Indications and usage ▲

Simvastatin is a drug from the group of statins used to reduce concentrations of total cholesterol, the so-called bad cholesterol (LDL) and fatty substances called triglycerides circulating in the blood.

In addition, this drug raises levels of "good" cholesterol (HDL-cholesterol).

While taking this medication should follow a proper diet to lower cholesterol.

Simvastatin is used along with a proper diet if you have:

- High blood cholesterol levels (primary hypercholesterolemia) or high blood levels of fatty substances (mixed hyperlipidemia).
- A hereditary illness (homozygous familial hypercholesterolemia), which increases the level of blood cholesterol. You may also receive other treatments.
- Coronary heart disease or you have a high risk of coronary heart disease (because you have diabetes, history of stroke or other blood vessel disease).

Simvastatin may prolong your life by reducing the risk of heart disease problems, regardless of the amount of cholesterol in your blood.

In most individuals, there are no immediate symptoms of high cholesterol, but can be detected with a simple test. Visit your doctor regularly and control your cholesterol.

This drug in tablets contains lactose. If your doctor has told you that you have an intolerance to certain sugars, contact him before taking.

Before taking the drug ▲

Do not use simvastatin

If you are allergic (hypersensitive) to simvastatin or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you currently have liver problems
- if you are pregnant or breastfeeding

Nor it is recommended to take simvastatin if you are receiving:

Itraconazole, ketoconazole (medicines for fungal infections), erythromycin, clarithromycin, telithromycin (antibiotics for infections) or inhibitors of HIV protease such as indinavir, nelfinavir, ritonavir and saquinavir (inhibitors of HIV protease are used to treat HIV infections) or nefazodone (a medicine for depression).

Take special care with simvastatin

1. Tell your doctor about all your medical problems that you suffer, including allergies.
2. Tell your doctor if you consume substantial quantities of alcohol.
3. Tell your doctor if you have ever had a liver disease. Simvastatin may not be right for you.
4. Tell your doctor if you will undergo surgery. You may need to stop taking the tablets of simvastatin for a short time.

Your doctor will do a blood test before you start taking simvastatin. This is to check how well your liver is working.

Consult your doctor immediately if you have pain, tenderness or unexplained muscle weakness. This is because, on rare occasions, muscle problems can be serious, including muscle breakdown resulting in kidney damage. Very rarely deaths have occurred.

The risk of muscle breakdown is greater at higher doses and is greater in certain patients.

Tell your doctor if any of these situations affect you:

- You consume large amounts of alcohol
- You have kidney problems
- You have thyroid problems
- You are older than 70 years
- You have had previous muscular problems during treatment with cholesterol-lowering drugs (statins and fibrates)
- You or a close relative have a hereditary muscle disorder.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

The effect of simvastatin may be increased or decreased by other medicines and vice versa. These drugs include:

It is especially important that you tell your doctor if you are taking any of these medicines. Taking simvastatin with any of these drugs may increase the risk of muscle problems:

- Cyclosporine (a medicine often used in organ transplant patients)
- Danazol (a synthetic hormone used to treat endometriosis)
- Medicines such as itraconazole or ketoconazole (medicines for fungal infections)
- Fibrates such as gemfibrozil and bezafibrate (medicines for lowering cholesterol)
- Erythromycin, clarithromycin, telithromycin or fusidic acid (medicines for bacterial infections)
- Protease inhibitors HIV, such as indinavir, nelfinavir, ritonavir y saquinavir (AIDS drugs)
- Nefazodone (a medicine for depression)
- Amiodarone (a medicine for irregular heart rhythm)
- Verapamil or diltiazem (medicines for high blood pressure, chest pain associated with heart disease or another heart disease)

In particular, also tell your doctor if you are taking any of these medicines:

- Drugs to prevent blood clots, as warfarin, phenprocoumon or acenocoumarol

- Fenofibrate
- Niacin.

Use with food and beverages

Grapefruit juice contains one or more components that alter how the body uses some drugs, including simvastatin. Consumption of grapefruit juice should be avoided.

Special physiological situations

Pregnancy and lactation

Do not take simvastatin if you are pregnant, if you are trying to get pregnant or think you may be pregnant. If you become pregnant while taking this medicine, stop taking it immediately and tell your doctor.

Children

This drug is not recommended for children.

Driving and using machines

How to use simvastatin ▲

Follow carefully instructions of use of simvastatin provided from your doctor. Consult your doctor or pharmacist when in doubt. Before you start taking this medicine, must be on a diet to lower cholesterol.

The usual dose is one tablet simvastatin once a day orally.

The higher doses only are recommended for patients with very high cholesterol levels and high risk for suffering heart diseases.

Your doctor will determine the appropriate dose for you, depending on your condition, your current treatment, and your personal risk status.

Take simvastatin in the evening. You can take it with or without food. The usual starting dose is 10, 20 or 40 mg per day. Your doctor may adjust your dose after at least 4 weeks, to a maximum of 80 mg daily. Do not take more than 80 mg per day.

Your doctor may prescribe lower doses (10 or 20 mg), especially if you are taking certain medications listed above or have certain kidney conditions.

Keep taking this medicine until your doctor tells you to stop.

If your doctor has prescribed simvastatin along with a bile acid sequestrant (medicines for lowering cholesterol), you should take simvastatin, at least, two hours before or four hours after taking the bile acid sequestrant.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take simvastatin ▲

If a doctor prescribes another medicine, tell you are taking simvastatin.

Check with your doctor regularly developments of disorder that motivates the use of simvastatin. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with simvastatin at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with simvastatin, immediately consult your doctor.

If you take more than you should:

If you take more simvastatin that you should, or if someone else has taken your medicine, consult your doctor or pharmacist immediately or call the National Poisons Information Service in your country. If you have taken a simvastatin overdose, go to the emergency department of the nearest hospital. Take the container with you, even if it is empty.

If you forget to take simvastatin

Do not take extra medicine to make up for a forgotten. Take your next doses as appropriate.

If you stop taking simvastatin

Your cholesterol level can increase again.

Side Effects of simvastatin ▲

Like all medicines, simvastatin can cause side effects although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Severe rare side effects

If any of these rare side effects occur, stop taking the medicine and tell your doctor immediately or go to the emergency room of the nearest hospital.

- Muscle pain, pressure sensitivity, weakness o muscle cramps. Rarely, these muscle problems can be serious, including muscular failure resulting kidney damage; and very rarely deaths have occurred.
- Hypersensitivity reactions (allergic) including swelling of the face, tongue and throat that may cause difficulty in breathing (angioedema), severe muscle pain usually in the shoulders and hips, rash with weakness of the muscles of the limbs and neck, pain or inflammation of the joints, inflammation of blood vessels, atypical hematomas, rashes e swelling, urticaria, skin sensitivity to sunlight, fever, hot flushes, difficulty in breathing and malaise and lupus-like syndrome (including rash, joint disorders, and effects on blood cells)
- Inflammation of the liver with yellowing of the skin and eyes (jaundice), itching, dark urine or pale colored stools, liver failure (very rare)
- Inflammation of the pancreas, often with severe abdominal pain.

Other rare side effects:

- Low red blood cell count (anemia)
- Numbness o weakness of the arms and legs
- Headache, tingling, Dizziness
- Digestive disorders (abdominal pain), constipation, flatulence, indigestion, diarrhea, nausea, vomiting)
- Rash, itching, hair loss
- Weakness.

Laboratory tests

Have been observed alterations in some liver function tests (increased transaminases) and muscle enzymes (increased CK, creatine kinase).
