Ramipril is an antihypertensive drug that belongs to a group called inhibitors of angiotensin converting enzyme (ACE). The active agent which produces reducing blood pressure is a product formed in the metabolism of ramipril.

This drug is indicated for the treatment of:

- Arterial hypertension, alone or in combination with other antihypertensives, for example, diuretics or calcium channel blockers.
- Heart failure subsequent to myocardial infarction.
- Treatment of renal complications of diabetes mellitus in patients insulin-dependent, both normotensive or hypertensive and The incipient nephropathy in insulin-dependent patients with hypertension characterized by microalbuminuria
- Ramipril is not suitable for the treatment of hypertension due to primary hyperaldosteronism.
- Prevention of myocardial infarction, stroke or cardiovascular death in patients with high cardiovascular risk, such as coronary heart disease (with or without a clinical history of myocardial infarction), medical history of stroke or peripheral vascular disease.

Tablets:

Besides the active ingredient ramipril, each tablet contains the excipients hydroxypropyl methylcellulose, pregelatinized cornstarch, microcrystalline cellulose and sodium stearyl fumarate.

Capsules:

Besides the active ingredient, the capsules may contain the following inactive ingredients: gelatin, lactose, magnesium stearate, titanium dioxide, black printing ink, yellow iron oxide, erythrosine, indigo carmine, black iron oxide.

The capsules contain lactose If your doctor has told you that you have an intolerance to some sugars, contact him before taking it.

Do not use ramipril

If you are allergic (hypersensitive) to ramipril, to any other medicine of the ACE inhibitors group or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

Take special care with ramipril

To assess if your blood pressure suffers a significant drop and thus apply the corrective measures that are appropriate, should monitor blood pressure regularly.

If you suffer a drop in blood pressure, it is important among other measures, to drink water or fluids to replace liquids and to lie down and raise your legs. You should also consult your doctor.

If you have or have had a kidney disease, or if you have had a kidney transplant or are over 65 years old, the doctor must monitor the functioning of your kidneys during the first few weeks that you are taking this medicine.

If you have a liver disease, your doctor will take appropriate measures for treatment with ramipril.

During your treatment with this medicine, regular monitoring of blood levels of sodium, potassium and white blood cells generally are required.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

If you are taking any diabetes drug, consult your doctor before taking ramipril as it could lower your blood sugar level (glucose).

Some substances can alter blood tests if administered with this medicine, such as:

- Allopurinol (medicament for the treatment of gout),
- Immunosuppressive (drugs that by their activity cause a decrease in the body's defenses),
- Corticosteroids (anti-inflammatory drugs) procainamide (medication for arrhythmia), cytostatics (cancer drugs).

Ramipril should not be used in conjunction with:

- Medicines that may cause a decrease in blood pressure, such as nitrates tricyclic antidepressants anesthetic;
- Some medications for inflammation and pain (NSAIDs) for example indomethacin and acetylsalicylic acid;

Use with food and beverages

Common salt reduces the antihypertensive effect of ramipril.

This drug enhances the effect of alcohol.

This medicine may be taken during or after meals as absorption is not affected by foods.

Special physiological situations

Pregnancy and lactation:

Always consult your doctor or pharmacist before taking any medicine during pregnancy or breastfeeding.

If you are pregnant or think you may be pregnant, do not take ramipril.

If you become pregnant during treatment, immediately stop taking this medicine and ask your doctor to prescribe another type of medication.

If administered during pregnancy, inhibitors of converting enzyme inhibitor (ramipril group to which it belongs) may cause fetal damage and death. If pregnancy is detected, administration of ramipril should be discontinued as soon as possible

During lactation, you should not take ramipril If its administration is required, your doctor will tell you to stop breastfeeding to prevent the baby from taking small amounts of the drug, because ramipril passes into breast milk.

Driving and using machines

This medicine may affect your ability to concentrate and to react due to the decrease in blood pressure that occurs, and therefore, could pose a risk to those situations where these abilities are of special importance, primarily at the beginning of treatment .

Take ramipril during or after meals, without chewing and ingesting plenty of fluids (approximately half a glass of water).

Your doctor will prescribe the lowest dose sufficient to produce the desired effect.

Cardiovascular or renal prevention

It is recommended to start treatment with 2.5 mg of ramipril daily. Your doctor may increase this dose gradually.

Usually, the doctor will prescribe a double dose (5 mg) after one week of treatment. After three weeks he will again increase the dose up to 10 mg.

The maintenance dose is 10 mg ramipril daily.

Treatment of arterial hypertension

An initial dose of 2.5 mg of ramipril is recommended. According to the results, your doctor will increase the dose up to twice after two or three weeks.

The usual maintenance dose may be 2.5 mg to 5 mg. The maximum dose allowed is 10 mg

Sometimes your doctor may prescribe this medicine with others to lower blood pressure.

Treatment of heart failure subsequent to myocardial infarction

Your doctor will recommend an initial dose of 5 mg daily, divided into two doses of 2.5 mg in the morning and one at evening.

If you can not tolerate this dose, your doctor will prescribe 1.25 mg twice a day for two days. Then your doctor will increase the dose depending on your response.

Then, the starting dose divided into two doses will be joined in a dose alone. The maximum dose that will let you take your doctor is 10 mg.

Special cases

If you suffer from kidney disease (reduced filtering capacity of the kidneys) with a creatinine clearance of 50-20 ml/min or do you have more than 65 years, the doctor will prescribe an initial dose of 1.25 mg ramipril, subsequently, the doctor can increase the dose up to a 5 mg tablet daily.

If you are taking any of the medicines called diuretics, it is recommended to stop taking it two or more days before starting treatment with ramipril or reducing the diuretic dose if your doctor tells you so.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

If you take more than you should:

An accidental overdosing could lead to a severe drop in blood pressure (which may lead to shock), to a reduction in heart rate (bradycardia) and kidney problems (renal failure, electrolyte disturbances).

If you forget to take ramipril

Do not take a double dose to make up for missed doses.

Like all medicines, ramipril can cause side effects although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Because ramipril works by reducing blood pressure, many adverse effects are secondary to this reason

Common side effects:

Respiratory system: dry, irritating cough, which is often worse at night when you are lying and is more common in women and nonsmokers.

Uncommon side effects

Digestive system: Nausea.

Skin: eruptions, itching, hives, angioedema manifesting by swelling of the throat and difficulty swallowing or breathing. If this effect is intense you should go to a medical center because it requires emergency measures.

Eyes: conjunctivitis

Nervous system: headaches, impaired balance, dizziness, weakness, daze.

Musculoskeletal system: muscle cramps

Eyes: vision disturbances

Nervous system: Tremors dizziness neurasthenia.

Other side effects:

Loss of appetite changes in sense of taste and smell total or partial loss of taste and/or smell temporary impotence decreased sexual appetite (libido) increase sweating. increased allergic sensitivity to insect stings.

Reduction in the parameters of blood tests (hemoglobin, platelets, leukocytes, agranulocytosis, hemolytic anemia, pancytopenia or bone marrow depression). These changes in the formula of blood are more common in patients with poor kidney function (renal failure) or diseases related with collagen (lupus erythematosus or scleroderma) or in patients who are taking at the same time other drugs that act on these blood components.
