Nortriptyline is a tricyclic antidepressant that has a low sedative and anticholinergic action.
It is indicated in depressive states.

Do not use nortriptyline
If you are allergic (hypersensitive) to nortriptyline, to other tricyclic antidepressants or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.
Allergy symptoms may include:
- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).
Nor you should take this medicine:
- In case of recent myocardial infarction
- arrhythmias
- In the manic phase of bipolar disorder
- If you have severe liver disease
- If you are pregnant
- In children under 6 years.
Take special care with nortriptyline
Use with caution in cases of:
- Kidney or liver failure
- cardiovascular disorders
- Glaucoma
- urinary retention
- Epilepsy
- hyperthyroidism
- Psychotic/schizophrenic patients.
It will be used with caution in patients who are undergoing concomitant treatment with anticholinergic, sympathomimetic, barbiturates and medicines for thyroid disorders.

Sensitive patients to tricyclic antidepressant may be sensitive to other tricyclic antidepressants and possibly also to carbamazepine, maprotiline and trazodone. Therapy should be started at low doses, gradually increasing.

Taking other medicines
Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.
Nortriptyline may potentiate the effect of:
- Carisoprodol
- Epinephrine
- Etilefrine
- Phenylephrine
- Pholedrine
- Mebutamate
- Meprobamate
- Norepinephrine
Nortriptyline can inhibit the effect of:
- Clonidine
- Guanfacine
- Methyldopa.
Nortriptyline enhances the effect and toxicity of Anticholinergics.
Concomitant administration with MAOIs is potentially dangerous and requires close monitoring. You must wait at least 10 days between the withdrawal of an MAOI and the administration of another antidepressant.
The effect of nortriptyline is inhibited by:
- Barbiturates
- Carbamazepine
Nortriptyline toxicity is enhanced by:
- Estrogens
- Levodopa
The effect and toxicity of nortriptyline are enhanced by
- Phenothiazines
- Cimetidine
- Haloperidol.
Mutually reinforcing its toxicity:
- Ethyl alcohol
- Diazepam.
Nortriptyline may alter the values of the following analytical determinations:
- Blood: increase (biological) of thyroid-stimulating hormone (TSH).
Use with food and beverages
It is recommended taking the medicine with food.
Avoid intake of alcoholic drinks.
Consult your doctor or pharmacist before taking any medicine.
Special physiological situations
Pregnancy and lactation
Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.
The safety of this drug during pregnancy is not established so that its administration is not recommended if thepatient is pregnant.
Heart problems, irritability, respiratory distress, seizures and urinary retention have been reported in infants when their mothers have taken tricyclic antidepressants immediately before delivery.
Tricyclic antidepressants are excreted in small amounts in breast milk, therefore, it is not recommended its use during breastfeeding.
Children:
Nortriptyline is contraindicated in children under 6 years.
Elderly
The dosage should be reduced in elderly.
Driving and using machines
Nortriptyline can decrease alertness in some patients, so drive and/or operate machinery can be potentially dangerous.
Follow carefully instructions of use of nortriptyline provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. This drug is for oral use.
The dosage should be established in a personalized way for each patient:
- Usual dose: 25-75 mg per day.
- Maximum dose 100 mg a day.
- Maintenance dose: 25-50 mg per day.
It is recommended the administration of a single daily dose at evening. It is suggested take the drug with a meal.
Swallow the tablet with a little water or other non-alcoholic beverage.
If you think the effect of the medicine is too strong or too weak, consult your doctor.

If you feel unwell during treatment with nortriptyline, immediately consult your doctor.
If you take more than you should:
Symptoms of acute overdose include: confusion; seizures; impaired concentration; severe drowsiness; dilated pupils; fast, slow or irregular heartbeat; fever; hallucinations; restlessness and agitation; shortness of breath or difficulty breathing; unusual tiredness or weakness intense; vomiting.
If you take more nortriptyline than you should, tell your doctor or go to the nearest hospital casualty department straight away. Take the medicine pack with you, even if it is empty.
If you forget to take nortriptyline
Do not take a double dose to make up for missed doses.

Like all medicines, nortriptyline can cause side effects, although not everybody gets them.
The frequency of side effects is classified into:
Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data
Common side effects include dry mouth, constipation, glaucoma, blurred vision and urinary retention.
Uncommon side effects include sedation, drowsiness and cardiovascular disorders.
Rare side effects confusion, reduced ability to concentrate, nausea, vomiting, decreased libido and menstrual abnormalities.
Treatment should be discontinued immediately if the patient experiences an episode of seizures, fever with sweating, hypertension or hypotension, difficulty in breathing, loss of bladder control or severe muscle stiffness.
