Indications and usage     

Quinapril belongs to the family of drugs called angiotensin-converting enzyme inhibitors (ACE), drugs from the group of antihypertensive which act by dilating blood vessels in the body.

Quinapril is indicated for treatment of patients with high blood pressure (hypertension) and treatment of patients with a heart condition known as chronic heart failure, associated to the usual treatment.

Each tablet contains as excipients: Hydrous lactose, magnesium carbonate (E504), magnesium stearate (E572), gelatin (E441), crospovidone, candelilla wax (E902), red iron oxide (E172) oxide yellow iron (E172), titanium dioxide (E171), hydroxypropyl cellulose (E463), hydroxypropyl methyl cellulose (E464) and macrogol 400.

Important information     

This medicine contains lactose. If your doctor has told you have an intolerance to some sugars, contact him before taking it.

Before taking the drug     

Do not use quinapril

If you are allergic (hypersensitive) to the active substance quinapril or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Also, you should not take this medicine:

If you have previously suffered severe allergic reactions such as swelling of the face, lips, tongue and/or throat with difficulty breathing, swallowing and speaking (angioedema of the head and neck) or inflammation of the intestinal tract (intestinal angioedema).

If this situation is applicable to you or you are not sure, tell your doctor before taking this medicine.

Take special care with quinapril

- If you have a liver disease.
- If you are diabetic.
- If you are going to have surgery, tell your doctor that you are taking quinapril.
- Do not use salt substitutes containing potassium without consulting your doctor.
- If you are pregnant, planning a pregnancy or breast-feeding your child.
- If you are taking other medicines.
- If you follow a strict diet without salt.
- If you have a kidney disease, or are on dialysis or have received a kidney transplant. The doctor may do you various tests regularly to see if your kidneys are working well.
- If you have a disease that affects the blood vessels (collagen vascular disease) or kidney (renal failure) your doctor may do various tests to check the status of your immune system.
- If you are taking other medicines to reduce your blood pressure, as it could be reduced excessively (hypotension). Other processes such as vomiting, diarrhea, dehydration and excessive sweating can also lower blood pressure.

Check with your doctor if you experience dizziness and vertigo. If feel dizzy, lie down until this sensation passes. After recovering, always should get up slowly to avoid dizziness. If you faint or tide, tell your doctor.

As with other ACE inhibitors, treatment with quinapril may produce dry cough. If this occur, consult your doctor. This cough disappears after stopping treatment.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, included OTC medicines (over the counter, no medical prescription required) and herbal products.

This medicine may interact with the drugs listed below. Do not take these medicines while you are taking quinapril, unless it has been prescribed the doctor:

- Lithium (medicine to treat depression)
- Tetracyclines (antibiotic)
- Diuretics
- Potassium supplements, medicines or salt substitutes containing potassium
- Diabetes medications (insulin or oral hypoglycemic)

Use with food and beverages

This medicine may be taken with or without food.

Special physiological situations

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy and lactation:

If you are pregnant, you want to become pregnant or become pregnant while being treated with quinapril, stop taking them as soon as possible and tell your doctor immediately.

If taken during the second and third trimester of pregnancy, angiotensin converting enzyme inhibitors (group to which belongs quinapril) may cause fetal damage and death.

Administration of this drug while breastfeeding is recommended only if, in opinion of doctor, there is not a safer alternative treatment and when the disease itself implies a greater risk to the mother or baby.

Children:

Quinapril should not be given to children.

Elderly:

Driving and using machines

Quinapril can cause a sudden drop in blood pressure, causing dizziness and fainting in some cases. This can occur during the first few days of treatment. How to use quinapril     

Follow exactly instructions of use of quinapril provided by your doctor. Consult your doctor or pharmacist when in doubt. This drug is given orally. Your doctor will tell you how many tablets you should to take each day.

How to take:

Swallow the tablets whole with a sufficient amount of liquid (eg a glass of water). Do not chew them. Take your tablets approximately the same time each day.

The tablets are usually taken once or twice a day.

Follow these instructions unless your doctor gave you different advice.

Remember to take your medicine.

Your doctor will tell you how long your treatment with quinapril. Do not stop treatment early because it can be harmful to your health.

Arterial hypertension:

The usual starting dose is 10 mg. Your doctor may increase the dose up to 20-40 mg a day, depending on the response obtained.

Heart failure:

The usual starting dose is 2.5 mg. Your doctor may increased up to 10-40 mg a day, depending on the response obtained. For this indication quinapril are used in combination with other drugs such as diuretics (water pills) or cardiac glycoside (eg, digoxin).

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take quinapril     

If you become pregnant, contact your doctor immediately.

If a doctor prescribes another medicine , tell you are taking quinapril.

Check with your doctor regularly developments of disorder that motivates the use of quinapril. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with quinapril at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with quinapril, immediately consult your doctor.

If you use more medicine than you should

If, for any reason, you have taken more medicine than you should, immediately consult your doctor or pharmacist or call the Toxicological Information Service in your country, indicating the medicine and the amount used.

The most likely symptoms would be expected following an overdose of quinapril is the drop in blood pressure. It will proceed to symptomatic treatment to offsetting the effects of this medicine.

In severe cases, should be taken appropriate measures as well as maintenance of blood volume; medications may be needed to adequately restore blood pressure.

If you forget a dose:

If you forget to take a dose, do not take a double dose to make up for a forgotten dose and continue with treatment as usual.

If you stop taking quinapril

Blood pressure may return to pre-treatment values, so it is not recommended without the advice and prior approval from your doctor.

Side Effects of quinapril     

Like all medicines, quinapril can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Common sede effects (affect 1-10 in 100 patients):

Headache, vertigo, inflammation of the mucosa of the nose (rhinitis), cough, fatigue, nausea and/or vomiting, muscle pain (myalgia), diarrhea, chest pain, abdominal pain, digestive function disorders (dyspepsia), shortness of breath (dyspnea), back pain, inflammation of the pharynx (throat), difficulty falling asleep (insomnia), low blood pressure (hypotension), and tingling (paraesthesia).

Uncommon side effects(affect 1-10 in 1.000 patients):

Allergic reaction (anaphylactic reaction), increased abnormal reactivity to sunlight (photosensitivity), palpitations, dilation of blood vessels (vasodilation), oppressive chest pain (angor pectoris), increased heart rate (tachycardia), decrease in blood pressure when standing up (postural hypotension), síncope, flatulencia, dry mouth or throat, inflammation of the pancreas (pancreatitis), decreased levels of platelets (thrombocytopenia), depression, nervousness, drowsiness, vertigo, hair loss (alopecia), enfermedades de la piel (dermatitis exfoliativa, pemphigus, pruritus and rash), increased sweating, urinary tract infections, erectile dysfunction, decreased visual acuity, excessive accumulation of fluid in the body (edema), joint pain (arthralgia) and anemia characterized by rapid destruction of red blood cells (hemolytic anemia).

Rare side effects (affect up to 1 in 1.000 patients):

Severe allergic reaction includion swelling of the face, lips, tongue and/or throat with difficulty breathing, swallowing and speaking (head and neck angioedema), inflammation in the gastrointestinal tract (intestinal angioedema), respiratory involvement (eosinophilic pneumonitis) and liver diseases (hepatitis, liver failure).

Laboratory tests:

ADisorder of blood cells (agranulocytosis, neutropenia), increased blood potassium (hyperkalemia), and alterations in various laboratory tests.
