Indications and usage     

Spironolactone is in a group of drugs called diuretics. These are drugs that increase the removal of body fluids.

This medicine is used to reduce high blood pressure, swelling due to the accumulation of fluid (edema) that cause certain diseases of the kidney, liver or heart and for the treatment of heart failure in combination with other drugs.

It is also used in the diagnosis and treatment of diseases caused by an abnormally high production of the natural hormone aldosterone.

Medizzine recommends consulting your doctor if you are unsure (or you do not know) why you have been given spironolactone.

Other ingredients of available commercial presentations:

Besides the active ingredient spironolactone, the other components are calcium sulfate dihydrate, gluten corn starch, polyvidone, peppermint flavor and magnesium stearate. The tablet coating contains hydroxypropyl methylcellulose, polyethylene glycol and Opaspray M-1-7111 B (white). Some specialties contain lactose.

Before taking the drug     

Do not use spironolactone

If you are allergic (hypersensitive) to spironolactone, to other similar medicines or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of the face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you have certain disorders of the kidney function (acute renal failure, a very impaired renal function, lack of arrival of urine to the bladder or anuria)
- If you have a high blood potassium level
- If you suffer from Addison's disease that produces a damage resulting in adrenocortical hormone deficiency.

Take special care with spironolactone

- If you have severe liver problems, including liver cirrhosis
- If you are diabetic
- If you have kidney or liver problems or if you are elderly, the use of spironolactone may alter blood levels of some electrolytes so your doctor will perform regular monitoring
- Spironolactone use together salt substitutes is not recommended because can cause severe poisoning.

During the treatment with this medicine, maybe your doctor may do periodic tests to control sodium levels in blood and thus predict the risk of adverse effects.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription

There are some medications which may interact with spironolactone:

- Drugs that may alter potassium levels in the blood (eg potassium-sparing diuretics)
- Medicines for pain and fever (eg Aspirin, antipyrine)
- Some anti-ulcer drug (eg carbenoxolone)
- Nonsteroidal anti-inflammatory drugs (NSAIDs)
- Drugs that may alter the balance of electrolytes in the body (eg ammonium chloride, cholestyramine)
- Certain anti-inflammatory drugs (corticosteroids)
- Other medicines that lower blood pressure (antihypertensives),
- Alcohol, barbiturates, narcotics
- Drugs used in emergency situations to raise blood pressure (eg norepinephrine)
- Medicines to regulate heartbeat (eg digoxin)
- Lithium

Interference with laboratory tests:

This medicine may interfere with the results of some diagnostic tests. If you must undergo any test to assess levels of digoxin tell your doctor that you are taking spironolactone as it can alter the results.

Use with food and beverages

Avoid drinking too much alcohol while taking this medication because it may potentiate the hypotensive effect of spironolactone.

Special physiological situations

Pregnancy and lactation:

Always consult your doctor or pharmacist before taking any medicine during pregnancy or breastfeeding.

You should not take spironolactone if you are pregnant, you think you might be pregnant or are trying to become pregnant. Women of childbearing age should take appropriate contraceptive measures.

Do not take this medicine if you are breastfeeding your child.

Driving and using machines

Spironolactone may cause drowsiness and dizziness, especially when starting treatment. If you notice such effects, you should consult your doctor before performing these activities.

Do not use any tools or machines if your ability to use them is affected by this medicine.

How to use spironolactone     

Follow carefully instructions of use of spironolactone provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

The usual dose of spironolactone is 50 to 100 mg daily.

In the case of heart failure, the usual starting dose is 25 mg per day.

If necessary, your doctor may increase or decrease the dose to achieve that you need. Your doctor will adapt the dose at intervals of 2 weeks or more. The maximum dose of this medication is 400 mg a day.

In children, the dose can be given if the child is able to swallow tablets.

Spironolactone tablets can be taken at any time of day with or without food. However, you should try to take your tablet at the same time every day.

Your doctor will decide the duration of treatment with this medicine.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take spironolactone     

If a doctor prescribes another medicine, tell you are taking spironolactone.

Check with your doctor regularly developments of disorder that motivates the use of spironolactone. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with spironolactone at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with spironolactone, immediately consult your doctor.

If you take more than you should:

If you have taken an overdose of spironolactone tell your doctor immediately or go to the emergency room of the nearest hospital. Take the pack and the rest of unconsumed medicine.

If you forget to take spironolactone

If you forget to take a dose, take the next scheduled dose at the correct time. Do not take a double dose to make up for missed doses.

Side Effects of spironolactone     

Like all medicines, spironolactone can cause side effects although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

The following side effects are important and if you suffer one of them require immediate action:

Sometimes, the patients have suffered electrolyte blood disturbances, which very rarely has resulted in a serious life-threatening illness (called hyperkalemia). If you have heart rhythm disturbances, tingling, muscular weakness, flaccid paralysis, and muscle spasms. If this occurs, stop taking this medicine and contact your doctor immediately

Common side effects:

- Malaise, fatigue, tiredness, headaches, erectile dysfunction, drowsiness
- excessive development of breasts in men (gynecomastia), changes in menstruation (irregularities, amenorrhea, postmenopausal bleeding)
- Diarrhea, nausea

Uncommon side effects:

- Skin disorders (rashes, spots), redness, urticaria, itching,
- Disturbances of blood electrolytes, as hypercalcemia (elevated blood calcium) or hyponatremia (decreased blood sodium),
- Cramps tingling (paraesthesia)
- Confusion
- Increased blood concentration of creatinine

Rare or very rare side effects:

- benign breast cysts chest pain
- Hair loss (alopecia) excessive hair growth (hypertrichosis)
- Dizziness incoordination of movements (ataxia)
- Changes in libido
- Acute renal failure
- Leucopenia (decreased number of white blood cells) ,   thrombocytopenia (decreased number of platelets in the blood)
- Disturbances of liver function
