Indications and usage

Sulpiride is indicated in the treatment of:

- Psychopathological disorders (neurosis, depression, neurotic somatizations).
- Functional psychological disorders.
- Psychosomatic syndromes.
- Senile dementia
- Gastrointestinal somatizations
- Vertigo.

Other ingredients of available commercial presentations:

50 mg capsules:

The other ingredients are lactose monohydrate, methylcellulose, talc, magnesium stearate. Gelatin capsules contain titanium dioxide (E171).

50 mg tablets:

The other ingredients are corn starch, microcrystalline cellulose, lactose, talc, magnesium stearate, colloidal silica and sodium starch glycolate.

200 mg tablets

The other ingredients are lactose monohydrate, potato starch gluten, methylcellulose, colloidal silica, talc and magnesium stearate.

Oral solution

The other ingredients are sodium cyclamate, hydroxyethyl cellulose; Methyl para-hydroxybenzoate (E-216); Propyl para-hydroxybenzoate (E218); Citric acid monohydrate; Hydrochloric acid; Sorbic acid; Lemon essence and purified water. The measuring cup (15 ml) = 75 mg sulpiride.

Injectable solution

The other ingredients are sulfuric acid, sodium chloride and water for injections. A 2 ml ampoule contains 100 mg of sulpiride.

Important information     

Capsules and tablet: This medicine contains lactose. If your doctor has told you have an intolerance to some sugars, consult him before taking it.

The oral solution contains methyl and propyl para-hydroxybenzoate, which may cause allergic reactions (possibly delayed).

Before taking the drug     

Do not use sulpiride

If you are allergic (hypersensitive) to sulpiride or any of the other ingredients of the medicinal product prescribed to you. If you think may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Nor you should take this medicine:

- If you have a prolactin-dependent tumor (a hormone that stimulates milk secretion), e.g. pituitary gland prolactinomas and breast cancer.
- If you have a type of tumor called pheochromocytoma.
- If you are taking a drug for Parkinson's disease called levodopa.
- If you have bradycardia (heart beats more slowly), if you have heart rhythm disturbances or another major heart disease.

Take special care with sulpiride

- If the drug is administered to elderly, since there is a risk of sedation and decrease in blood pressure upon standing (orthostatic hypotension).
- If the drug is given to children, since sulpiride has not been thoroughly investigated in this age group.
- If you have kidney failure because you probably need a dose adjustment.
- If you have or have had epilepsy, since the class of drugs to which it belongs sulpiride may promote the development of seizures.
- If you have Parkinson's disease, as sulpiride is contraindicated, apart from exceptional cases, in patients taking antiparkinsonian medication.
- If you have unexplained fever and muscle stiffness, as it could be a neuroleptic malignant syndrome, a potentially fatal complication characterized by high body temperature, which may cause muscle rigidity and autonomic nervous system dysfunction.

If you have an elevated body temperature from an undiagnosed illness, should be discontinued the sulpiride administration.

This medicine may increase the risk of serious heart rhythm disturbances.

Remember that your doctor has prescribed this medicine only for you. Never give it to someone else.

If in doubt, consult your doctor.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription.

Especially, tell your doctor or pharmacist if you are taking:

- Levodopa.
- antiarrhythmic agents such as quinidine, disopyramide, amiodarone y sotalol.
- Other medicines such as cisapride, thioridazine, intravenous erythromycin, vincamine IV, halofantrine, pentamidine, sparfloxacin.
- Medications that induce bradycardia (slowing of heart rate) such as diltiazem and verapamil, clonidine, guanfacine o digitalis.
- Medications that induce decreased levels of potassium and magnesium.
- CNS depressants including narcotics, anesthetics, painkillers, H1 antihistamines, sedatives, barbiturates, benzodiazepines and other anxiolytic drugs.
- lithium salts.
- Sucralfate.
- Antacids.
- drugs for high blood pressure.

Note that these warnings also include drugs which can be taken before or after sulpiride.

Use with food and beverages

Since alcohol enhances the sedative effect of sulpiride, it is not recommended drinking alcoholic beverages and other medications containing alcohol in its composition while taking this medicine.

Should not drink grapefruit juice during treatment with sulpiride.

Sulpiride should be taken preferably before meals.

Consult your doctor or pharmacist before taking any medicine.

Special physiological situations

Pregnancy and lactation

The use of this medicine is not recommended during pregnancy.

This medicine is excreted in breast milk, so breast-feeding is not recommended during treatment with sulpiride.

Use in children

There are not enough studies to support the use of sulpiride in children, so it is not recommended.

Use in elderly

The dosage of this medicine should be adapted to the conditions of the elderly patient. In any case, it should be used with caution.

Driving and using machines

Sulpiride may affect reaction time and, consequently, the ability to drive vehicles or operate machinery. So, it is recommended a special caution, checking previously if sulpiride affects your abilities for driving or operating machinery.

How to use sulpiride     

Follow carefully instructions of use of sulpiride provided by your doctor. Consult your doctor or pharmacist when in doubt. Follow the instructions of the physician in preference to those given in this website, which may be different. Ask your doctor to explain any aspect that you do not understand, contained in the instruction leaflet that comes with the medicine.

Your doctor will determine the appropriate dose and duration of treatment; Do not stop taking before or prolong more than prescribed by your doctor.

The duration of this treatment is limited.

Your doctor will indicate the duration of the treatment and how to increase the dose. Do not stop treatment abruptly since may reappear symptoms of your illness.

The dose is different depending on the illness you suffer:

As a rule, the usual dose is:

Adults:

The recommended dose is 3 to 6 capsules per day (50 mg). In vertigo, it can be increased up to 9 capsules per day. Divide the whole dose into three doses throughout the day, preferably administered prior to the meals.

If you use the oral solution: 2-4 dosing cups (150-300 mg) per day.

Sulpiride should be taken at least two hours before antacids and sucralfate.

Kidney failure:

In the case of renal impairment, dosage should be adapted to the kidney functional ability for removing. Your doctor will adjust the dose according to the following scheme:

- Creatinine clearance 30-60 ml/min: administer 50-70% usual dose
- Creatinine clearance 10-30 ml/min: administer 35-50% usual dose
- Creatinine clearance less than 10 ml/min: administer a maximum 35% usual dose.

Elderly:

Elderly patients may have higher sulpiride blood level than expected. Therefore, these patients need a lower starting dose and a more gradual titration than young adults.

If you think the effect of the medicine is too strong or too weak, consult your doctor.

While you take sulpiride     

If a doctor prescribes another medicine, tell you are taking sulpiride.

Check with your doctor regularly developments of disorder that motivates the use of sulpiride. Maybe there is some reason that prevented you properly receive the indicated doses and induce your doctor to erroneous conclusions about treatment.

Do not restart treatment with sulpiride at your own risk without first talking to your doctor, nor encourage its use by another person, even if have the same symptoms you have. Nor is it advisable to interrupt or reduce the dose without considering the opinion of your doctor.

If you feel unwell during treatment with sulpiride, immediately consult your doctor.

If you take more than you should:

In the case of overdose, spasms of the muscles of the face, neck and tongue may occur. Some patients may develop parkinsonian manifestations (tremor, stiffness) life-threatening and even coma.

There is no specific antidote to sulpiride; treatment is only symptomatic and hemodialysis is not effective in eliminating the drug. In the case of overdose, must be started appropriate support measures, recommending close monitoring of vital functions, controlling cardiac function until the patient recovers.

In the case of overdose or accidental ingestion, consult your doctor or pharmacist immediately or call the National Poisons Information Service in your country. If you have taken a sulpiride overdose, go to the emergency department of the nearest hospital. Take the container with you, even if it is empty.

If you forget to take sulpiride

Do not take a double dose to make up for a forgotten dose.

If you stop taking sulpiride

It is not recommended abrupt cessation of treatment with sulpiride unless your doctor tells you so.

Side Effects of sulpiride     

Like all medicines, sulpiride can cause side effects although not everybody gets them.

The frequency of side effects is classified into:

Very common: affect 1 out of 1-10 patients
Common: affect 1 out of 10-100 patients
Uncommon: affect 1 out of 100-1.000 patients
Rare: affect 1 out of 1.000-10.000 patients
Very Rare: affect less than 1 in 10,000 patients
Incidence not known: Can not be established the true incidence from the available data

Heart disorders:

- Low blood pressure upon standing.
- Very rare : problems in the conduction of the electrical impulse of the heart and torsade de pointes.

Hormonal disorders:

- Increased levels of prolactin.

General disorders:

- Like all neuroleptics, neuroleptic malignant syndrome, a potentially fatal complication.
- Weight gain.

Liver disorders:

- Very rare: increase in liver enzymes.

Nervous System Disorders:

- Sedation or drowsiness.
- Extrapyramidal symptoms and related disorders.
- Parkinsonism and related symptoms: tremor, increased muscle tone, slowness of movement, increased salivation
- slowness of movement and decreased muscle tone (spasmodic torticollis, decreased tone in the craniofacial region, pressure exerted on the teeth involuntarily).
- Inability to sit or to remain sitting (akathisia). These symptoms are usually reversible if administered antiparkinsonian medicines.
- Tardive dyskinesia (characterized by rhythmic, involuntary movements primarily of the tongue and/or face), as happens with all neuroleptics, after administration for more than 3 months. The antiparkinsonian medication is ineffective or may aggravate the symptoms.
- Very rare: seizures

Disorders of the reproductive system and breast. Disorders associated with increased prolactin levels:

- Abnormal secretion of breast milk (outside of pregnancy period).
- Ther absence of menstruation in a fertile woman (amenorrhea).
- Enlarged breasts in men (gynecomastia).
- Enlarged breasts in women.
- Breast pain.
- Orgasmic dysfunction.
- Impotence.

Skin and subcutaneous tissues disorders:

- Very rare: Maculopapular rashes (with spots and hives).
