Indications and usage     

Ropinirole belongs to a group of medicines called dopamine agonists. Dopamine agonists act the same way as a natural substance found in the brain called dopamine.

Ropinirole is used to treat idiopathic restless legs syndrome moderate to severe. A moderate or severe restless legs syndrome includes patients who have difficulty sleeping or severe discomfort in the legs or arms.

Restless legs syndrome is a condition characterized by an irresistible urge to move the legs and, occasionally, the arms, usually accompanied by uncomfortable sensations such as tingling, burning or prickling. These symptoms occur during periods of rest or inactivity such as sitting or lying down, especially in bed and worsen in the afternoon and evening. Usually, the only relief is obtained by walking about or moving the affected limbs, which often leads to problems sleeping.

Ropinirole relieves the discomfort and reduces the urge to move the limbs that disrupt nighttime sleep.

The tablet formulation of sustained release is used to treat Parkinson's disease.

Patients suffering Parkinson's disease have low levels of dopamine in certain parts of the brain. Ropinirole has a similar effect to natural dopamine and thus, reduces the symptoms of Parkinson's disease.

In addition to the active ingredient, ropinirole hydrochloride, this medicine contains lactose monohydrate, microcrystalline cellulose, croscarmellose sodium, magnesium stearate, hypromellose, macrogol 400, titanium dioxide, yellow iron oxide, red iron oxide, and indigo (indigo carmine).

The prolonged release tablets contain, besides ropinirole hydrochloride, hypromellose, hydrogenated castor oil, carmellose sodium, povidone, maltodextrin, magnesium stearate, lactose monohydrate, colloidal anhydrous silica, mannitol, ropinirole oxide yellow iron, glycerol di behenate, pink OPADRY OY-S-24900 (hypromellose, iron oxide yellow, titanium dioxide, macrogol 400, iron oxide red).

Important information     

This drug in tablets or extended-release tablets contains lactose. If your doctor has told you that you have an intolerance to certain sugars, contact him before taking.

Before taking the drug     

Do not use ropinirole

If you are allergic (hypersensitive) to ropinirole or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you have a severe liver disease
- If you have a severe kidney disease

Take special care with ropinirole

Tell your doctor before you start taking this medicine:

- If you are pregnant or think you are pregnant
- If you are breastfeeding
- If you have an intolerance to some sugars (eg lactose)
- If you have a liver disease
- If you have any heart ailment
- If you have a serious mental disorder
- If you've had impulses and/or unusual behaviors (such as gambling or excessive sexual behavior). Tell your doctor if you or someone in your family notice that you are having unusual behaviors (as unusual urges or impulses to gambling and/or increased sexual behavior) during treatment with ropinirole. Your doctor may adjust the dose or interrupt ropinirole treatment.

During treatment with ropinirole should take special care when driving or using machinery. If you experience excessive sleepiness or fall asleep suddenly without apparently feeling sleepy, do not drive or use machinery, and contact your doctor.

If during treatment, your symptoms become worse, begin earlier in the day or after less time at rest, or affect other parts of your body such as arms, consult your doctor who may adjust the ropinirole dose you are taking.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

If in doubt, consult your doctor.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

The effect of ropinirole may be increased or decreased by other medicines and vice versa. These drugs include:

- Ciprofloxacin
- Enoxacin
- Fluvoxamine
- Theophylline
- Hormone Replacement Therapy
- Antipsychotics and other drugs that block dopamine in the brain, such as sulpiride or metoclopramide.

Tell your doctor if:

- You are already receiving treatment for Restless Legs Syndrome.
- If you are quitting or starting smoking while taking ropinirole. Your doctor may need to adjust your dose.
- If you are taking ropinirole and your doctor will prescribe any other medicine.

Use with food and beverages

Taking Ropinirole with food may reduce the likelihood of being dizzy.

Consult your doctor or pharmacist before taking any medicine.

Special physiological situations

Pregnancy and lactation

The use of ropinirole is not recommended during pregnancy. This medicine should only be used during pregnancy if your doctor considers that the benefit to you is greater than any possible risk to the fetus.

Tell your doctor immediately if you are pregnant, if you think you are pregnant or planning to become pregnant. Your doctor will advise you to discontinue treatment.

Ropinirole should not be used during breast-feeding as milk production may be affected. Tell your doctor immediately if you are breastfeeding or plan to breastfeed. Your doctor will advise you to discontinue treatment.

Use in children

Not studied the use of ropinirole in children with restless legs syndrome, so this drug is not usually prescribed for patients under 18.

Driving and using machines

This medicine does not usually affect the daily activities of patients. However, it can cause extreme sleepiness (somnolence) and sudden sleep onset episodes. If you experience any of these effects, you must refrain from driving or engaging in activities in which, if you suffer sudden sleepiness or falling asleep, may be a risk of serious injury or death (for example using machinery) until these episodes are resolved.

How to use ropinirole     

Follow carefully instructions of use of ropinirole provided from your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Take ropinirole once a day by mouth, every day at about the same time.

Ropinirole is usually taken just before bedtime, but can be taken up to three hours before bedtime.

Swallow the tablets whole with a glass of water. You can take it with or without food. Taking Ropinirole with food may decrease the occurrence of nausea, which is a possible side effect of this medicine.

Do not chew the tablets.

Treatment of restless legs syndrome:

The exact dose of ropinirole may be different depending on the person. Your doctor will decide the dose you need each day and you should follow their instructions. When you start taking it for the first time, your dose will be increased gradually.

The initial dose is 0.25 mg once a day. After two days your doctor will probably increase to 0.5 mg once a day for the rest of the first week of treatment. Then your doctor may increase your dose by 0.5 mg per week over three weeks up to 2 mg per day.

In patients who have not experienced a sufficient improvement, the dose may be increased gradually up to a maximum of 4 mg daily.

After three months of treatment with ropinirole, it is possible that your doctor may adjust your dose or discontinue your treatment, depending on your symptoms and how you feel.

Treatment of Parkinson's disease:

The usual dose of prolonged release tablets is 2 mg once daily during the first week. After, your doctor may increase the dose to 4 mg once a day for the second week of treatment. In very elderly patients, the doctor may increase the dose more slowly.

Then, your doctor may adjust more accurately the dose to achieve the best dose for you. Some people take up to 24 mg extended release tablets daily.

If, at the beginning of the treatment, you experience side effects that can not tolerate, consult your doctor.

Swallow the extended-release tablets whole with a glass of water. Do not break, chew or crush these tablets. If you do, there may be a danger of receiving an overdose, because the medicine is released into your body too quickly.

Remember to take your medicine. If you have trouble remembering when to take your medicine, ask your pharmacist for advice.

You should continue to take your medicine even if you do not feel better, as several weeks may be necessary for the medicine to take effect.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take ropinirole     

If a doctor prescribes another medicine, tell you are taking ropinirole.

Check with your doctor regularly developments of disorder that motivates the use of ropinirole. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with ropinirole at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with ropinirole, immediately consult your doctor.

If you take more than you should:

Symptoms of overdose include nausea and vomiting, dizziness, drowsiness, fatigue (mental or physical tiredness), stomach pain, lightheadedness, or nervousness.

If you take more ropinirole that you should, or if someone else has taken your medicine, consult your doctor or pharmacist immediately or call the National Poisons Information Service in your country. If you have taken a ropinirole overdose, go to the emergency department of the nearest hospital. Take the container with you, even if it is empty.

If you forget to take ropinirole

If you miss a dose, do not take a double dose to make up for a forgotten dose.

When you remember, take the next dose at the usual time.

If you forget to take ropinirole for several days, consult your doctor to start taking it again.

If you stop taking ropinirole

Consult your doctor if your symptoms worsen after you stop treatment.

Side Effects of ropinirole     

Like all medicines, ropinirole can cause side effects although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Tell your doctor if you notice any side effects that worry you. The most common ropinirole adverse reactions may occur early in treatment and/or when increasing the dose. They are generally mild and may decrease after you have taken the medicine for a while.

Common and very common side effects:

- Feeling sick
- Dizziness (or vertigo)
- Drowsiness
- Fatigue (mental or physical tiredness)
- Stomach pain
- Lightheadedness
- Nervousness

Uncommon side effects:

- Confusion and Hallucinations.
- Reduction in blood pressure, which can cause that you feel dizzy or faint, especially when standing up from a sitting or lying.

During treatment with this medicine, you may experience an unusual worsening of symptoms, (eg symptoms become worse, start earlier in the day or after less time at rest, or affect other body parts like arms). If this happens, consult your doctor.

Consult your doctor if your symptoms worsen after you stop taking ropinirole.

Very rarely, there have been reports of altered liver function (increased enzyme levels).

This medicine may cause an extreme daytime sleepiness (excessive drowsiness) and very rarely sudden onset of sleep, in which patients suddenly fall asleep without apparently feeling sleepy.

Have been reported other psychotic reactions in addition to hallucinations, such as confusion severe (delusions), irrational ideas (misconceptions) and irrational suspicion (paranoia).

Some patients taking ropinirole may have different behavior than usual, for example, unusual urge to gambling and/or increased sexual behaviors.
