnia, sleep abnormalities.

Eye disorders:

Vision disorders, corneal opacity (ranging from faint smudge until a larger whitish gray stain that looks to the naked eye), papilitis (inflammation of the papilla), retrobulbar optic neuritis (inflammation of the optic nerve) and papilledema.

Ear disorders:

Changes in hearing, tinnitus (ringing in the ears), hearing loss (decreased hearing).

Cardiovascular Disorders:

Palpitations, vasculitis (inflammation of blood vessels).

Such drugs may be associated with a moderate increase in risk of heart attack or stroke.

Also, can occur edema (fluid retention), arterial hypertension and heart failure in association with treatments as NSAIDs ( group to which belongs naproxen).

Respiratory disorders:

Asthma, eosinophilic pneumonitis (lung inflammation), dyspnea (shortness of breath), pulmonary edema (swelling due to fluid in the lung).

Gastrointestinal disorders:

The most common adverse effects that occur with drugs such as naproxen are gastrointestinal: peptic ulcers, gastrointestinal bleeding, perforations (sometimes fatal), especially in the elderly.

It have also been observed nausea, vomiiting, diarrhea, flatulence, constipation, heartburn, abdominal pain, blood in the stool, mouth ulcers, worsening of ulcerative colitis and Crohn's disease.

Less often: Gastritis.

Hepatobiliary disorders:

Hepatitis (liver inflammation), jaundice (yellowing of the skin). Medicines such as naproxen may be associated in rare cases to liver damage.

Disorders of the skin and subcutaneous tissue:

Edema, ecchymosis (extravasation of blood to the tissues), pruritus (itching), purple (blood stains under the skin), rashes, sweating, alopecia (hair loss), erythema (redness of the skin), exanthema, lichen planus (skin inflammation with red papules mainly on neck, forearms and abdomen), reaction with pustules, other rashes, systemic lupus erythematosus, urticaria, photosensitivity reactions (allergy to sunlight).

If skin fragility or formation of blisters (blisters) occurs immediately consult your doctor.

Rarely, medicines such as naproxen may be associated to very serious bullous reactions such as Stevens Johnson syndrome and toxic epidermal necrolysis.

Disorders of musculoskeletal, connective tissue and bones:

Muscular fatigue and myalgia (muscle pain and weakness).

Renal and urinary disorders:

Hematuria (blood in urine), interstitial nephritis (kidney inflammation), nephrotic syndrome (manifested by dark urine and swelling of the face), kidney disease, renal failure, renal papillary necrosis.

Reproductive system and breast disorders:

infertility.

General disordes:

general discomfort, pyrexia (chills and fever), thirst, sore throat.

Complementary examinations:

Abnormal liver function test values, elevated serum creatinine, hyperkalemia.
