Indications and usage     

Secnidazole is a synthetic derivative of the group of nitroimidazoles. Its antiparasitic activity affects Entamoeba histolytica, Giardia lamblia, Trichomonas vaginalis and Gardnerella vaginalis.

This substance is indicated for the treatment of intraintestinal and extraintestinal amebiasis (liver), intestinal diseases caused by Giardia lamblia (giardiasis or lambliasis), trichomoniasis (vaginitis and urethritis) and nonspecific vaginosis.

Other ingredients of available commercial presentations:

Besides secnidazole, this medicine can include excipients such as lactose, tartrazine, magnesium stearate, microcrystalline cellulose, corn starch, sodium glycolate and talc.

Warning:

Important information     

This medicine contains lactose. If you have an intolerance to sugars, contact your doctor before taking it.

This medicine contains tartrazine (yellow dye), which can lead to allergic reactions, especially in patients with a history of reactions to acetylsalicylic acid (aspirin). If you have known allergies such as asthma, rhinitis, urticaria, etc. and/or have allergic reactions to aspirin, consult your doctor before taking it.

Before taking the drug     

Do not use secnidazole

If you are allergic (hypersensitive) to the active substance secnidazole or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you have a severe liver disorder
- If you have a blood dyscrasia or porphyria
- If you have an active organic disease of the nervous system

Take special care with secnidazole

If you are being treated with anticoagulants, lithium or phenobarbital. Ask your doctor how to control the possible undesirable effects of coadministration of any of these medications with secnidazole.

it has been observed a slight decrease in white blood cell count during treatment with secnidazole, so it is recommended to do blood tests before and after treatment, especially if is needed to repeat treatment in patients with amebiasis and trichomoniasis.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any medications, or you receive treatment for kidney problems or for any present illness such as antacids, sucralfate, even those without a prescription.

- Secnidazole enhances the effects of anticoagulants (warfarin, acenocoumarol), so the concurrent administration with these drugs may increase the risk of bleeding.
- Simultaneous administration with phenobarbital reduces the effects of secnidazole and increases its elimination speed.
- Simultaneous administration with lithium derivatives increases the risk of toxicity due to antidepressant.
- It should not be administered simultaneously secnidazole and disulfiram (medicine used in alcohol detoxification), as it may cause neurological symptoms such as confusion and delirium.

There are published reports of psychotic reactions in patients who have taken secnidazole together with disulfiram (see below Use with food and beverages).

Simultaneous administration with cimetidine decreases its elimination and increases the risk of overdose.

Laboratory tests:

Secnidazole can interfere with liver function tests, masking the results of those tests, giving lower values of the real. This substance can also interfere with the determination of blood glucose obtained by hexokinase procedure.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Use with food and beverages

Secnidazol absorption is not influenced by food intake; can be administered before, after or while meals.

Secnidazole should not be administered with drugs whose composition includes ethyl alcohol neither with alcoholic beverages since may occur aches, malaise, severe hypotension, tachycardia, flushing and vomiting, which is known as disulfiram effect. For this reason, drinking alcoholic beverages should be avoided during treatment including up to 4 days after completion.

Secnidazole should not be given to patients who have taken disulfiram in the past two weeks.

Special physiological situations

Always consult your doctor or pharmacist before taking any medicine during pregnancy or lactation.

Pregnancy and lactation:

Secnidazole should not be administered during pregnancy, especially during the first trimester. Secnidazole reaches in breast milk similar concentration to that of maternal blood, so it must not be administered to nursing mothers.

Children:

Not to administer to children under 2 years.

Elderly:

No special precautions are required for reason of age.

Driving and using machines

No adverse effects have been observed. However, in the event that appears dizziness or other adverse effects that could prevent you to carry out these activities, avoid driving and the use of potentially hazardous machinery.

How to use secnidazole     

Follow exactly instructions of use of secnidazole provided by your doctor. Consult your doctor or pharmacist when in doubt. Your doctor will set the dose and duration of treatment most appropriate for your particular case. As a guide, the most common treatment guidelines with secnidazole are indicated :

Intestinal amebiasis and giardiasis:

Children over 2 years: single dose of 30 mg/kg/day, with a maximum of 2000 mg.

Adults: Single dose of 2000 mg.

Observations:

For asymptomatic amoebiasis (called cystic form) should prolong treatment for 3 days.

In giardia lamblia infestation, it is recommended treat all cohabitants with the patients, to avoid reproducing the disease.

hepatic amoebiasis:

Children over 2 years: 30 mg/kg/day with a maximum of 2000 mg for 5-7 days.

Adults: 500 mg 3 times daily for 5 to 7 days.

Urethral or vaginal trichomoniasis:

Adults: 2000 mg as single dose.

Observations:

It should be treated simultaneously the patient's partner.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take secnidazole     

If you become pregnant, contact your doctor immediately.

If a doctor prescribes another medicine , tell you are taking secnidazole.

Check with your doctor regularly developments of disorder that motivates the use of secnidazole. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with secnidazole at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with secnidazole, immediately consult your doctor.

If you use more medicine than you should

May appear nausea, vomiting, abnormal gait (ataxia) . See your doctor immediately or go to the nearest hospital. Do not forget to show to doctor the prospectus and the drug not consumed

If you forget a dose:

Take it as soon as you remember and then continue normally if required.

Do not take a double dose to make up for forgotten individual doses.

Side Effects of secnidazole     

Like all medicines, secnidazole can cause side effects, although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

This medicune usually does not cause severe adverse reactions, although have been reported occasionally adverse effects as decreased appetite, headache, discomfort, heartburn and gastric pain, nausea, vomiting, diarrhea o constipation, dry mouth and bad taste (metallic) and vertigo. These effects are transient and do not require discontinuation of treatment. They may reduce gastrointestinal symptoms by administering a single dose at night, compared with reactions that occur when administered in the morning.

Other adverse reactionss:

Hematological side effects:

Decrease in white blood cells, particularly neutrophils (leukopenia, neutropenia)) and platelets (thrombocytopenia), both transients.

Dermatological side effects:

Urticaria, rash and pruritus (itching).

Neurological side effects:

Seizures, peripheral neuropathy, vertigo, motor incoordination con abnormal gait (ataxia), confusional state, irritability, depression, weakness and sleep disturbance.

Cardiovascular side effects:

Alterations in the ECG: QT prolongation.

Genitourinary side effects:

Cystitis, polyuria, dysuria, urinary incontinence and dark urine, vaginal candidiasis, dyspareunia, decreased libido.

Other disorders:

Abdominal distension, glositis, proctitis, fever, hot flushes, nasal congestion and dry mucous membranes (oral, vaginal).
