Indications and usage     

This medicine belongs to the group of drugs known as Angiotensin II Receptor Blockers (ARBs), which are used in the treatment of arterial hypertension.

Other ingredients of available commercial presentations:

Besides the active ingredient olmesartan medoxomil, each tablet contains microcrystalline cellulose, lactose monohydrate, hydroxypropylcellulose, magnesium stearate, titanium dioxide (E171), talc and hypromellose.

Warning:

Important information     

This medicine contains lactose. If your doctor has told you have an intolerance to sugars, contact him before taking it.

Before taking the drug     

Do not use olmesartan

If you are allergic (hypersensitive) to olmesartan,to tetracyclines or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you are in the second or third trimester of pregnancy.
- If you are breastfeeding your baby.
- If you suffer an obstruction of the bile ducts, which implies an abnormal flow of bile from the gallbladder.

This medicine should not be used in children and adolescents under 18.

Take special care with olmesartan

If you suffer:

- A kidney disease or have had a kidney transplant. There is no experience regarding the administration of olmesartan shortly after a kidney transplant or in patients with severe renal impairment, so its use is not recommended in these cases.
- A liver disease. There is no experience with olmesartan medoxomil in patients with severe liver failure, so the use of this medicine is not recommended in these cases.
- Stenosis of heart valves (narrowing of the mitral or aortic valve) or other factors which reduce the flow of the left chamber of heart (hypertrophic obstructive cardiomyopathy) .
- Conditions that reduce the volume of fluid or sodium in the body (eg vomiting, diarrhea or treatment with high doses of drugs that increase urine flow). These conditions should be corrected before treatment. Otherwise, there is a risk of an excessive decrease in blood pressure.
- Increased aldosterone levels (primary aldosteronism). In these cases, patients do not respond adequately to the implementation of ARBs, including olmesartan.
- Elevated level blood potassium
- Narrowing of renal arteries, which increases the risk of a serious decrease in blood pressure and kidney failure
- Severe heart failure or renal disease, including narrowing of renal arteries.

Excessive reduction in blood pressure can cause a coronary ischemic attack (myocardial infarction) or a stroke in patients with impaired circulatory system in the heart or brain.

You should tell your doctor if you are taking a low sodium diet (salt).

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

If in doubt, consult your doctor.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

You should tell your doctor especially if you are being treated with:

Tell your doctor if you are taking some compounds containing potassium or substitutes of seasoning salt with potassium or potassium-retaining drugs (such as certain diuretics) or any other medicine that increase concentration of potassium in the blood (such as heparin), as they can cause increased levels of blood potassium. The joint use of these medications with olmesartan is not recommended .

The effect of telmisartan can be increased if used while you are using other medicines that lower blood pressure.

The effect of olmesartan on blood pressure may be reduced if you take nonsteroidal antiinflammatory drugs (NSAIDs).

It is not advisable concurrent use of olmesartan and lithium. If you must take this last drug, your doctor may need to measure lithium's level during treatment with olmesartan.

Use with food and beverages

This medicine can be taken before, during or after meals.

Special physiological situations

Pregnancy and lactation

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

This medicine should not be taken in the last two trimesters of pregnancy neither while breastfeeding.

Available information is insufficient to ensure the safe use of olmesartan during the first trimester of pregnancy, so it is not advisable to use during this period of pregnancy.

Ultimately, your doctor may decide to recommend its use, depending on your personal circumstances.

Use in children

This medicine should not be used in patients under 18.

Driving and using machines

Although it is hardly likely that olmesartan may influence the ability to drive or operate machinery, sometimes patients may experience dizziness or fatigue during treatment. Should you observe any of these effects, see a doctor before performing these activities.

How to use olmesartan     

Follow exactly instructions of use of olmesartan provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

This medicine is taken orally. The tablets should be swallowed with some water. It is recommended, if possible, to take your daily dose at the same time each day.

Your doctor will tell you how long your treatment with olmesartan. Do not stop treatment without consulting him.

Unless your doctor tells you otherwise, the usual dose is specified below:

The recommended starting dose is 10 mg. If cannot properly control blood pressure with this dose, your doctor may increase it to 20 mg. If it is necessary an additional reduction in blood pressure, your doctor can increase it to 40 mg and/or add a second drug, such as hydrochlorothiazide.

Elderly:

These patients generally do not require an adjustment of dose.

Patients with renal impairment:

The maximum dose in patients with mild to moderate renal insufficiency is 20 mg olmesartan once daily.

Patients with hepatic impairment:

No dose adjustment required.

In cases of moderate hepatic impairment, it is recommended an initial dose of 10 mg of olmesartan once daily. The maximum recommended dose for these patients should not exceed 20 mg daily.

While you take olmesartan     

If a doctor prescribes another medicine, tell you are taking olmesartan.

Check with your doctor regularly developments of disorder that motivates the use of olmesartan. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with olmesartan at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with olmesartan, immediately consult your doctor.

If you take more than you should:

Consult your doctor or pharmacist immediately or call the National Poisons Information Service in your country. If you have taken an overdose of olmesartan, go to the emergency department of the nearest hospital. Take the container with you, even if it is empty.

If you forget to take olmesartan

Tf you forget to take a daily dose, take your usual dose the next day. Do not take a double dose to make up for missed doses

If you stop taking olmesartan

You should not stop taking this medication without approval of your doctor, because your blood pression could increase.

Side Effects of olmesartan     

Like all medicines, olmesartan can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

During clinical trials, it has only been observed more frequently dizziness in patients receiving olmesartan (2.5%) than in patients taking placebo (0.9%).

The following side effects have been observed both in patients treated with olmesartan and placebo-treated patients:

Common side effects:

- Bronchitis, cough, pharyngitis (sore throat), rhinitis (inflammation of nasal mucosa).
- Stomach pain, diarrhea, dyspepsia (indigestion), nausea, gastroenteritis (inflammation of the stomach and intestine).
- Inflammation of joints, back pain, bone pain.
- Blood in urine, urinary tract infection.
- Chest pain, fatigue, flu-like symptoms, peripheral edema (swelling of the ankles), pain.

Uncommon side effects

- Vertigo.
- Angina.
- Rash

Rare side effects:

- Hypotension (low blood pressure).

In addition, during the post-marketing phase, have been observed the following side effects:

Very rare side effects:

- Thrombocytopenia (decreased number of blood platelets).
- Dizziness, headache.
- Cough.
- Abdominal pain, nausea, vomiting.
- Pruritus (itching), skin rash, angioneurotic edema (angioedema), allergic dermatitis, facial edema and urticaria.
- Muscle cramps, myalgia (muscle pain).
- Acute renal failure, renal failure.
- Asthenia, fatigue, lethargy, general discomfort.

The following findings in laboratory parameters have been observed both in patients treated with olmesartan and placebo-treated patients.

Common:

- Increased creatine phosphokinase (CPK).
- Increased blood fat (hypertriglyceridemia).
- Increased blood level of uric acid (hyperuricemia).
- Increased liver enzymes.

Rare:

- Increased blood potassium levels (hyperkalemia).

In addition, during the post-marketing phase, have been observed the following findings related to laboratory parameters.

Very rare:

- Increased blood creatinine.
- Increased blood urea.
- Increased liver enzymes.

In elderly patients the frequency of hypotension is slightly increased from rare to uncommon.
