Indications and usage     

Paroxetine is used for treating depression and/r anxiety disorders in adults.

Anxiety disorders for which paroxetine is indicated are:

Obsessive Compulsive Disorder (OCD) (obsessive and repetitive thoughts with uncontrolled behavior), anguish disorder (panic attacks, including those caused by agoraphobia, which is the fear of open spaces), social phobia (fear or avoid social contact situations), posttraumatic stress disorder (anxiety caused by a traumatic event) and Generalized Anxiety Disorder (GAD) (usually feeling a lot of anxiety and nervousness).

The active ingredient paroxetine belongs to a group of medicines called selective inhibitors of serotonin reuptake (SSRIs). Everyone has a substance in the brain called serotonin. People who are depressed or have anxiety have lower levels of this substance. It is not known precisely the mechanism of action of paroxetine and other SSRIs, but exert its action by increasing the level of serotonin in the brain.

Tablets:

Contain, besides the active ingredient paroxetine (20 mg) as hydrochloride hemihydrate: In the tablet core: calcium hydrogen phosphate dihydrate (E341), magnesium stearate (E470b) and sodium carboxymethylstarch (type A). In the tablet coating: hypromellose (E464), titanium dioxide (E171), macrogol 400 and polysorbate 80 (E433).

Oral drops (33 mg/ml):

The active substance is paroxetine mesylate. (1 ml of the solution contains 33.1 mg of active substance). A drop containing 1 mg paroxetine (as mesylate). The other ingredients (excipients) are: sodium saccharin (E954), acesulfame K (E950), peppermint flavor (peppermint essential oil, menthol, eucalyptol, ethanol, water), polysorbate 80 (E433), ethanol (114 mg / mL) and propylene glycol (E1520).

Oral solution (10 mg/ml):

The active substance is paroxetine hydrochloride. The other ingredients are: hydropropyl-B-cyclodextrin, sucrose, aroma anise (ethanol, anethole and water), sodium benzoate E211, 1N hydrochloric acid and purified water.

Each pack contains one bottle of 30 ml and 60 ml and a graduated eyedropper. Each ml of liquid (20 drops) contains 10 mg of paroxetine.

Additional warning     

Solution 33 mg/ml:

This medicine contains 16% by volume of alcohol. Take 20 drops of drug is equivalent to about 65 mg alcohol, that means, to consume 1.5 ml of beer or 0.5 ml wine.

This medicine is hazardous to those suffering from alcoholism. It is necessary to take this into account in pregnant or breast-feeding women, in children and high risk groups such as patients with liver disease or epilepsy.

Solution 10 mg/ml:

This medicine contains sucrose. If your doctor has told you that you have an intolerance to some sugars, contact him before taking this medicine.

This product contains 3.3% ethanol (3.3 ml ethanol per 100 ml of product) in anise aroma. Therefore, 1 ml of the solution contains the equivalent of less than 1 ml of wine or 0.3 ml of beer (6 ml is equivalent to 4 ml beer or 0.3 ml wine). This is harmful for those suffering from alcoholism.

Before taking the drug     

Do not use paroxetine

If you are allergic (hypersensitive) to the active substance paroxetine or any of the other ingredients of the medicinal product prescribed to you. If you think you may be allergic, ask your doctor.

Allergy symptoms may include:

- Asthma attacks with shortness of breath, audible wheezing or rapid breathing.
- More or less sudden swelling of face, lips, tongue or elsewhere in the body. It is especially critical if it affects the vocal cords.
- Hives, itching, rash
- Anaphylactic shock (loss of consciousness, paleness, sweating, etc.).

Also, you should not take this medicine:

Take special care with paroxetine

- If you are taking other medicines
- If you have any problems of kidney, liver or heart.
- If you suffer epilepsy or if you have seizures.
- If you have had manic episodes (behavior or thoughts overactive).
- If you are being treated with electroconvulsive therapy (ECT).
- If you are prone to bleeding or bruising, or being treated with a medicine that can increase the risk of bleeding (including medicines such as warfarin or acenocoumarol, antipsychotics such as perphenazine or clozapine, tricyclic antidepressants, drugs to treat pain and inflammation called NSAIDs, such as aspirin, ibuprofen, celecoxib, etodolac, diclofenac or meloxicam).
- If you have diabetes.
- If you are on a low sodium diet.
- If you have glaucoma (increased eye pressure).
- If you are pregnant or planning to become pregnant.
- If you are under 18 years of age.

If you are in any of these situations and still have not consulted your doctor, ask him about taking paroxetine.

Children and adolescents under 18 years of age:

Paroxetine should not be used in the treatment of children and adolescents under 18. Some patients taking paroxetine develop a condition called akathisia, and feel restless and unable to sit or stand still.

Other patients develop the so-called "serotonin syndrome", and may have some or all of the following symptoms: confusion, restlessness, sweating, shaking, shivering, hallucinations (strange visions or sounds), sudden jerky movements or increased heart rate.

Taking other medicines

Tell your doctor or pharmacist if you are taking or have recently taken any other medicines, including herbal products or drugs obtained without a prescription. It is important, especially if you are taking:

- Medicines called monoamine oxidase inhibitors (MAOIs, including moclobemide).
- Drugs called thioridazine, or pimozide, which are anti-psychotics.
- Aspirin, ibuprofen and other drugs called NSAIDs, celecoxib, etodolac, diclofenac and meloxicam, used to treat pain and inflammation

- A dietary supplement called tryptophan
- Medications such as lithium, risperidone, perphenazine, clozapine (antipsychotics) used to treat some psychiatric conditions

Lactation

Paroxetine passes into breast milk in very small amounts. Consult your doctor if you are taking paroxetine before starting to breastfeed. Your doctor, according to you, could advise maintaining breastfeeding while taking the drug.

Driving and using machines

Some of the side effects that can occur while are taking paroxetine include dizziness, confusion, drowsiness or blurred vision. If you experience any of these effects, do not drive or use machines.

How to use paroxetine

Social phobia:

- Starting dose: 20 mg
- Recommended dose: 20 mg
- Maximun dose: 50 mg.

Generalized Anxiety Disorder:

- Starting dose: 20 mg
- Recommended dose: 20 mg
- Maximun dose: 50 mg.

Your doctor will tell you what dosage should be taken when starting treatment with paroxetine. Most patients start to feel better after a couple of weeks. If after this time does not begin to feel better, consult your doctor who will tell you what to do.

Your doctor may decide to increase the dose gradually, in steps of 10 mg up to the maximum daily dose.

Take the tablets in the morning with food. The tablets should be swallowed whole with water and should not masticarse.Su doctor will indicate the duration of treatment. This period may last several months or even longer.

Elderly:

The maximum dose for people over 65 is 40 mg a day.

Patients with kidney or liver disorders:

If you feel unwell during treatment with paroxetine, immediately consult your doctor.

If you use more medicine than you should

Never take more tablets than recommended by your doctor. If you, or someone else, take more Paroxetine than you should, immediately consult your doctor or pharmacist, call to the Poison Information Center in your country or go to the emergency room of the nearest hospital, carrying the package with the rest of medicine not consumed.

A patient who has taken an overdose of paroxetine may have some of the side effects mentioned below or any of the following symptoms:

Vomiting, enlarged pupils, fever, headache, involuntary muscle contraction.

If you forget a dose:

Take your medicine at the same time each day. If you miss a dose, and you remember before going to bed, take it immediately. Take the next dose at the usual time.

If you remember during the night or the next day, skip the missed dose. You may have any symptoms of withdrawal, but should disappear after the next dose at the usual time.

What to do if you do not feel better

Paroxetine will not improve your symptoms immediately; usually, all antidepressants take time to act. Some people start to feel better after a few weeks, but others may take more. Some people taking antidepressants feel worse before you start to notice improvement.

If you do not begin to feel better after two weeks, tell your doctor, who will tell you what to do. It is possible that the doctor will quote again after two weeks to check your evolution.

If you stop taking paroxetine

Do not stop treatment until your doctor tells you to. When you need stop taking paroxetine, your doctor will tell you how to reduce the dose slowly over a period of several weeks or months, this will help reduce the risk of withdrawal symptoms.

One way of doing this is to gradually reduce the dose of paroxetine that you are taking in 10 mg each week.

Most patients consider that the possible symptoms occuring when treatment is discontinued are mild. These disappear on its own within two weeks. For others patients, these symptoms may be more severe or last longer.

If you have withdrawal symptoms while is decreasing progressively treatment, the doctor may decide to reduce the speed of decrease.

Possible withdrawal effects when interrupt the treatment

- Feeling dizzy, instability or deterioration in the balance.

- Tremor
- Confusion or disorientation
- Diarrhea (loose stools)
- Feeling very sensitive or irritable
- Visual disturbances
- Fast or pounding heart palpitations

Consult your doctor if you are concerned about the effects due to the withdrawal of paroxetine.

Side Effects of paroxetine     

Like all medicines, paroxetine can cause side effects, although not everybody gets them.

It is more likely that adverse effects occur during the first weeks of treatment with paroxetine.

Uncommon side effects (affect less than 1 patient in 100):

- If you have unusual bruising or bleeding, such as blood in vomit or stools, contact your doctor or go to hospital immediately.
- If you have inability to urinate, contact your doctor or go to hospital immediately.

Rare side effects (affect less than 1 in 1000 patients):

- If you experience convulsions (seizures), contact your doctor or go to the nearest hospital immediately.
- If you have feelings of restlessness, inability to sit or stand still, called akathisia. Increasing the dose of paroxetine can worsen your symptoms. If you feel worse, see your doctor..

Very rare side effects (affect less than 1 in 10.0000 patients):

- Disorders in the liver that produce a yellow colour in skin and eyes (jaundice)
- Water or liquid retention causing swelling of the arms or legs
- Sensitivity to sunlight
- Prolonged and painful erection of the penis (priapism)
- Decreased number of blood platelets

Some patients have experienced ringing, hissing, whistling, ringing or other persistent noise in the ears (tinnitus) when they take paroxetine.
